How to Compare Hospital Bills and Spot Duplicate or Unexplained Charges

Learn how to compare hospital bill line items with your medical records and EOB, identify possible duplicate or unexplained charges, and raise specific questions with the billing office.

To check a hospital bill, get an itemized statement, compare each line with your medical records and any insurer Explanation of Benefits (EOB), then ask the billing office to explain or correct specific discrepancies. A repeated-looking charge is worth checking, but it is not automatically a duplicate: different clinicians or facilities may bill separately for distinct services.

Gather the documents for the visit

Collect the detailed bill, every EOB related to the care, available records for the encounter, receipts or proof of payments, and—if you were uninsured or self-pay—a written good faith estimate. A hospital visit may generate separate bills and EOBs from the facility and clinicians, or notices for different treatment dates. Sort documents by provider and date of service rather than assuming one statement covers the whole episode.

Ask the provider’s billing office for an itemized bill listing the medical items and services and their costs. If you need records, ask how to request them and whether an access fee applies.

Read the bill before comparing totals

Check the billing date, date of service, item or service descriptions, charges, adjustments, payments already made, and remaining balance. Adjustments generally reflect a discount or lower amount accepted by the provider; patient payments may include amounts such as a copayment paid at the visit. Focus on the remaining amount shown as owed, not just the original gross charge.

CMS explains how to read these fields in its medical bill guide. Keep each bill with the EOB for the same provider and date so you can reconcile the patient share.

Check for duplicate or unsupported charges

Review each line against what happened during the visit and what is documented in the relevant records. CMS advises patients to check that they were not billed twice for the same service and that billed services are documented. If a billed item is absent from the records, ask the provider to identify where it is documented or correct the bill.

Compare potentially repeated entries across all bills for the episode, not only within one statement. For each pair, check:

  • Provider and date: Are they the same provider and service date, or are separate clinicians or facilities billing for different parts of care?
  • Description and code: Do the entries describe the same service or supply? Does the code description appear to fit what you received?
  • Record support: Is there a corresponding entry in the medical record?
  • Amounts: Are you comparing the gross charge, an adjustment, a prior payment, or the final patient responsibility?

Separate bills are not proof of duplicate billing. Two clinicians may bill distinct services during one encounter, and a code that looks unfamiliar does not by itself show an error or wrongdoing. Use a mismatch as a question for the provider rather than trying to decide clinical necessity from a code label.

CMS’s medical-bill error checklist recommends looking up code descriptions and contacting the provider if a code does not match the care received.

Reconcile an insured bill with the EOB

An EOB is the insurer’s explanation of how it processed a claim; it is not itself a provider bill. Compare the services and patient-responsibility amount on the EOB with the provider’s statement. If the amounts differ, ask the insurer whether the claim was reprocessed or an adjustment is pending, and ask the provider to explain the balance. Check that you have the EOB for the particular provider and date: separate notices may arrive for clinicians, facilities, or different days.

CMS recommends paying only after you receive a provider bill and check that the services and amount owed agree with the EOB. If you do not have the matching EOB, contact the insurer before treating a difference as an error.

Ask the billing office about specific line items

Call or write the billing office with the date, line description or code, and amount in question. Ask for a plain-language explanation and, where relevant, a correction. Questions such as these can make the review more concrete:

  • What service or supply does this line represent, and where is it documented?
  • Does this code describe the service performed?
  • Is this charge separate from another provider’s bill, or does it cover the same service?
  • How were the adjustment, insurer payment, and my prior payments applied?
  • Can you send the explanation or any correction in writing?
  • If I cannot afford the balance, what payment plans, reductions, or financial assistance are available?

Keep a record of the date, the person you spoke with, what they said, and any reference number. You can ask whether collection activity can be paused while the item is reviewed, but an ordinary billing question does not automatically pause collections. CMS’s provider conversation guide also recommends asking about payment plans, reductions, and financial assistance.

Know when federal surprise-billing protections may apply

The No Surprises Act took effect January 1, 2022. It protects people with most types of health insurance from certain unexpected out-of-network bills, including specified emergency services, certain non-emergency services connected with care at an in-network hospital, hospital outpatient department, or ambulatory surgical center, and air ambulance services. Whether a protection applies depends on the coverage and circumstances. A deductible that has not been met, by itself, does not establish a violation.

For people without insurance or who choose not to use it, providers generally must give a good faith estimate when care is scheduled at least three business days in advance or when the patient asks. This estimate process and the No Surprises Act do not automatically cancel every confusing or unexpected charge; ordinary bill errors still need to be raised with the provider and, for insured care, reconciled with the insurer. Consider the federal dispute process for an estimate gap

Uninsured or self-pay patients may qualify for the federal patient-provider dispute resolution (PPDR) process if a provider or facility charges at least $400 more than its good faith estimate and other eligibility conditions are met. CMS lists conditions that include having not had or not used insurance and having told the provider in advance, receiving care on or after January 1, 2022, possessing the written estimate, and having an initial bill dated within the previous 120 calendar days.

Starting the PPDR process carries a $25 non-refundable administrative fee; if the decision favors the patient, CMS says the fee is deducted from the amount owed. While an eligible bill is under this process, the provider or facility cannot move it into collections, threaten to do so, collect existing late fees, or take action against the patient because they are disputing it. This specific protection applies to the PPDR process, not every routine billing inquiry or insurance appeal.

Eligibility and process details can change. Check CMS’s current dispute guidance before filing.

Get help if the issue is not resolved

If the provider and insurer do not resolve the discrepancy, CMS points consumers to state Consumer Assistance Programs and patient advocates, who may help handle billing issues. You can also ask the provider about financial assistance or payment arrangements if the balance is correct but unaffordable.

Hospitals publish standard charges under federal Hospital Price Transparency rules, and comparing prices for a CPT code at nearby facilities may help with price research. A posted price comparison does not establish whether a past charge was billed correctly or what an insurer will ultimately allow.
