Yes—Lilly lists self-pay Zepbound vial prices starting at $299 per month for the 2.5 mg dose, but the vial does not come with the syringe and needle needed for each injection. The key update is that buying your own syringe is no longer the only self-pay device choice: Lilly also offers a KwikPen at the same dose-specific self-pay price. Prices and availability are subject to program terms.
What does the cheaper Zepbound price include?
LillyDirect currently lists self-pay vial prices of $299 per month for 2.5 mg, $399 for 5 mg, and $449 for each dose from 7.5 mg through 15 mg. The monthly vial prescription contains four single-dose vials, one for each once-weekly injection. These are company-listed prices; terms apply, and your actual cost depends on the prescribed dose and the current program terms. LillyDirect’s Zepbound page is the source for the current displayed price.
The vial medicine is self-pay only and available exclusively through LillyDirect, which lists home delivery and retail pickup options. Lilly’s December 2025 announcement described the same price tiers, but the live LillyDirect listing is the better place to check current prices and terms.
Do you need to buy syringes for Zepbound?
If your prescription is for the vial presentation, yes: Lilly’s vial instructions say the syringe and needle are supplied separately, and a new syringe and needle are required for each injection. Do not reuse them.

Lilly’s vial Instructions for Use list a vial, a separately supplied syringe and needle, an alcohol swab, gauze, and a sharps container. The instructions give a 1 mL syringe as an example if recommended by your healthcare provider. The prescribing information says the syringe must be suitable for the prescribed dose, giving a 1 mL syringe capable of measuring 0.5 mL or 0.6 mL as examples. That is a supply checklist, not a dosing guide: ask your prescriber which equipment is appropriate for your prescription and follow the instructions for your exact presentation. How do the vial, KwikPen and single-dose pen compare?
Lilly’s three presentations differ in handling and how the medicine is supplied. The vial and KwikPen have the same dose-specific LillyDirect self-pay price, according to Lilly’s 2026 announcement; that means the vial is not automatically the cheaper option once device choice is considered.

Which option is likely to cost less?
For self-pay customers comparing vial and KwikPen, Lilly’s announced price is the same for a given dose. The vial therefore does not offer a lower listed medicine price than the KwikPen in that self-pay program; it instead requires separately obtained injection supplies. The amount you spend overall can still depend on supply costs, fulfillment, and current terms.

For someone using insurance, a single-dose pen may be a better-value route if the plan covers it and the person qualifies for Lilly’s savings-card terms. Coverage, eligibility, and final out-of-pocket cost are individual, so compare the amount your pharmacy or plan quotes for your exact prescription rather than assuming one format is cheapest.

The formats have different instructions and require the matching device-specific training. Do not convert doses, substitute one presentation for another, or rely on vial instructions when using a pen. Follow the instructions for the prescribed presentation and your clinician’s directions.
