The Civil Service Commission’s Work Experience Sheet (WES) accompanies the Personal Data Sheet (PDS) when you apply for a vacant government position. It describes the actual duties you performed in relevant jobs already listed in your PDS, helping the hiring office assess your experience against that vacancy’s qualifications. For the current form and submission requirements, use the instructions in the specific vacancy announcement—not an older form simply because it remains available online.
What is the CSC Work Experience Sheet for?
The WES gives you space to explain the actual duties connected with relevant work entries in your PDS. The CSC adopted the WES alongside the Revised 2017 PDS in Resolution No. 1700656, describing its role in helping CSC offices evaluate an applicant’s qualifications for an open position. The WES supplements the PDS; it does not replace it or, by itself, prove that you meet a vacancy’s requirements. CSC Resolution No. 1700656
Do you need to submit the WES with your PDS?
For an application to a vacant government position, CSC Memorandum Circular No. 16, s. 2017 says to complete and submit the WES with the PDS. The circular also clarifies that the WES is not required merely to update a PDS. CSC Memorandum Circular No. 16, s. 2017
Which CS Form No. 212 version should you use?
Check the current vacancy notice and the official CSC download before filling out a form. The 2017 CSC materials explain the WES’s origin and purpose, but do not establish that the 2017 PDS files remain the current editions. In vacancy announcements published in 2026, CSC Region XI and CSC Caraga referred to the PDS as CS Form No. 212 Revised 2025 and continued to request a WES. Those notices document recent naming and practice; they were for specific vacancies and are not current job openings or a guarantee that every agency uses the same process. Download the WES from the official CSC forms page or follow the link in the vacancy announcement. Compare the form’s revision with the version requested in the notice; do not assume an older standalone download is current just because it appears on a CSC page. Current form availability and agency instructions can change.
Where can you download the CS Form 212 Work Experience Sheet?
Start at the CSC ORAOHRA downloads page or use the form linked by the recruiting agency’s live vacancy announcement. The CSC page lists the 2017 PDS and WES materials, so verify the revision rather than treating that listing as proof that those files are current. If the hiring office supplies a specific form or upload link, use that version and follow its directions.

How should you complete and submit it?
- Read the vacancy announcement. Note the required PDS and WES revisions, eligibility and experience requirements, submission channel, file format, supporting documents, and deadline.
- Choose relevant positions from your PDS. Use the WES to explain duties for work entries already declared in the PDS, focusing on details that help the hiring office assess experience for that particular role.
- Describe your actual duties accurately. Keep job details and dates consistent with the PDS. Do not overstate responsibilities or claim experience you cannot support.
- Gather evidence the notice asks for. Requirements vary by vacancy. For example, a CSC-hosted DENR Caraga vacancy posting dated 28 March 2025 requested a service record, special orders, and/or certificate of employment in relation to experience claimed in the WES/PDS. - Submit exactly as instructed. Some vacancies use online applications and require a complete PDF; others may specify different documents or procedures. A CSC Region XI notice in 2026, for example, specified online application and one complete PDF. Follow the live notice for your vacancy, including any file-size limit and deadline.
Why accuracy matters
The CSC’s Resolution No. 1700656 warns: “Any misrepresentation made in the Personal Data Sheet (CS Form No. 212, Revised 2017) and the Work Experience Sheet shall cause the filing of administrative/criminal case/s against the person concerned.” Provide truthful information in both documents and ensure that your WES account of duties matches the relevant PDS entries.
