If someone used your information to apply for an SBA loan, report the identity theft to the FTC and submit the required documents to the SBA using the route for that loan program. An FTC report alone does not start the SBA’s review. Save the loan notice or credit-report entry, file at IdentityTheft.gov, then check the SBA’s current reporting instructions before sending personal documents.
What to do first
- Save evidence. Keep the SBA or lender notice, collection or payment letter, credit-report entry, suspicious messages, and any loan or application number. Save copies of anything you send and note the dates and details of calls.
- File an identity theft report. Use IdentityTheft.gov to create an FTC Identity Theft Report and recovery plan. You can also report the theft to local police; the SBA accepts a report filed with the FTC or local police.
- Submit the SBA identity-theft package. Include the three documents SBA requires and use the current submission route for the loan category. The FTC report is useful evidence, but filing it does not by itself initiate SBA review.
- Protect and check your credit files. Consider a fraud alert or credit freeze, and review your reports for unfamiliar accounts and inquiries.
- If the loan was PPP, contact the lender if known. Dispute the loan as unauthorized and ask the lender to address the loan and any related credit reporting.
What to send the SBA
The U.S. Small Business Administration says: “If someone used your information to get SBA funding without your permission, you must submit three documents to SBA.” Its Report fraud or identity theft page lists:
- A copy of government-issued photo identification, such as a driver’s license, state ID, or U.S. passport.
- An Identity Theft Report from IdentityTheft.gov or local police.
- A completed and signed SBA Declaration of Identity Theft form.
Use the SBA page’s listed portal, email, fax, or postal instructions. Confirm the current details there before sending sensitive documents; submission contacts and procedures can change.

The SBA also lists its Loan Portal, fax number 202-481-5200, and a postal address. Because these details may change, consult the live SBA instructions for the accepted channel and exact address before submitting your package.
Decide whether to place a fraud alert or freeze
Both options are free. A fraud alert asks potential creditors to take steps to verify your identity before opening new credit. A freeze restricts access to your credit report unless you lift or remove it. Choose based on how much access you need for legitimate credit applications.

FTC guidance also explains how to request a block of fraudulent information using an Identity Theft Report and proof of identity; a block or dispute is not an instant or guaranteed removal.
If the loan was a PPP loan, contact the lender too
PPP loans were issued through private lenders. If you know which lender made the loan, contact it, state that you did not apply for or authorize the loan, and ask it to address the fraudulent loan and any related credit reporting. Keep a record of your contacts. The FTC provides a sample dispute letter for a fraudulent SBA Paycheck Protection Program loan. This lender contact complements, rather than replaces, the SBA identity-theft submission.

When to contact the SBA Office of Inspector General
The SBA Office of Inspector General hotline handles reports about fraud, waste, abuse, mismanagement, or misconduct involving SBA programs or employees. Use the SBA’s identity-theft process to ask the agency to review a loan opened in your name; an OIG complaint is not a substitute for sending the required identity-theft documents.
