How Much Does Online PrEP Cost? Insurance and Assistance Explained

Online PrEP costs depend on visits, tests, medication, and delivery. Insurance often covers PrEP without cost sharing, while assistance programs have product-specific limits and exclusions.

There is no single online PrEP price. Your total may include a consultation, lab tests, medication, follow-up care, and shipping. If you have qualifying insurance, PrEP medication and the visits and tests needed to maintain a prescription are generally covered without cost sharing under almost all health plans, though prior authorization and plan rules can still matter. Assistance programs may reduce medication costs, but they do not necessarily pay for appointments or testing.

What makes up the cost of online PrEP?

Think of “online PrEP cost” as a set of separate charges, not just the price of a monthly prescription. Ask the service for an itemized estimate covering:

  • Initial consultation and follow-up visits
  • Required HIV and other laboratory tests
  • Medication and prescription processing
  • Clinician administration, if you choose an injectable medication
  • Specimen-kit shipping or other delivery fees

Telehealth and mail-in specimen collection can enable the testing needed to start or continue PrEP when an in-person appointment is not possible, according to the CDC’s HIV Risk Reduction Tool. That does not mean a particular online provider’s visit, kit, or shipping is free.

The federal sources cited here do not provide comparable, current all-in prices for named online PrEP providers. A provider’s total depends on its fees and what your insurance or assistance program pays; do not estimate it from a medication copay cap alone.

What insurance and Medicaid may cover

The CDC says most insurance plans and state Medicaid programs cover some form of PrEP. Under the Affordable Care Act, almost all health plans must cover PrEP without cost sharing, including the medication and clinic visits and lab tests needed to maintain a prescription. That is a coverage rule, not a promise that every online service or every charge will be free. Prior authorization may be required, and plan network, drug, and billing rules still apply. Before enrolling, ask your insurer and the telehealth provider to confirm the covered medication, whether the provider and labs are in network, whether authorization is needed, and how remote visits and tests will be billed.

Tablets and injections may use different benefits

Daily oral medication may be billed through pharmacy coverage. An injection administered by a clinician can involve medical-benefit billing and administration costs. HIV.gov’s assistance information lists Apretude cost-sharing across medical and pharmacy benefits, illustrating why an injection’s billing may differ from a tablet’s. Confirm how your own plan handles the drug and administration.

Medicare

Since January 1, 2025, Medicare fee-for-service has covered PrEP services under its additional preventive services payment framework, according to CMS’s PrEP billing and payment guidance. CMS describes supplier billing, not a consumer’s online-care price or individual out-of-pocket amount. Ask your plan and provider what you would owe.

Which PrEP medication is right for you?

The CDC lists three FDA-approved PrEP medications: daily oral emtricitabine/tenofovir disoproxil fumarate (F/TDF, brand Truvada or generic equivalent), daily oral emtricitabine/tenofovir alafenamide (F/TAF, brand Descovy), and cabotegravir injection (Apretude) given every two months. Its clinical guidance recommends F/TDF for people with sexual or injection-drug-use risk factors, and F/TAF for sexual transmission except for people likely to acquire HIV through receptive vaginal sex. A clinician should determine which option is appropriate; price alone is not a safe basis for choosing a regimen.

When comparing options, check clinical suitability, medication and care charges, whether coverage uses pharmacy or medical benefits, authorization and network requirements, and which assistance program applies. The available sources do not establish one regimen or delivery format as cheapest for everyone.

What assistance programs can pay for

HIV.gov’s assistance table says its information is current as of January 2026. It lists the following annual copay-assistance limits. These are program maximums, not guaranteed awards or estimates of your total annual bill; eligibility and covered expenses vary by product and program.

Descovy support from Gilead

Gilead says eligible people with commercial insurance may receive Descovy copay assistance up to $7,200 per year. People enrolled in government health programs such as Medicare Part D, Medicaid, TRICARE, or VA are not eligible for that copay coupon, though Gilead may direct them to other funding sources. Uninsured patients may qualify for free Descovy through Gilead’s Medication Assistance Program. The company says actual out-of-pocket cost depends on the person’s circumstances and insurance plan. State assistance

Some states have PrEP assistance programs. Depending on the state, a program may help with medication, visits, lab costs, or a combination. There is no single nationwide set of state-program benefits, so check the rules for your state.

How to get a realistic estimate

  1. Ask the online provider for an itemized estimate. Request the consultation, required tests, medication, follow-ups, and specimen-kit shipping as separate amounts.
  2. Check your coverage. Ask your insurer whether the provider, labs, and medication are covered; whether authorization is required; and which benefits apply to the chosen medication.
  3. Check assistance eligibility. Match the program to the specific medication and your insurance status, then confirm which costs it excludes.
  4. Compare the full expected cost. Include any visit, test, administration, or delivery charges that insurance or assistance will not pay.
