Medicare Advantage (Part C) replaces the way you receive Medicare benefits through a private plan, while Medigap supplements Original Medicare. You generally choose one approach for the same coverage period: Original Medicare (Parts A and B), usually with a separate Part D plan and optional Medigap, or a Medicare Advantage plan. Your best choice depends on doctors, prescriptions, location, travel, expected care and eligibility for Medigap—not on a universal “cheapest” label.
What’s the difference between Medicare Advantage and Medigap?

How much does each option cost?
There is no nationwide cost winner. Compare the complete annual picture: premiums, deductibles, copayments or coinsurance, prescriptions, provider charges and the services you expect to use. A plan with a $0 advertised premium can still create substantial costs when you receive care.
Costs shared by both approaches
Medicare’s standard Part B premium is $202.90 per month in 2026; a higher income-related amount may apply. The 2026 Part B deductible is $283. These figures are Medicare amounts, not a guarantee of what your total insurance spending will be.
Original Medicare with Medigap
You pay the Part B premium and a separate Medigap premium. If you select Part D, that plan adds another premium and its own deductible and pharmacy cost sharing. Medigap premiums vary by insurer, letter plan, location and pricing method, and can change annually. For 2026, Medicare lists a $8,000 annual out-of-pocket limit for Plan K, a $4,000 limit for Plan L, and a $2,950 high-deductible threshold for Plans F and G. Those are specific policy rules, not estimates for every Medigap enrollee.

Medicare Advantage
Plans set their own premiums and cost sharing for covered Medicare services. The annual out-of-pocket limit offers protection that Original Medicare lacks on its own, but you still pay the plan’s copayments or coinsurance until reaching that limit. Prescription costs, inpatient care, specialist visits and outpatient services can all affect the total.
Which choice gives you more provider freedom?
Original Medicare generally lets you see any U.S. doctor or hospital that accepts Medicare, without a plan network. Check that a particular provider accepts Medicare assignment and that the service is covered. Medigap does not usually restrict that provider choice; it helps pay eligible Original Medicare cost sharing.

Medicare Advantage plans may use health maintenance organization (HMO), preferred provider organization (PPO) or other network designs. Non-emergency care outside the network may cost more or may not be covered, and some plans require referrals for specialists. Verify every important doctor, hospital and specialist in the exact plan’s current directory and read its service-area rules.
How do prescriptions, authorization and extra benefits differ?
With Original Medicare, prescription coverage is normally a separate Part D decision. Compare each medication’s formulary tier, required authorization, quantity limits, preferred pharmacies and expected annual cost.

Most Medicare Advantage plans include Part D, but the same plan name can have different formularies or pharmacy terms by service area. Advantage plans may also require prior authorization for certain services. Extra benefits are not standardized: confirm what is offered, eligibility conditions, networks and annual limits instead of assuming every plan includes the same extras.
Can I have both Medicare Advantage and Medigap?
No. Medigap is designed to supplement Original Medicare. It cannot pay a Medicare Advantage plan’s copayments, deductibles or premiums, and insurers generally cannot sell you Medigap for use alongside Advantage coverage. If you enroll in Medicare Advantage, your Medigap policy may not pay benefits for that period; do not cancel an existing policy until your replacement coverage and rights are confirmed.
Can I switch back to Medigap later?
Returning to Original Medicare is not the same as having an assured right to buy a Medigap policy. Access depends on your Medigap Open Enrollment Period, a federal or state guaranteed-issue event, medical underwriting rules and your state’s protections. Outside a protected period, an insurer may be able to decline coverage, charge more because of health history or impose other lawful terms.
The first-time Medicare Advantage trial right
Medicare describes a one-time, 12-month trial opportunity for someone who drops Medigap to join Medicare Advantage for the first time. If that person returns to Original Medicare during the trial period, they may be able to recover the same Medigap policy if the same insurer still sells it. Conditions apply, so confirm the dates, insurer availability and your prior coverage before disenrolling.

Other guaranteed-issue situations
Specific events, including certain plan terminations or non-renewals, can create guaranteed-issue rights. CMS’s 2026 Medicare Advantage enrollment appendices describe a 63-day period after coverage ends in stated circumstances. That is not a universal deadline for every switch. Obtain a written determination of your right before dropping coverage.

- List your providers. Check network participation, hospital affiliation, service area and referral rules for each Advantage plan under consideration.
- Enter every prescription. Compare the Advantage formulary or separate Part D plan, pharmacy choice, authorization requirements and estimated yearly drug cost.
- Estimate a full year. Add the 2026 Part B premium, the plan or Medigap premium, deductibles, routine copayments, coinsurance and likely specialist, hospital and therapy use.
- Review the worst case. For Advantage, record the plan’s annual limit for covered Medicare services. For Medigap, identify what the letter plan pays, what remains exposed and any deductible or annual limit.
- Check administration. Read prior-authorization, referral, appeals and notification rules for services you expect to need.
- Match travel needs. Review emergency coverage outside the U.S. and whether domestic travel will take you outside an Advantage service area.
- Protect switching rights. Before canceling Medigap or leaving Advantage, ask Medicare, your State Health Insurance Assistance Program (SHIP) or your state insurance department which enrollment and guaranteed-issue rules apply to your history.
Who may prefer each arrangement?
Original Medicare plus Medigap may fit when
- You value broad access to Medicare-participating providers nationwide.
- You want predictable help with covered Original Medicare cost sharing and can afford the separate Medigap premium.
- You travel frequently and a Medigap policy’s foreign-emergency benefit is important.
- You prefer choosing Part D separately and generally avoiding Medicare Advantage network administration.
Medicare Advantage may fit when
- Your doctors and hospitals participate in a suitable local network.
- You prefer one plan that commonly combines Part A, Part B and Part D administration.
- You want an annual limit on covered Medicare-service spending and accept plan-specific networks, referrals or prior authorization.
- Plan extras are valuable to you after checking their actual limits and provider requirements.
Neither list is a guarantee of eligibility, availability or savings. Plan contracts, counties and personal medical needs control the result.
