There is no single best Medicare Supplement (Medigap) insurance company for everyone. In most states, insurers offering the same plan letter provide the same standardized basic benefits, so the useful comparison is the current price and policy details for that letter in your state—not a national brand ranking. Compare quotes from multiple licensed insurers, check how premiums are priced and whether you qualify for discounts, and understand your enrollment rights before applying.
Why there is no universal best Medigap company
Medigap is private insurance that helps pay certain copayments, coinsurance, and deductibles for services covered by Original Medicare. It is not Medicare Advantage; generally, you need both Medicare Part A and Part B to buy a Medigap policy. In most states, a lettered plan’s standardized basic benefits are the same regardless of the company selling it. Medicare puts it plainly: “The benefits in each lettered plan are the same, no matter which insurance company sells it.” Massachusetts, Minnesota, and Wisconsin standardize Medigap differently, so shoppers there should use their state’s plan structure rather than assume the standard letter comparison applies. That standardization makes price and policy details central to a same-letter comparison, but it does not make every company interchangeable. Availability, premiums, pricing methods, discounts, underwriting, and policy terms can differ. The sources available do not establish a defensible national company ranking, current nationwide premiums, or a company with superior claims or service performance.

What to compare between insurers

Medicare advises shoppers that “Costs can vary widely between companies, so contact more than one company to get an estimate.” Its Medigap costs guidance also points shoppers to pricing methods and discounts as factors to consider. An estimate is not a substitute for a current quote from the insurer.
How to compare Medigap quotes in your state
- Choose the plan design first. Use Medicare’s plan-benefit comparison to identify the letter or design you want. Massachusetts, Minnesota, and Wisconsin have different standardization.
- Check your enrollment situation. Confirm your Part B start date, whether your one-time open enrollment period is active, and whether a guaranteed issue right or state protection applies.
- Find insurers that sell it locally. Use your state insurance department’s resources to identify licensed companies and ask about complaint information. State listings are local resources, not nationwide rankings.
- Request apples-to-apples quotes. Ask multiple insurers for the same plan letter and coverage design. Give each the same relevant applicant details, then compare premiums, pricing method, discounts, and policy terms.
- Review documents and get independent help if needed. Read the policy summary and resolve questions before applying. A local State Health Insurance Assistance Program (SHIP) offers free counseling; Medicare says SHIPs “aren’t connected to any insurance company or health plan.” Find guidance through Medicare’s How do I buy a Medigap policy? page.
State availability can change, and no state list should be treated as a national directory. For examples of regulator listings, the California Department of Insurance company list is stated as of August 2026, while the South Dakota Division of Insurance listing is stated as of May 2025. Neither determines which companies are available in another state.

How plan design changes the trade-off
A plan letter determines the standardized benefit design in most states; choosing among designs affects how much the policy covers and what you pay yourself. The federal Medigap Open Enrollment Period is generally a one-time six-month period that begins when you are 65 or older and enrolled in Part B. During this period, you can buy any Medigap policy sold in your state without an insurer denying coverage or changing the price because of health problems. Medicare’s overview describes the general rule.
After that period, an insurer may be able to use medical underwriting or deny an application unless a guaranteed issue right or state protection applies. Verify your dates and circumstances with Medicare, your state insurance department, or SHIP before dropping or switching coverage. Medicare’s buying guidance covers steps to take before enrolling.

What Medigap does not cover
Medigap generally helps with cost sharing for services covered by Original Medicare; it is not comprehensive coverage for services Medicare excludes. It generally does not cover long-term care, dental or vision care, hearing aids, glasses, or private-duty nursing. Policies sold after 2005 do not include prescription drug coverage, so a separate Part D plan may be needed for prescriptions. See Medicare’s explanation of what Medigap covers.
How to evaluate a company name you already have in mind
A familiar brand name is a starting point for a local quote, not proof that it is the best fit. For example, AARP’s comparison page identifies AARP-branded Medicare Supplement plans insured by specified UnitedHealthcare entities and notes that availability varies by state. That example does not establish comparative superiority or a recommendation for every location.
