These prescription, custom-made trays require an evaluation, a treatment plan, and consistent wear; a condition on a common-use list is not proof that aligners alone will work for you.
What determines whether clear aligners are right for you?
A licensed dentist or orthodontist needs to assess your teeth, bite, dental history, and treatment goals. The examination may include X-rays; scans or impressions help plan custom trays. Ask the practitioner to explain which findings support the recommendation and what the plan is expected to change.
The FDA describes aligners as prescription devices used under a licensed practitioner’s supervision. The American Association of Orthodontists (AAO) lists crowded or gapped teeth, crooked teeth, overbite, underbite, open bite, and crossbite among conditions aligners may treat. Those are examples of common uses, not a universal eligibility checklist or a promise that aligners alone can correct a particular bite.
Mild-to-moderate alignment issues may be treatable with aligners. More complex conditions may call for braces, another treatment, or a combined approach; some movements can be more predictable with braces. What treatment involves—and what it asks of you
- Evaluation and planning: A practitioner assesses your mouth, reviews relevant dental history, and determines whether aligners are suitable. Ask who evaluates and approves the plan.
- Records and custom trays: Scans or impressions are used to plan a series of trays made for your teeth.
- Daily wear and tray changes: The FDA says patients typically wear each tray for one to two weeks and generally wear aligners at least 20 hours per day, or as directed by their practitioner. Follow your own prescribed schedule.
- Progress checks: The practitioner monitors treatment. Ask how often you need visits, whether they will be in person, and whom to contact if a tray does not fit or you have symptoms.
- Retention: A retainer may be recommended after active treatment. Ask what your retainer plan involves.
The FDA’s consumer comparison gives a broad possible treatment duration of six to 18 months. That range is not a forecast for an individual; the time depends on the movements needed and your treatment plan. (FDA: Braces Have Changed)

Neither option is established as universally faster, safer, or better. Compare the proposed movements, adherence demands, appearance, cleaning and eating routines, follow-up, expected duration and cost, refinements, retention, and what happens if treatment does not progress as planned. Risks, supervision, and mail-order aligners
Aligner treatment may not progress as intended if you do not wear trays as prescribed. Trays can be lost or damaged; some patients experience temporary speech changes, irritation, or allergic reactions. Existing crowns, fillings, or implants may loosen or be dislodged and need repair or replacement. The FDA also lists adverse events such as swelling and cuts.
The FDA advises an evaluation, preferably in person, and periodic monitoring. It says insufficient supervision can increase the chance of bite problems, unintended tooth movement, pain, and tooth loss. The agency has received reports involving unauthorized direct-to-consumer aligners of new or worsening dental pain, jaw pain, bite problems, worsening misalignment, and new gaps. Those reports do not establish how often such problems occur. Contact your dentist or orthodontist if pain or bite changes develop or worsen.

FDA guidance states: “The FDA has not authorized clear aligners that are sold directly to consumers without a prescription and supervision by a dentist or orthodontist.” This is about treatment lacking practitioner prescription and supervision; it is not a blanket claim about every virtual-care arrangement. The FDA recognizes that virtual visits may be useful in some care plans, while noting that periodic in-person visits can help reduce risks and check progress.
“FDA registered” is not the same as FDA authorization of a specific product. Ask which product is being used and whether it is FDA-authorized; the FDA says consumers can search its 510(k) database using product code NXC. The ADA separately describes direct-to-consumer dental laboratory services as removing the dentist’s role in diagnosis, treatment planning, and management; that is the association’s policy position, distinct from FDA regulatory guidance. Questions to ask at a consultation
- Based on my teeth, bite, dental history, and goals, am I a candidate for aligners alone? What findings support that recommendation?
- Which tooth and bite movements are realistic, and what may remain untreated?
- Would braces, a combination approach, or no treatment be reasonable alternatives? Why?
- What examination and records do you need, and who reviews and approves the treatment plan?
- How many hours a day should I wear each tray, when should I change trays, and how will progress be checked?
- How often should I have in-person visits, and whom do I contact if a tray does not fit or I have pain or bite changes?
- What are the likely duration and total cost, including refinements, retainers, and possible correction or replacement?
- Which aligner product will be used, and how can I confirm its FDA authorization?
