Compare each plan’s premium and likely costs when you use care, then check its network, drug coverage, and rules. A plan name alone cannot tell you which choice is best.
How should you compare health insurance plans at work?
Ask your benefits administrator for the Summary of Benefits and Coverage, employee contribution rates for each coverage tier, provider directory, prescription formulary, and enrollment deadline. Compare the same coverage tier and benefit year across plans.
- Calculate the recurring premium. Multiply the amount withheld each pay period by the number of pay periods in the year. Include the cost to cover a spouse or dependents if relevant.
- Check the deductible and cost sharing. Find the individual and family deductibles, which services are subject to them, and what copayments or coinsurance apply. A deductible is what you pay for certain covered services before the plan begins paying, subject to exceptions such as preventive services.
- Compare the out-of-pocket maximum. This helps show your potential exposure for covered, in-network care in a high-cost year. Review the plan terms to understand which expenses count and how individual and family limits work.
- Verify access to care. Check whether your preferred primary-care clinician, specialists, hospitals, and pharmacies are in the current network. Confirm referral requirements, service-area limits, and telehealth terms.
- Review prescription coverage. Check the formulary, drug tiers, copays or coinsurance, and any prior authorization or specialty-drug rules for medicines you use.
- Account for employer contributions. If offered, include any employer contribution to an HSA or other benefit account, and check the account’s eligibility and spending rules.
HealthCare.gov advises considering both premiums and costs when receiving care. For a practical comparison, estimate your annual premium plus the care costs you reasonably expect, then separately consider what you could owe in a high-cost year. What is the difference between an HMO, EPO, PPO, and POS?
These labels describe common network and referral arrangements, but they are not a substitute for the employer plan’s contract or current provider directory. HealthCare.gov notes that some plan types restrict provider choices or encourage members to use an in-network set of doctors, hospitals, pharmacies, and other services. These are general descriptions, not promises about a specific employer plan. Confirm details in the plan materials before enrolling.
How do premiums, deductibles, and metal categories affect the choice?
The premium is the recurring amount you pay to keep coverage, whether or not you use care. The deductible, copayments, and coinsurance affect what you pay when you receive care. A lower premium can come with higher costs when you need services, while a higher premium does not automatically make a plan the best fit. Compare the full cost picture for your actual coverage tier and benefit year.

Bronze, Silver, Gold, and Platinum are categories that describe how costs are shared between the plan and enrollee; they do not indicate quality of care. Do not choose based on the metal label alone. What other employer health benefits might be available?
Some employers offer arrangements in addition to, or alongside, group health insurance. Availability and terms depend on the employer and plan documents.

- Flexible spending account (FSA): Has its own rules for contributions and eligible expenses.
Ask the benefits administrator about eligibility, contribution limits, rollover or use-by deadlines, and which expenses qualify. HealthCare.gov outlines employer coverage and account arrangements in its business health insurance overview.
Can you use the Marketplace if your employer offers insurance?
You can consider Marketplace coverage, but declining an employer offer does not by itself make you eligible for premium tax credits. Under HealthCare.gov’s guidance, an affordable job-based plan that meets minimum standards generally makes the employee ineligible for those credits. The affordability determination depends on the applicable plan year and household circumstances, including the relevant lowest-cost offer. For 2027 Marketplace coverage, HealthCare.gov says open enrollment starts November 1 and runs through January 15. Certain qualifying life events—such as losing coverage, getting married, or having a child—may allow enrollment outside that period through a Special Enrollment Period. These Marketplace dates are separate from your employer’s enrollment schedule. Check current rules for coverage outside Open Enrollment and confirm your employer’s deadline with HR.

HealthCare.gov describes Marketplace coverage and COBRA continuation as possible routes after losing job-based insurance. The Marketplace loss-of-coverage enrollment window is 60 days. COBRA commonly permits continuation for 18 months, with the person usually paying the full premium plus a small administrative fee. Check your written notices and deadlines: do not assume either option starts automatically. Compare effective dates, provider access, benefits, and total costs before deciding. Where can you get help with enrollment?
Your employer’s benefits administrator can explain the specific plans and contribution rates available to you. HealthCare.gov says small employers generally may use the Small Business Health Options Program (SHOP), which serves employers with 1–50 employees and allows an employer to offer one plan or a choice of plans; not every employer uses SHOP, and availability can vary by location.
