To assess whether ELV merits investment, weigh those signals against membership mix, rate adequacy, legal and regulatory exposure, and a valuation calculated from a dated share price—not from one quarter’s adjusted EPS alone.
What Elevance reported for the second quarter of 2026
Elevance Health, Inc. (NYSE: ELV) is a U.S. health benefits and services company. Its two reporting segments are Health Benefits and Carelon. The figures below come from the company’s July 15, 2026 earnings release and refer to the quarter ended June 30, 2026, unless noted.

Segment results add context but should not be summed to reconstruct consolidated revenue: segment definitions and eliminations matter. In Q2, Health Benefits reported $42.7 billion of revenue and $0.9 billion of operating gain; revenue grew 3% year over year while operating gain declined. Carelon reported $19.2 billion of revenue and $0.9 billion of operating gain; revenue grew 6%. Management cited Carelon Services risk-based solutions and CarelonRx product revenue as contributors.
How medical costs and premiums shape earnings
Read the benefit expense ratio as a margin signal
The benefit expense ratio compares benefit expenses with the applicable premium revenue. Investors often use “medical loss ratio” for a similar concept. A rising ratio means more premium revenue is going to benefits, all else equal, but one quarter cannot establish a lasting margin trend. Look for several quarters of results and management’s explanation of claims experience before deciding whether the change is temporary or persistent.

Elevance identifies utilization and provider unit costs as important drivers. Service mix, member demographics and acuity, provider-contract inflation, labor costs, high-cost drugs, and regulation can also affect claims. The central question is whether premium rates will cover the resulting costs.
Test rate adequacy and timing
Health-plan premiums are generally set in advance against expected cost trends. If actual claims outpace those assumptions, or competitive and regulatory limits prevent sufficient rate increases, profitability can weaken. Medicare and Medicaid results also depend partly on government funding, and the timing of rate-setting may leave a lag between rising costs and updated payments. When assessing a recovery, check whether the relevant rates have actually taken effect and whether subsequent claims experience supports management’s assumptions.
Look beneath the membership total
The quarter-end total does not reveal which products gained or lost members, how much premium exposure changed, or whether the remaining members have a different risk profile. Redeterminations and other attrition can reduce enrollment while changing the average health needs of those who remain. Fee-based membership can also affect member totals differently from risk-based premium exposure.

Track enrollment and operating performance by Individual ACA, Medicaid, Medicare Advantage, employer risk-based and fee-based products, and federal programs. Pair the counts with segment revenue, operating gain, and the company’s explanation of mix. A falling aggregate count is not automatically a sign of worsening profitability; nor does stable enrollment guarantee that margins are sound.

Assess the two segments separately
Health Benefits
Health Benefits includes Individual, Employer Group risk-based and fee-based, BlueCard, Medicare, Medicaid, and Federal Employee Program businesses. Its Q2 revenue increased while operating gain fell, so investors should ask which product lines drove the change and whether government-program costs are being matched by rates. The segment’s size makes its medical-cost and membership trends central to the investment case.
Carelon
Carelon consists of CarelonRx and Carelon Services. Its Q2 revenue and operating gain provide a separate view of the services and pharmacy businesses, but revenue growth alone is not enough: watch operating gain and the sources of growth as well. Management pointed to risk-based solutions in Carelon Services and product revenue in CarelonRx for the quarter.

Key risks to investigate
Medical-cost trends may outrun pricing
Higher utilization, unit costs, acuity, provider-contract inflation, labor expense, or expensive pharmaceuticals can raise claims faster than premiums. Competitive pricing and the advance rate-setting cycle make it possible for a cost increase to hit results before rates can respond.
Medicaid and ACA enrollment may change in both size and mix
Elevance’s quarterly release cited anticipated Individual ACA and Medicaid attrition, while its filing described elevated Medicaid trends and membership shifts associated with eligibility redeterminations. Follow both enrollment and the experience of the members who remain; either can affect revenue and claims costs.

Government rules, risk adjustment, quality ratings, and payment methodology influence Medicare Advantage results. Elevance reported that approximately 59% of its Medicare Advantage members were in plans rated at least 4.0 Stars or equivalent under the 2026 Star Ratings used to determine 2027 bonus payments. That is an Elevance-specific figure, not a market-wide measure. Consider how plan ratings and payment changes could affect the company’s business rather than treating enrollment growth alone as a sufficient signal.

Separate the CMS notice from the DOJ litigation
In its filing for the quarter ended June 30, 2026, Elevance said CMS closed the enforcement process concerning certain diagnosis-code submissions for dates of service from 2015 through April 2023 on July 13, 2026. The company said sanctions would not be imposed after required steps were completed. It reported a $342 million payment and a $593 million accrual at June 30, 2026, and cautioned that the ultimate estimate could differ.
That process is distinct from ongoing DOJ litigation concerning Medicare risk-adjustment allegations. Elevance said the outcome of that litigation could not then be determined. Closure of the CMS process does not mean the separate lawsuit has been resolved.
Broader execution, legal, and financial exposures
Elevance’s filing identifies risks involving changes in health and insurance laws, Medicare and Medicaid compliance, litigation and government investigations, cybersecurity and privacy, information systems and technology, provider contracting, competition, debt, and capital requirements. As of June 30, 2026, the company reported no material changes to the risk factors in its 2025 Form 10-K. Investors should still consider how a major incident or regulatory change could affect operations, costs, or capital needs.

Judge management guidance against what has to happen
Elevance CEO Gail K. Boudreaux said the company was raising adjusted EPS guidance and investing in medical-cost management, member experience, provider connectivity, operating efficiency, and Carelon’s value-based solutions. She also expressed confidence in returning to at least 12% adjusted EPS growth in 2027 from the 2026 earnings baseline. These statements describe management’s outlook; they are not independently established forecasts.
To test that outlook, compare the next reported results with the assumptions implied by guidance: whether medical-cost trends moderate, whether rates catch up, how membership and product mix evolve, and whether Carelon’s operating gain supports its growth. Keep GAAP earnings, adjusted earnings, and cash flow distinct when evaluating progress.
How to value ELV without overstating precision
A defensible valuation requires a share price with an explicit date and an earnings measure matched to a clear period. The materials summarized here do not establish a date-stamped market price, a peer multiple, or a price target, so they do not support a conclusion that ELV is cheap or expensive. Do not infer a normalized earnings level from one quarter or treat company guidance as a guaranteed outcome.
- Choose the price date. Record ELV’s market price on the date of analysis. A valuation changes as the share price changes.
- Choose and label the earnings denominator. Calculate price-to-earnings using either GAAP or adjusted EPS for a stated period. Do not mix a current share price with an unlabeled or mismatched earnings period.
- Treat guidance as a scenario, not a fact. A calculation using the company’s at-least-$27.00 adjusted EPS guidance is guidance-based. Account for the release’s approximately $0.80-per-share net below-the-line benefit when considering how repeatable the earnings base may be.
- Cross-check cash generation and financial capacity. Compare earnings with operating cash flow, then review debt, capital requirements, and capital returns in current filings. Cash conversion and the uses of cash can change the investment case.
- Stress the operating assumptions. Consider alternative outcomes for the benefit expense ratio, premium-rate adequacy, membership and product mix, Medicare Advantage performance, Medicaid margins, and Carelon growth and operating gain.
- Use consistent inputs for any insurer comparison. Align GAAP versus adjusted EPS, expense-ratio trends, risk-based membership mix, government-program exposure, cash conversion, debt, capital returns, and valuation period. A comparison based on inconsistent definitions can mislead more than it informs.
