When Should You Stay Home From Work With the Flu?

Stay home with flu symptoms. Most workers can return after symptoms improve and they have been fever-free for 24 hours without fever-reducing medicine; no-fever cases have a separate five-day rule.

Stay home when you have flu symptoms. For most workers, return to normal activities only after your symptoms have been improving overall and you have been fever-free for at least 24 hours without using fever-reducing medicine. If you have suspected or confirmed flu but no fever, CDC says to stay home from work for at least five days after symptoms begin.

When can you go back to work after the flu?

For most workers, the Centers for Disease Control and Prevention (CDC) says both of these conditions must be true for at least 24 hours before you return to normal activities:

  • Your symptoms are getting better overall.
  • You have not had a fever, and you are not taking fever-reducing medication.

Meeting only one condition is not enough. The rule is not simply to wait 24 hours after a fever breaks: your symptoms must also be improving, and medication must not be masking a fever. CDC defines fever in its workplace stay-home guidance as a temperature of 100°F (37.8°C) or higher. See CDC’s flu illness guidance.

What if you have the flu but no fever?

Flu does not always cause a fever. If you have suspected or confirmed flu without a fever, CDC’s flu-specific advice is to stay home from work for at least five days after your symptoms begin. Do not use the fever-free return rule to skip this interval.

What precautions should you take after returning?

Going back to normal activities does not mean the risk of spreading flu is gone. People with flu are most contagious during the first three days of illness, but some people—including young children and people with weakened immune systems—may be contagious longer. These are general patterns, not a way to determine whether a particular person is still infectious. CDC’s respiratory-virus guidance, dated August 18, 2025, recommends taking added precautions for five days after resuming activities.

  • Improve ventilation or otherwise bring cleaner air into shared spaces.
  • Practice good hygiene, such as covering coughs and washing hands.
  • Wear a well-fitting mask around others.
  • Keep some distance from other people when practical.
  • Consider testing when you will be around others.

Take particular care around people at higher risk of severe respiratory illness. A mask or other precaution is not a substitute for staying home until you meet the return criteria.

Does the rule differ for healthcare and patient-care jobs?

Yes. Healthcare personnel have separate infection-control guidance, so patient-care workers should follow their facility’s policy and occupational-health instructions rather than applying the general workplace rule alone.

Under CDC’s seasonal influenza infection-control guidance, healthcare personnel with fever and respiratory symptoms should not report to work. If symptoms begin at work, they should stop patient-care activities, put on a facemask, and notify the appropriate supervisor or infection-control or occupational-health contact before leaving. Exclusion continues until at least 24 hours after fever ends without fever-reducing medication; ongoing respiratory symptoms may require occupational-health assessment, especially for patient contact. CDC also has broader healthcare-personnel viral respiratory infection recommendations, updated August 31, 2026, with separate work-restriction criteria. Those healthcare-specific rules are not a general standard for every workplace.

When should you contact a healthcare provider?

If you are at higher risk of serious flu complications, contact a healthcare provider promptly to ask about antiviral treatment. CDC says treatment benefit is greatest when started within two days of symptom onset, though treatment started later may still help some people who are at higher risk or very ill. What to check before your next shift

  • Flu symptoms and a fever: Stay home until symptoms are improving overall and you have been fever-free for at least 24 hours without fever-reducing medication.
  • Suspected or confirmed flu without a fever: Stay home at least five days from symptom onset.
  • Patient-care work: Follow your facility’s infection-control and occupational-health instructions.
  • After returning: Use added precautions for five days, especially around people at higher risk of severe illness.
