Use your employer’s required form and approval process; there is no single universal format. Use this basic table to record the trip. The core fields reflect the Jharkhand State Livelihood Promotion Society (JSLPS) HR Manual’s published tour diary format. The follow-up and claim-reference column is optional unless your organization requires it.

JSLPS’s format also instructs employees to enclose the diary with the travel claim form. That is a JSLPS-specific instruction, not a general requirement. Check your own employer’s form and claim rules before submitting. JSLPS HR Manual — Tour Diary Format.
What to include in each entry
- Date and time: Record when the travel or activity took place, using the level of detail your employer asks for.
- Route or destination: Note where you started and the place visited. Add a host or meeting detail if it helps identify the business activity.
- Purpose: State why the visit was work-related, such as a meeting, inspection, training, or field visit.
- Activity summary: Briefly record what you did. Keep it factual and specific enough for a reviewer or colleague to understand the work performed.
- Supporting information: Add a claim reference, receipt, or attachment only where policy or the approved form calls for it.
When a tour diary needs a narrative report
A movement log may be enough for travel accounting. A report intended to share learning or support a decision needs more context: what you observed, what resulted, and what should happen next. The distinction is practical, not universal; employers set their own requirements.

For example, the Samtse College of Education’s official staff form asks for the tour’s purpose and places and dates visited, plus a description highlighting learning and observations, possible effects on professional practice, and follow-up actions. Its submission instruction connects the form to that college’s travel-allowance claim process. These details describe that institution’s procedure only. Structure for a fuller trip report
- Heading: Employee name and role, organization, report date, trip dates, and recipients.
- Purpose: Explain why the trip or visit took place.
- Itinerary: List dates, times, destinations, and relevant host or meeting details.
- Activities and observations: Describe meetings, visits, or work completed. Separate directly observed facts from interpretation.
- Outcomes: State what was learned, found, decided, or completed.
- Application or recommendations: Explain how findings may affect work or what action you recommend, when the assignment calls for it.
- Follow-up: Identify the action, responsible person, and due date when known.
- Approval and attachments: Include signatures, receipts, claim references, or supporting records required by local policy.

Submission and recordkeeping depend on employer policy
Ask which form to use, who approves it, whether it must accompany a travel claim, and what supporting records are required. The examples above do not establish a universal submission deadline or record-retention period. USDA APHIS, for instance, sets documentation and timekeeping procedures for its own employees, including paper or electronic logsheets; that agency guidance should not be treated as a rule for other employers.
