For 2026, the standard Medicare Part B premium is $202.90 per month, while the Part D deductible can be as high as $615 and covered Part D drug out-of-pocket costs are capped at $2,100 for the calendar year. These U.S. Medicare figures do not determine every enrollee’s bill: income, plan design, prescriptions and pharmacy can change what a person pays.
1. The standard Part B premium rises to $202.90 a month
The 2026 standard Part B premium is $202.90 monthly, up $17.90 from $185 in 2025. CMS attributed the increase mainly to projected price changes and assumed increases in use consistent with historical experience. This is the standard premium, not a guarantee of an individual’s bill: higher-income beneficiaries may owe an income-related monthly adjustment. 2. The Part B annual deductible increases to $283
In 2026, the Part B deductible is $283, $26 more than the $257 deductible in 2025. It is an annual deductible for covered Part B services, separate from the monthly premium.
3. Part A buy-in premiums and hospital deductible change
Most people who qualify for premium-free Part A based on their own or a spouse’s work history do not pay a monthly Part A premium. Those who must buy coverage pay $311 or $565 a month in 2026, depending on covered work history. The inpatient hospital deductible is $1,736 per benefit period—not per calendar year. 4. Part D plans may not charge a deductible above $615
The maximum deductible a Medicare drug plan may impose in 2026 is $615. A plan may set a lower deductible or none at all, so $615 is a ceiling rather than a required charge. After a deductible, Medicare.gov describes the initial coverage stage as 25% coinsurance for covered generic and brand-name drugs until the enrollee reaches the annual out-of-pocket threshold. Actual plan terms and drug coverage differ. Medicare.gov’s cost guidance outlines how plan costs vary.
5. The covered Part D drug out-of-pocket limit is $2,100
For 2026, an enrollee’s qualifying out-of-pocket costs for covered Part D drugs are capped at $2,100. The threshold includes the deductible and qualifying payments made on the enrollee’s behalf, including certain payments through Extra Help. Once the threshold is reached, covered Part D drugs receive catastrophic coverage with no further enrollee out-of-pocket cost for the rest of that calendar year. Monthly premiums do not count toward this drug-cost cap. The limit does not mean every drug is covered or that plans have identical premiums and cost sharing. 6. Negotiated prices for the first ten selected drugs begin January 1
CMS-negotiated Maximum Fair Prices for the first ten drugs selected for negotiation apply beginning January 1, 2026. The negotiated price is not necessarily the cash price a patient sees or a guaranteed saving for every person: the drug, coverage, coinsurance and plan design affect individual costs. CMS estimated in 2024 that, if the negotiated prices had applied in 2023, net spending across those ten drugs would have been about $6 billion (22%) lower. That is a retrospective estimate across the selected drugs, not a projection of any individual’s 2026 savings. The effective date and estimate are described in CMS’s negotiated-price announcement.
7. The insulin cap is a formula, not a universal $35 price
For each covered insulin product supplied for a month, Part D enrollee cost sharing is limited to the lesser of $35, 25% of the applicable negotiated Maximum Fair Price, or 25% of the plan’s negotiated price. Because the rule applies to covered products and uses the lesser of those amounts, it should not be read as a promise that every insulin product or every purchase always costs $35. 8. Recommended adult vaccines covered under Part D have no cost sharing
Part D plans cannot charge cost sharing for adult vaccines that are recommended by the Advisory Committee on Immunization Practices and covered under Part D. Both conditions matter; this is not a blanket statement about every vaccine or every coverage arrangement. CMS’s 2026 announcement summarizes the provision.
9. Enrollees can spread prescription payments across the year
Part D sponsors must offer the Medicare Prescription Payment Plan, which lets enrollees spread covered prescription out-of-pocket payments across the calendar year rather than paying all costs at the pharmacy. It changes payment timing, not the amount owed, and does not lower total drug costs. Medicare.gov provides the plan’s details under Medicare drug coverage costs.

How to compare plans for your own prescriptions
National limits do not settle which plan is cheapest for an individual. Before choosing or changing coverage, compare the costs and coverage for your prescriptions and preferred pharmacy in your area. Check:
- Monthly premiums, including any income-related adjustment to Part B and the separate premium for a Part D plan.
- Each plan’s deductible and whether your prescriptions are on its formulary.
- Pharmacy network status and any preferred-pharmacy pricing.
- Expected copays or coinsurance for your medications, plus your potential annual out-of-pocket spending for covered Part D drugs.
Use the official Medicare plan comparison and cost information, and verify your specific prescriptions and pharmacy before switching. For people considering going without drug coverage, Medicare.gov says the 2026 national base beneficiary premium used to calculate the late-enrollment penalty is $38.99. Generally, the penalty is 1% of that amount for each full month after eligibility without creditable drug coverage, and it is added to the premium; people with Extra Help do not pay this penalty. Check eligibility and timing before dropping existing creditable coverage. More information is available at Medicare.gov’s cost guidance.
