Change Healthcare breach: the 100 million figure is outdated—latest estimate 192.7 million

“100 million U.S. citizens” is outdated and imprecise. Change Healthcare later reported approximately 192.7 million individuals impacted as of July 31, 2025. Here is what the figures mean, what data may be involved and how to protect yourself.

Short answer: Yes. “100 million” was a genuine interim figure reported by Change Healthcare, but it referred to notices sent by October 22, 2024—not a final count of U.S. citizens. Change Healthcare later reported approximately 192.7 million individuals impacted as of July 31, 2025, according to the U.S. Department of Health and Human Services (HHS) Office for Civil Rights (OCR). That remains the latest figure identified in HHS’s official FAQ, and it is an estimate of individuals, not an independently audited census of unique Americans.

How the official figure changed

The dates are successive updates, not contradictory counts. HHS relayed figures supplied through Change Healthcare’s reporting process; they are not a government audit proving that exactly 192.7 million distinct U.S. citizens had identical data exposed.

What happened in the Change Healthcare attack?

In February 2024, ransomware disrupted Change Healthcare, a UnitedHealth Group company that handles claims, pharmacy transactions, payments, prior authorizations and other administrative exchanges for much of the U.S. health system. The company shut down systems to contain the intrusion. HHS described the event as having an unprecedented effect on patient care, privacy and healthcare operations in its cyberattack report.

The operational outage and the privacy incident are related but distinct. A patient could experience a delayed prescription or a provider’s billing problem without being in the affected data, while someone whose information was involved might never notice an interruption in care.

What does “individuals impacted” mean?

“Impacted” means information associated with a person may have been involved in the incident. It does not mean every person had the same information exposed, that every file was downloaded, or that anyone’s data was misused.

  • Notices are not records: A notice is a communication to a person or responsible organization. Records can contain multiple data elements and may relate to more than one individual.
  • Potential exposure is not confirmed misuse: Change Healthcare’s estimate concerns possible involvement of information. It is not proof that each person suffered identity theft.
  • Citizenship is not established: HHS and Change Healthcare use “individuals impacted,” not “U.S. citizens.” The population could include people whose information was processed through healthcare relationships without establishing nationality or residence.

What information may have been involved?

Change Healthcare’s substitute notice says potentially affected information may include:

The categories varied by person. The notice does not mean that every individual’s complete medical file or every listed identifier was exposed. Read the official substitute notice for the company’s current instructions and descriptions.

Were complete medical records stolen?

There is no basis for saying that full medical histories for all affected people were stolen. UnitedHealth said in April 2024 that its preliminary review found files containing protected health information or personally identifiable information but no evidence at that point of exfiltrated doctors’ charts or complete electronic medical-record databases. Those are company statements about the data it analyzed, not an independent guarantee that no detailed health information was involved. Change Healthcare’s notice still says personal and health information may have been affected. (UnitedHealth update)

How can you tell whether your information may be involved?

Change Healthcare used direct letters where it had usable contact information and a public substitute notice because it provides services to providers, health plans and other organizations and may not have had a direct way to reach everyone. You may instead hear from a healthcare provider, insurer, employer plan or another organization responsible for notification.

  • Search your mail and secure online accounts for a Change Healthcare notice or a notice from a provider or plan.
  • Do not treat the absence of a letter as proof that no information was involved; addresses and contact details may have been incomplete, or another organization may have handled notice.
  • Use contact details published on Change Healthcare’s official site. Do not click unsolicited “breach,” refund, settlement or verification links or disclose account credentials to callers claiming to represent the incident.
  • You may receive more than one letter if different organizations connected to the same event notify you separately.

What to do now

  1. Save the original notice. Record the date, notifying organization, reference number and the data categories it identifies.
  2. Use the official assistance channel. The substitute notice describes up to two years of complimentary credit monitoring and identity-theft protection, with enrollment and telephone instructions. Confirm that enrollment is still open and that you qualify before submitting information.
  3. Freeze your credit. Place free freezes directly with Equifax, Experian and TransUnion. A freeze blocks prospective creditors from accessing your file; it does not stop medical identity theft, phishing or insurance fraud.
  4. Review reports and accounts. Look for unfamiliar accounts, inquiries, address changes and financial activity.
  5. Secure online access. Change passwords reused on health-plan, provider, email or financial accounts, and turn on multifactor authentication.
  6. Check medical activity. Review explanation-of-benefits statements, prescriptions, providers and claims for services you did not receive. People without a conventional credit file can still face medical-identity or account-takeover risks.
  7. Document and report. Keep notices, suspicious messages and expenses. Report suspected identity theft to the Federal Trade Commission and the relevant financial institution, insurer or provider.

Credit monitoring is an alerting and restoration service, not a guarantee that misuse will be prevented or reimbursed. Paid monitoring may be useful after any complimentary period, but first use the official offer and free freezes; do not assume a commercial service is affiliated with Change Healthcare.

How providers and patients were affected operationally

Because Change Healthcare processed a major share of U.S. healthcare transactions, hospitals, pharmacies, physician practices, laboratories and other providers faced claim, payment and prescription disruptions. UnitedHealth said it provided billions of dollars in advance funding and interest-free loans to support providers during the outage. (UnitedHealth provider update)

Regulatory and legal status

HHS OCR opened investigations into Change Healthcare and UnitedHealth Group concerning whether unsecured protected health information was breached and whether HIPAA obligations were followed. HIPAA’s Breach Notification Rule generally requires notice without unreasonable delay and no later than 60 days after discovery of a qualifying breach, although duties differ depending on whether the responsible organization is a covered entity or business associate. Lawsuits and regulatory inquiries may continue. Nothing about the 192.7 million estimate automatically establishes eligibility for compensation, and readers should rely on current court or settlement documents rather than unsolicited claims websites.

What remains unknown

  • The estimate is not a real-time 2026 count and may not identify unique people in the way a population census would.
  • Public figures do not specify the exact data elements for every individual.
  • Known misuse is not the same as proof that misuse can never occur later.
  • No public statement supports claims that every affected person’s complete medical history was taken or that every person is entitled to payment.
