Amazon Pharmacy launched in 2020. Here’s how the service works now

Amazon Pharmacy began in 2020 as an online prescription service with Prime delivery and savings. Here’s how it works, what it costs, and when a local pharmacy may be a better fit.

Amazon launched Amazon Pharmacy in the United States on November 17, 2020, bringing prescription ordering, insurance and cash-price comparisons, and home delivery into Amazon’s website and app. It was a licensed pharmacy service—not a marketplace for buying prescription drugs without authorization—and it did not replace every role of a local pharmacy. As of August 16, 2026, Amazon Pharmacy remains available for eligible prescriptions at physical addresses in all 50 states, with delivery speed, medication eligibility, and cost varying by location and prescription.

What Amazon launched in November 2020

Amazon introduced two related offerings: Amazon Pharmacy, an online prescription-filling service, and a Prime prescription-savings benefit for eligible prescriptions paid for without insurance. Customers could use Amazon’s website or app to manage prescriptions, enter insurance information, choose a payment option, and arrange delivery. A prescription from a licensed healthcare provider was still required, and prescriptions were reviewed and dispensed through the pharmacy.

At launch, Amazon said it accepted most insurance plans. That did not mean every plan treated Amazon as an in-network pharmacy, or that using insurance would always produce the lowest price. Customers could compare insurance costs with cash prices and, where eligible, Prime savings.

Amazon also said it would not deliver Schedule II controlled substances, including most opioids, through the service at launch. That is a statement about the 2020 launch; medication eligibility can vary, so customers should check the current pharmacy listing rather than assume a drug is deliverable.

Amazon Pharmacy and PillPack serve different needs

Amazon had acquired PillPack in 2018 for approximately $753 million. PillPack was already serving people who took multiple medications and wanted doses organized. Amazon described it as a distinct service rather than simply renaming it Amazon Pharmacy.

The current Amazon Pharmacy how-it-works page describes both services. Packet sorting is a different proposition from ordering ordinary prescriptions for delivery; not every medication or patient will be suited to it.

How to get a prescription filled

  1. Sign in and start an order. Use Amazon Pharmacy on the website or in the app, then search for the medication or begin the enrollment process.
  2. Add insurance details if you plan to use insurance. The pharmacy can show available payment options, but network status and plan rules still matter.
  3. Send or transfer the prescription. Ask the prescriber to send a new prescription to Amazon Pharmacy, or provide the medication name and current pharmacy information so Amazon can request a transfer. The transfer instructions describe the information Amazon needs.
  4. Compare the available prices and delivery estimate. Check the insurance copay, cash price, eligible savings or coupon, and estimated arrival for the specific medication and quantity.
  5. Wait for pharmacist review. Amazon says a pharmacist reviews and verifies prescriptions before shipment; a clarification or other check can affect timing. Pharmacists are listed as available 24/7 on Amazon’s how-it-works page.
  6. Track the shipment. Once the prescription is cleared and dispatched, use the order information to follow delivery.

Amazon says prescriptions arrive in discreet packaging. Its delivery information also notes that some medications need specialized handling.

What delivery looked like at launch—and what it looks like now

In November 2020, Amazon promoted unlimited free two-day delivery for Prime members ordering through Amazon Pharmacy. That was a launch-era offer, not a guarantee that every prescription would arrive in two days or that the same terms apply now. Timing depends on the prescription and fulfillment process.

As of August 16, 2026, Amazon advertises same-day delivery in many locations, but only for eligible medications and ZIP codes. The current delivery page says a first order of new medications may qualify for free same-day delivery; Prime members may pay $2.99 for same-day refill delivery where offered. Outside same-day areas, Amazon says delivery typically takes one to four days. Special packaging, medication checks, or lack of Sunday delivery in some locations can affect the options shown.

Amazon has announced a target to expand same-day prescription delivery to nearly 4,500 U.S. cities and towns by the end of 2026. That is a company target, not confirmation that the expansion has been completed: see Amazon’s announcement for its stated plan.

Amazon says it delivers to eligible physical addresses in all 50 states, but not to P.O. boxes, APO/FPO/DPO addresses, Amazon Lockers, or other pickup points. Statewide reach does not mean that every medication or delivery speed is available at every address.

How to compare the cost

The financial question is not simply whether Amazon accepts insurance or advertises a discount. Compare the cost for the same drug, strength, quantity, and refill schedule; a plan’s copay, deductible, formulary, and pharmacy-network rules can change which option is cheapest.

