Aetna’s exit from the ACA public Marketplaces is no longer just a plan: CVS Health says it took effect in January 2026 in every state where Aetna sold individual public-exchange plans. Contemporary reporting estimated that about 1 million members across 17 states were affected. That does not mean all of them became uninsured, and it does not mean CVS Health or Aetna left health care or stopped offering every type of insurance.
What changed in January 2026
CVS Health’s 2025 Form 10-K, filed February 10, 2026, says Aetna exited the states where it operated on the Public Exchanges effective January 2026. The filing describes Aetna’s individual public-exchange business, so the change concerns those Marketplace plans—not every Aetna product or CVS Health service. CVS Health 2025 Form 10-K

How many members were affected?
The Associated Press reported in May 2025 that about 1 million Aetna members in 17 states would be affected. This is an approximate estimate of the population affected by the exit, not a verified count of people who lost all health insurance. CVS later reported that its total medical membership fell by approximately 600,000 between December 31, 2025, and March 31, 2026. The company said the individual-exchange exit contributed to the decline, while growth in Commercial ASC membership partly offset it. What former Aetna Marketplace members should do
If you had an Aetna individual Marketplace plan, review the notices from your insurer and the Marketplace serving your state. The exit does not establish that every member had the same transition, eligibility, or replacement options. A plan available to one household may not be available to another.

- Confirm your coverage status. Check your insurer’s notices and your state Marketplace account for information specific to your plan and household.
- Look for available coverage. Start with your state’s official Marketplace to see which plans, enrollment options, and financial assistance may apply to you. Availability and eligibility vary by state and household.
- Compare total costs and access. Review the monthly premium after any applicable tax credit, deductible and other out-of-pocket costs, provider and hospital network, prescription formulary, and coverage for your clinicians and medications.
- Check before enrolling. Confirm details directly with the Marketplace and the insurer offering the plan. Do not assume a replacement plan includes the same doctors, medicines, or benefits as your former Aetna plan.
The Marketplace remains available, but choices vary
CMS reported 23.0 million 2026 Marketplace plan selections nationwide in its January 28, 2026 snapshot. That national figure shows that Marketplace enrollment continued; it does not establish which plans were available or affordable in a particular county, nor does a plan selection necessarily mean coverage was ultimately effectuated.
