If you are applying for a Philippine government position, check the vacancy announcement or ask the receiving agency which version of CS Form 212 to submit. As of October 8, 2026, the Civil Service Commission (CSC) downloads page lists a Revised 2026 Personal Data Sheet (PDS) and guide, while the CSC general forms index still lists the Revised 2025 form. The pages do not establish a universal transition date. Download the version the agency requires from the CSC downloads page.
Which CS Form 212 version should you use?
Use the version named in the job vacancy announcement or confirmed by the government office receiving your application. The CSC pages are inconsistent: the CSC downloads category lists “CSC Form 212 (Revised 2026) – Personal Data Sheet” and a Revised 2026 guide, while the CSC general forms index still lists “Personal Data Sheet (PDS) – (CSC Form 212 Revised 2025).”
The CSC circular downloads page also lists MC No. 05, s. 2026, titled “Adoption of CS Form No. 212, Revised 2026 (Personal Data Sheet).” The listing identifies an adoption issuance, but does not by itself establish an effective date or a universal deadline for switching forms. Do not assume the Revised 2025 form is invalid everywhere or that every agency has the same submission rule.
Check these details before completing the form
- The revision named in the vacancy announcement or application instructions.
- Whether the agency requires a paper or electronic submission and any specified attachments.
- Any agency-specific rules for signatures, photographs, or formatting.
- That the form and guide you download are the versions the agency told you to use.
If the announcement does not say, contact the receiving agency. The CSC downloads page is the official place to check for the currently posted form and guide.

What information is in the Revised 2025 PDS?
The Revised 2025 CS Form 212 is the CSC Personal Data Sheet. Its first page covers personal information, family background, and educational background; the complete form includes additional pages and references a Work Experience Sheet. Do not assume that the 2026 form has the same fields or instructions: the available CSC listing identifies the newer form and guide but does not provide enough information here to verify field-by-field changes.
The 2025 educational section asks for information such as the school and degree or course, attendance period, units or highest level completed if the applicant did not graduate, graduation year, and scholarship or academic honors. Follow the labels and instructions in the specific form version you were told to complete.

How to complete the Revised 2025 form
The CSC form directs applicants to read its attached guide before filling out the PDS. Its instructions say: “Print legibly if accomplished through own handwriting. Tick appropriate boxes [ ] and use separate sheet if necessary. Indicate N/A if not applicable. DO NOT ABBREVIATE.” The form also directs applicants to write names and other entries in full where specified.
- Get the correct file and guide. Confirm the required revision with the agency, then download that version and its accompanying guide from the CSC downloads page.
- Read the guide before entering information. The Revised 2025 form expressly says to read the attached PDS guide before accomplishing the form. Use the guide corresponding to your required version.
- Complete each applicable field accurately. Follow the form’s prompts, write entries in full where directed, and enter “N/A” for items that do not apply when the form instructs you to do so.
- Follow the format instructions. If handwriting the 2025 form, print legibly; tick the appropriate boxes; do not abbreviate; and use a separate sheet when necessary. Check the guide and receiving agency’s instructions for any additional requirements.
- Check the finished PDS and any required Work Experience Sheet. Review entries against your records and make sure required pages or attachments are included before submission.
Printing and submission details to confirm
The CSC general forms index says to specify 8 x 14 inches as the paper size when printing forms. Because that note appears on the general forms page, check the downloaded file and the receiving agency’s instructions before printing; do not treat it as a substitute for requirements stated with your application.

Submission procedures can depend on the hiring agency. The CSC listings do not establish one universal rule for paper versus electronic filing, signatures, photographs, or attachments, so follow the vacancy announcement or ask the office receiving your application.
