There is no evidence-based universal “best” medical billing company for every U.S. practice. The right choice depends on your specialty, payer mix, claim volume, EHR, the work you want to outsource, and the contract and pricing terms. AdvancedMD, R1 RCM, and Tebra illustrate different service models—not an independently tested or ranked top three.
Start by deciding whether you need billing software your staff will operate, a partial billing service, or broader revenue-cycle management (RCM). Then compare vendors against the same scope and verify claims with references from similar practices.
What does a medical billing company actually do?
“Medical billing” can describe software, outsourced labor, or a broader RCM service. Before comparing providers, list which work your practice needs handled and which responsibilities will remain in-house.
- Front-end work: patient registration and insurance eligibility verification.
- Claim preparation: charge capture, coding support, claim edits or scrubbing, and submission.
- Payment and follow-up: electronic remittance and payment posting, denial follow-up, underpayment follow-up, and credit-balance handling.
- Patient billing and oversight: patient statements, collections workflows, and reporting.
Ask vendors to identify exactly which tasks they perform, which your staff must perform, and how exceptions are handed off. A software subscription does not necessarily include billing labor; a billing service may not include every RCM function.

Which companies are worth evaluating?
The available vendor descriptions support considering the following as examples of different approaches. They do not establish that any provider is best for a particular practice, or document comparative outcomes.

AdvancedMD’s published amounts are vendor-listed prices, not a market benchmark or a guaranteed quote. The reviewed R1 and Tebra materials do not provide comparable public RCM prices. CareCloud’s “Top 10 Medical Billing Companies for 2026” page names vendors including Tebra, NextGen Healthcare, R1 RCM, and MediBillMD, but it is vendor-authored; use it as a lead list rather than an independent ranking.

AdvancedMD’s pricing page, accessed in 2026, lists the following amounts. The vendor says pricing varies by configuration; confirm the current quote, billing basis, and included work directly.

How should a small practice choose?
Practice size alone does not identify the best fit. A small office may want software and retain billing work in-house, outsource specific tasks, or contract for broader RCM. The right model depends on internal staffing, claim volume, systems, and how much operational control the practice wants to retain.
- Define the work to transfer. Mark each billing task as vendor-owned, practice-owned, or shared, including exception handling and patient questions.
- Match experience to your work. Ask for experience with your specialty and payer mix, including Medicare, Medicaid, commercial plans, and specialty-specific workflows relevant to your practice.
- Check the system fit. Confirm compatibility with your EHR and practice-management platform, clearinghouse arrangements, integrations, data conversion, and data portability.
- Compare a matched quote. Give every bidder the same practice profile, claim volume, systems, and requested services. Compare total expected cost rather than the headline fee.
- Validate service quality. Request references from practices similar in specialty, payer mix, size, and system configuration, plus a sample dashboard or service-level report.
- Review the contract and transition plan. Confirm accountability, reporting, audit rights, subcontractors, security incident notice, record access, termination terms, and help moving the work or data if the relationship ends.
What should you compare in vendor proposals?
Use one request-for-proposal scope for every candidate. This makes differences in price and service easier to interpret.

- Practice profile: specialty, clinician count, locations, monthly encounters and claims, and payer mix.
- Included tasks: eligibility, charge capture, coding support, claim edits and submission, payment or electronic remittance posting, denials, underpayments, patient billing, credit balances, and reporting.
- Technology and data: EHR and practice-management integration, clearinghouse setup, conversion work, access to records, and data portability.
- Fees: flat subscription, per-provider, per-encounter or per-claim pricing, percentage of collections, minimums, implementation costs, add-ons, and termination charges.
- Service ownership: named account contact, staffing and coverage, escalation routes, onboarding schedule, and the division of work between vendor and practice.
- Measurement: definitions, time windows, exclusions, and data sources for clean-claim rate, days in accounts receivable (A/R), denial and overturn rates, net collection rate, submission lag, and aging.
Put metric definitions and reporting expectations in the contract. Ask how each figure is calculated and what is excluded; do not treat an unsupported performance guarantee as evidence of results.

There is no comparable public price across the providers described here. AdvancedMD publishes the ranges above, while the reviewed R1 and Tebra pages do not state comparable public RCM prices. Request itemized quotes based on your volume and requested scope, and check whether the fee is recurring, variable, or tied to collections.

To compare proposals, calculate the expected total for the same period and the same tasks. Include minimum fees, onboarding, add-ons, and termination costs, and distinguish a software fee from the cost of outsourced billing labor. A lower headline rate may cover less work.
What to verify about claims and billing operations
CMS lists the adopted electronic health-claim transaction format as ASC X12N 837 Version 5010 for professional, institutional, and dental claims. Ask the vendor how it handles your claim types, payer-specific edits, rejections, remittances, and exceptions—not simply whether it “supports claims.”
The Medicare Claims Processing Manual describes a billing service’s role as collecting claim information and billing appropriate insurers, including Medicare. Your agreement should translate that broad role into specific responsibilities, turnaround expectations, and access to the information your practice needs.
HIPAA responsibilities when you outsource
HHS identifies claims processing or administration, billing, and practice management as examples of business-associate functions. If a vendor creates, receives, maintains, or transmits protected health information (PHI) on your practice’s behalf, it may be a business associate. Before granting PHI access, obtain and review the required written business-associate arrangement, including a business-associate agreement (BAA) that specifies the work and requires appropriate HIPAA compliance.
Outsourcing does not remove the practice’s own obligations. HHS explains that a provider remains a covered entity when it conducts qualifying electronic transactions, whether it submits them directly or uses a billing service. Review the BAA, retain appropriate oversight, and establish how the vendor will report incidents, support access to records, and manage any subcontractors.
