The ₹4,000 figure in the headline was a tout’s reported offer in a 2016 investigation near AIIMS in Delhi—not a current price, a verified transaction or a measure of how widespread illegal blood sales are. India’s official system is built around licensed blood centres and voluntary, unpaid donation. Government reviews published since then describe continuing gaps in access and service delivery, but do not establish that the specific practices reported in 2016 remain widespread.
What the ₹4,000 figure actually refers to
On 5 December 2016, Scroll.in and IndiaSpend published an investigation titled “Blood for Rs 4,000 a unit: Inside India’s blood black market.” Its search extract describes an undercover encounter near AIIMS in Delhi and quotes a tout saying, “Don’t worry, you’ll get the blood – Rs 4,000 a unit.” The figure is therefore an offer reported by that investigation at that time and place.
It should not be read as a present-day quote, an official tariff, proof that blood was supplied for that amount, or an estimate of the scale of illegal sales across India. The reported encounter is a historical case study, not a current market survey.
How India’s regulated blood system is meant to work
Unpaid donation, not professional paid donors
The Directorate General of Health Services describes the 1996 Supreme Court-led reform as resulting in a ban on professional blood donation. The National Blood Policy, formulated in 2002, promotes voluntary, non-remunerated donation through a coordinated transfusion service. This is the official model; it does not mean that every district has equal access to services or that implementation is uniform.

Licensed centres and screening
Official guidance says every blood donation must be screened for HIV/AIDS, Hepatitis B, Hepatitis C, Syphilis and Malaria. The wider service goals include safe and sufficient supply, access, quality management, preparation of blood components and coordination across regions. Screening is a core safeguard, but it does not by itself establish that a centre is licensed or that every service standard is being met.
National policy, state implementation
The national system includes the National Blood Transfusion Council (NBTC), State Blood Transfusion Councils and central and state regulators. In a 2 August 2022 parliamentary reply, the Ministry of Health and Family Welfare said State and Union Territory governments have primary responsibility for concrete action against illegal activity in blood banks and during transfusion, and for implementing and monitoring the Act and rules. The Ministry also described e-RaktKosh for blood-centre data management and a hub-and-spoke service approach.

What the published figures say—and what they do not
The available official figures refer to different measures and dates. They cannot be combined into a single estimate of supply, unmet need or black-market activity.

The national review also assessed service performance against 10 key performance indicators, drawing on eRaktKosh, CDSCO, BBMS and regulatory inspections. The annual requirement, a centre count and the share of districts without a centre are not interchangeable indicators. None, by itself, shows how many people obtain blood through illegal channels or whether the 2016 offer reflected a broader pattern.

What the April 2026 review identified
At its national review on 22 April 2026, the Ministry reported continuing variation in district-level centre availability, licensing compliance, voluntary donation, follow-up for donors with reactive test results, component-separation capacity and real-time digital reporting. It set a goal of having at least one blood centre in every district by December 2026. That is a stated target, not confirmation that the target has been met.
These findings describe system-level challenges as of the review date. They provide useful context about access and implementation, but they do not verify that the reported 2016 touting practice is still occurring or quantify illegal trade.

What to do if someone needs blood
For an actual transfusion need, use the treating hospital’s blood-service process and a licensed blood centre rather than an informal seller. Ask the hospital team which component and quantity are needed, which centre is supplying it, and what documentation or arrangements the hospital requires. Do not treat an online or in-person tout’s quote as a safe or legitimate way to obtain blood: an offer does not establish licensing, screening or traceability.
If you suspect illegal activity at a blood bank or during transfusion, the Ministry’s 2022 reply places implementation and monitoring responsibilities with the relevant State or Union Territory government. The responsible authorities and practical complaint channels can vary by location; contact the state or UT health authorities or the treating institution to identify the appropriate reporting route.
