Working While Sick vs. Taking a Sick Day: How to Decide

Use CDC’s symptom-improvement and fever-free threshold to guide when to stay home, then weigh your ability to work, exposure risk, remote-work options, and leave policy.

If you have respiratory-virus symptoms, stay home and away from others until your symptoms are improving overall and you have been fever-free for at least 24 hours without using fever-reducing medicine. If you are too unwell to work, take a sick day. If you feel able to work but could expose others, ask whether you can work remotely and check your workplace’s leave policy. This is general U.S. guidance, not a diagnosis or a determination of your legal rights.

Should you go to work if you’re sick?

For respiratory-virus symptoms that are not better explained by another cause, the CDC advises staying home and away from other people until both conditions are met:

  • Your symptoms are improving overall.
  • You have had no fever for at least 24 hours without taking fever-reducing medication.

That is a general threshold for resuming normal activities, not a guarantee that you are no longer contagious. For five days after returning to normal activities, the CDC recommends added precautions to reduce the chance of spreading illness. Its guidance says to “Stay home and away from others (including people you live with who are not sick) if you have respiratory virus symptoms that aren’t better explained by another cause.” See the CDC guidance on preventing spread of respiratory viruses when you’re sick.

How to choose between a sick day, remote work, and going onsite

Take a sick day if you need rest or cannot work effectively

If symptoms leave you unable to concentrate, perform your duties safely, or get through the workday, taking leave is a sensible choice. There is no CDC-defined fitness-for-work threshold in this guidance; consider what your job requires and whether working would compromise your recovery or performance.

Ask about remote work if you feel able to work but should avoid contact

If your role allows it and you feel well enough, working from home can avoid exposing coworkers, customers, or other people onsite. Remote work is an option, not an instruction to push through illness. The CDC recommends flexible telework and leave policies as ways employers can support workers staying home when sick; that recommendation does not create an individual entitlement to work remotely.

Do not return onsite before meeting the respiratory-virus threshold

If you have not yet had overall symptom improvement and 24 fever-free hours without fever-reducing medicine, stay home rather than treating a mask or a remote-work option as a reason to return onsite. If you do return after meeting the threshold, use added precautions for the next five days. The CDC lists wearing a well-fitting mask among those possible precautions.

What the work-attendance evidence does—and does not—show

A CDC-published observational study in Emerging Infectious Diseases examined about 8,000 people over a four-year period spanning prepandemic and pandemic periods. Among study participants with COVID-19, 51.2% reported working onsite on illness day 1, 22.3% on day 2, and 14.1% on day 3. These figures describe that study sample and period; they are not estimates of current population prevalence.

Workers with hybrid experience were less likely than onsite-only workers to work onsite during early illness. For onsite work on illness day 3, the study reported adjusted odds ratios of 0.38 (95% CI 0.29–0.49) during the pandemic period and 0.69 (95% CI 0.54–0.87) during prepandemic influenza seasons. Those are comparisons within the study, not absolute risk estimates. The findings describe attendance patterns; they do not show that working while sick speeds recovery or is medically advisable. Check your leave options before deciding

Sick-leave rules depend on where you live, your employer, your employment arrangement, and your eligibility. Review your employer’s policy and any applicable local requirements; a general public-health recommendation does not establish a legal right to paid leave or remote work.

The CDC’s healthcare-personnel guidance summarizes the Family and Medical Leave Act (FMLA) as providing unpaid, job-protected leave for eligible employees of covered employers for specified reasons. When illness may need medical care

People at higher risk of flu complications, people who are very sick, or anyone worried about their illness should contact a healthcare provider. CDC says antiviral treatment for higher-risk flu patients has the greatest benefit when started within two days of symptom onset, so contacting a clinician promptly can matter.
