When Does Health Insurance End After Leaving a Job?

Health insurance does not always end on your final day at work. The employer’s plan sets the termination date: coverage may stop when employment or eligibility ends, or continue through the end of that month or another stated date. Ask the benefits administrator to confirm your last covered day in writing, then line up replacement coverage before that date.

Find your exact last day of coverage

Do not assume your final workday is your final insured day. Employers use different plan rules, and the controlling date may be your separation date, the date you lose eligibility, the end of the pay period, or the end of the month.

  1. Ask HR, the benefits administrator, or the plan’s insurer for the precise termination date.
  2. Check your separation paperwork, benefits portal, and Summary Plan Description for the plan’s termination rule.
  3. Request the answer in writing. That date determines when a replacement policy must begin and when enrollment deadlines start.

If you are leaving voluntarily, being laid off, or moving from full-time to part-time work, confirm whether the event changes eligibility immediately or at a later date.

What happens when job-based coverage ends?

Once active coverage terminates, you generally need to elect continuation coverage or enroll in another plan. The main possibilities are federal COBRA, a special enrollment opportunity through another employer, a Marketplace plan, or Medicaid or CHIP if you qualify.

COBRA: keep the same group plan temporarily

COBRA is continuation of your former employer’s group health plan, not an automatic extension. Federal COBRA generally applies to covered private-sector and state or local government plans that meet the applicable employer-size threshold. Job loss other than gross misconduct and a reduction in hours are common qualifying events.

How long can COBRA last?

After job loss or reduced hours, continuation is usually available for up to 18 months, although exceptions and extensions can apply in some circumstances. Your election notice should state the maximum period available to you and your dependents.

How much does COBRA cost?

You normally pay the full group-plan premium that was previously shared with your employer, plus up to a 2% administrative fee. Because the employer contribution may disappear, the monthly bill can be substantially higher even though the plan’s deductible, network, and covered medicines remain familiar.

What is the COBRA election deadline?

You generally have 60 days to elect COBRA, measured from the later of the date active coverage ends or the date the election notice is provided. If you elect and pay as required, continuation generally reaches back to the date active coverage ended, helping bridge a gap while you compare alternatives.

Payment rules matter

The initial COBRA payment is generally due within 45 days after election. Later payments ordinarily have a 30-day grace period. Missing an election or payment deadline can end your right to continuation, so keep the notice, election form, and proof of delivery.

Other ways to replace employer coverage

Option Coverage and cost considerations Typical enrollment timing
COBRA Keeps the former group plan, including its network and drug coverage, but you generally pay the entire premium plus up to 2%. Generally 60 days from the later of coverage ending or delivery of the election notice.
Another employer’s plan May be available through a spouse or another job. Compare the new plan’s premium, deductible, network, medicines, and dependent eligibility. Special enrollment generally must be requested within 30 days after losing other coverage.
Marketplace plan Offers individual coverage; eligibility for financial assistance depends on household circumstances and the plan selected. Loss of job-based coverage generally opens a 60-day special enrollment period around the loss.
Medicaid or CHIP Public coverage for people who meet program requirements; eligibility and benefits vary by state and household circumstances. Applications are available year-round for people who qualify.

The dates above are general statutory timing rules summarized in current U.S. Department of Labor guidance. Your notice, state rules, and plan documents control the deadline in your case.

How to compare COBRA with a replacement plan

Compare the plans for the same period and include the date each option actually starts. A lower premium can be offset by a high deductible or an out-of-network doctor.

  • Effective date: Identify the first day claims will be covered and whether any gap exists.
  • Total premium: For COBRA, include the full group premium and administrative fee; for other plans, include any employer contribution or available subsidy.
  • Cost sharing: Compare deductible, copays, coinsurance, and the annual out-of-pocket maximum.
  • Network: Verify your doctors, hospitals, specialists, and pharmacies, not just the insurer’s name.
  • Prescriptions: Check each regular medicine, its tier, prior-authorization rules, and pharmacy requirements.
  • People covered: Confirm whether a spouse or dependents can enroll and whether their eligibility differs from yours.
  • Deadline: Record the election or application deadline and the date you submitted it.

Important traps and exceptions

Federal COBRA is not universal

Some smaller employers are outside federal COBRA. State continuation laws, sometimes called “mini-COBRA,” may provide different rights, durations, or deadlines. Ask the plan administrator which law applies to your employer and state rather than assuming federal COBRA is available.

Electing COBRA is optional, but waiting has consequences

You must elect COBRA and pay on time; coverage does not continue merely because you are eligible. The retroactive feature can help while you arrange a Marketplace or other plan, but it is available only when the election and payment requirements are met.

Ending COBRA early may not reopen enrollment

Voluntarily terminating COBRA generally does not, by itself, create a new Marketplace or employer-plan special enrollment period. Exhausting COBRA or experiencing another qualifying event can change your enrollment rights. Confirm the consequence with the Marketplace or the relevant plan before ending coverage.

A practical timeline after leaving a job

  1. Before your last day: Obtain the plan’s written termination date, current premium information, and contact details for the administrator.
  2. When the notice arrives: Read the COBRA election notice, identify the 60-day election deadline, and calendar the payment deadlines.
  3. Within the available enrollment windows: Request special enrollment in a spouse’s or new employer’s plan within the applicable 30-day period, or apply for Marketplace coverage during the 60-day loss-of-coverage window.
  4. Before choosing: Compare total costs, providers, prescriptions, dependents, and effective dates side by side.
  5. After enrolling: Save confirmation, premium receipts, and the effective-date notice. Use those records if a claim is processed during a transition.

What to ask the benefits administrator

  • What is the exact last date my active coverage is in force?
  • Does the plan end on my separation date, at month-end, or on another date?
  • When and how will the COBRA election notice be delivered?
  • What is the monthly COBRA premium for me and each dependent?
  • Does a state continuation rule apply if federal COBRA does not?
  • Will a spouse’s or new employer’s plan accept a special-enrollment request, and what effective date will apply?
