USA TODAY reported on October 7, 2026, that a customs change expected October 22 could disrupt some Americans’ personal prescription shipments from foreign pharmacies and raise their costs. But the report’s date and its prescription-specific effects have not been confirmed by a primary CBP or FDA notice identified here. A broader customs change is already in effect, and separate FDA restrictions generally limit personal imports of prescription drugs.
What is supposed to change on October 22?
USA TODAY reported that, starting October 22, prescription packages mailed from foreign pharmacies would no longer receive the previous low-paperwork, duty-free treatment and could face documentation, duties and FDA-related product details. The report said CBP, the White House and FDA had not responded to requests for comment. No primary agency notice confirming that specific date or the precise prescription-shipment process was identified in the official guidance discussed here. There is an important customs backdrop: CBP says duty-free de minimis treatment ended August 29, 2025, for covered shipments, including international mail, and describes duties and postal processing requirements under the broader policy. Can you legally order prescription drugs from Canada or another country?
Do not assume that a foreign pharmacy’s license—or payment of customs duties—makes a mailed prescription drug lawful to import. CBP says FDA generally does not allow individuals to import prescription drugs purchased abroad. Will a shipment arrive before October 22?
There is no reliable way to infer delivery or legal status from the reported date alone. USA TODAY quoted Ken Hunter, executive director of the Campaign for Personal Prescription Importation, advising that medication would need to be in a person’s mailbox by October 22, not merely ordered by then. That is Hunter’s advice as reported by USA TODAY—not an agency rule, a delivery guarantee or confirmation that an order is lawful. Contact the pharmacy about shipment status, but do not delay care or change a dose based on an estimated delivery date.
What to do if you rely on a foreign supply
- Contact your prescriber and pharmacist. Ask whether a U.S. prescription or a clinically appropriate, affordable alternative is available. Ken Hunter offered this as advice to people affected by the reported change; it is not a guaranteed solution.
- Check your treatment timeline. Ask your care team what to do if a shipment is delayed. Do not stockpile, skip doses or substitute medication without clinician guidance.
- Review domestic insurance coverage. Compare coverage for the exact medicine and pharmacy before changing plans or filling a prescription. If you are considering Medicare coverage for 2027, USA TODAY reported that open enrollment runs October 15 through December 7, 2026; verify current dates and plan terms with Medicare before enrolling.
- Verify the rules with agencies. Check current CBP and FDA guidance rather than assuming that customs duty payment resolves FDA import restrictions.
What the reported savings figures do—and do not—show
USA TODAY reported that pharmacy experts estimate more than 4 million Americans buy lower-cost prescription drugs directly from foreign pharmacies. It also attributed to CPPI an estimate that 89% of people purchasing drugs from Canadian pharmacies are 65 or older, and an average savings of more than 64% on 90-day supplies from licensed Canadian pharmacies compared with U.S. cash prices. These are reported estimates, not federal counts or independently verified savings for every patient; the price comparison depends on the benchmarks used. Mail-order imports are not the same as carrying medicine while traveling
CBP provides separate guidance for travelers, including keeping medication in its original packaging and carrying a prescription or doctor’s note; it describes 90 days as a rule of thumb.