At launch, Amazon advertised savings of up to 80% on generic drugs and up to 40% on brand-name drugs for eligible customers paying without insurance. Those were maximum claims, not promised prices for every medicine or customer. Amazon’s current pricing page describes average savings of 70% for all generics and 37% for selected brand medications, based on average savings using Inside Rx data, with restrictions. These figures are not a substitute for checking the price of a particular prescription.

The pricing page says customers can compare an insurance copay with the lowest available cash price, including eligible Prime savings, and lists manufacturer coupons among the available pricing tools. The option that works best depends on the drug and the customer’s plan. A discount-card price may not be used together with insurance, so check the displayed terms before choosing.

RxPass is a later subscription, not part of the launch

RxPass was introduced after Amazon Pharmacy launched. As described by Amazon as of August 16, 2026, it costs $5 per month for Prime members and covers more than 50 eligible generic medications, regardless of how many eligible prescriptions a subscriber has. It does not use insurance for covered medications and is not insurance. Details and eligibility are on the RxPass page.

  • Only medications on the eligible list are covered; the subscription does not cover every prescription.
  • Amazon’s current terms say RxPass is unavailable for shipments to California and Washington, even though Amazon Pharmacy itself serves eligible addresses in those states.
  • Medicaid and CHIP beneficiaries are excluded under the current eligibility information; Medicare beneficiaries may qualify under the program’s rules.
  • State and insurance-related restrictions apply, and program terms can change. Confirm eligibility and the covered-drug list before subscribing.

RxPass can be worth considering only when the membership and subscription requirements fit the household and the eligible medicines would otherwise cost more than the subscription. Compare the actual alternatives rather than treating the monthly fee as a general prescription price.

When delivery may not be the right choice

Home delivery can suit routine prescriptions, but it is not ideal for every clinical or logistical situation. Amazon itself advises customers to use their current pharmacy for urgent needs, such as antibiotics or a medication that will run out within a week; see its how-it-works guidance.

  • You need medicine immediately. A transfer, pharmacist review, or delivery delay can leave a gap. Use a pharmacy that can meet the timing you need.
  • The medication requires special handling. Refrigerated, controlled, or otherwise specially managed medications may have additional eligibility and shipping requirements. Confirm availability and handling before relying on delivery.
  • Your plan directs you elsewhere. An insurer may require a particular retail, specialty, or mail-order pharmacy, or may charge differently across pharmacies.
  • You need in-person help. A local pharmacy can be more practical for face-to-face counseling, injection instruction, vaccines, or fast resolution of a prescription problem.
  • A missed shipment could interrupt treatment. Build in time for transfers and refills, and avoid relying on an estimated delivery date for a time-critical supply.

A pharmacist review is an important part of dispensing, but an online ordering interface is not a substitute for medical care. Ask a pharmacist or prescriber about clinical questions, interactions, or whether a medication is appropriate.

What the launch meant for pharmacy competition

Amazon entered prescription fulfillment after buying PillPack, putting its digital storefront and delivery network in competition with chain pharmacies, supermarket pharmacies, mail-order providers, and discount services. Contemporary TechCrunch coverage of the launch discussed the competitive context, including PillPack and prescription discount services.

Amazon’s model offered an account-based checkout, delivery, online price comparisons, and access to pharmacists. The trade-off is that customers may need to transfer a prescription and depend on delivery; they do not get the same walk-in access as at a neighborhood pharmacy. Nor does an online price comparison establish that Amazon is cheaper than CVS, Walgreens, Walmart, or an independent pharmacy for every drug. Actual prices depend on the prescription, plan, location, and discount eligibility.

Amazon said at launch that it would handle pharmacy information in compliance with HIPAA and would not use protected health information for advertising or marketing outside the pharmacy without clear permission. That is Amazon’s stated policy, not an independent privacy audit.

How the service has changed since 2020

The original story was free two-day delivery for Prime members and a prescription-savings benefit. By August 16, 2026, Amazon’s service also included location-dependent same-day delivery, RxPass for eligible generics, insurance and cash-price comparisons, coupons, automatic refills, and the separate PillPack service. Current delivery estimates and prices are shown by prescription and location, so launch-era terms should not be treated as current terms.

For routine prescriptions that do not need immediate pickup, Amazon Pharmacy may offer a convenient way to manage orders and compare costs. For an urgent fill, a medication with special requirements, or a situation where in-person support matters, a local pharmacy may be the more practical choice.
