There is no universal bottle-nipple size chart or required age-based schedule for moving up. Flow labels such as “newborn,” “slow,” “medium” and “fast” mean what each manufacturer says they mean within its own bottle system. Choose based on how your baby manages feeds, use responsive bottle-feeding, and ask a pediatrician or feeding specialist about persistent difficulties or special medical needs.
What bottle nipple sizes and flow labels mean
Manufacturers use their own names, numbers and age ranges for nipple flow. A “level 2” from one brand is not necessarily equivalent to a “medium” nipple from another, and the reviewed official guidance does not establish a standardized cross-brand flow-rate chart.
Package age ranges are not a universal timetable for changing nipples. A baby does not need to move to a faster flow simply because they have reached a particular age. The useful question is whether the baby can feed comfortably and coordinate sucking, swallowing and breathing—not whether a label says it is time to size up.
How to choose a flow for your baby
Babies vary, so there is no single nipple flow that suits every infant. The American Academy of Pediatrics (AAP) says trying different nipples and bottles is often a trial-and-error process. In some bottle-feeding situations, a slower-flow nipple may help a baby manage milk and coordinate swallowing and breathing. That makes slower flow an option to consider, not a diagnosis or a cure for gas, fussiness, reflux or other feeding problems.

If the milk seems to arrive too quickly or your baby has trouble organizing a feed, pause and discuss whether a slower-flow nipple might be appropriate. Avoid cycling through sizes indefinitely if feeding remains difficult. Coughing or choking, repeated struggles to coordinate feeding, unusually long feeds, or concerns that your baby is not feeding or growing as expected are reasons to seek individualized professional advice. They do not, by themselves, prove that a nipple size is wrong; the AAP guidance does not provide a validated symptom checklist for diagnosing a flow mismatch.
Use responsive bottle-feeding with any nipple
Nipple choice is only one part of a feed. The Centers for Disease Control and Prevention (CDC) advises holding the bottle at an angle so milk comes when the baby sucks, allowing breaks, watching for fullness cues and stopping when the baby is full—even if milk remains. As the CDC puts it, “Let your baby take breaks from drinking when they seem to want them.” Do not prop the bottle or force your baby to finish it. These steps keep the baby’s cues central rather than treating a size change as a substitute for feeding technique.

The AAP’s 2025 guidance on crying says that, when bottle-feeding, a slower-flow nipple and occasionally tipping the bottle to pause the flow may help a baby organize swallowing and breathing and recognize fullness. Its 2025 gas guidance recommends slowing liquid flow and says, “Trial and error with different bottles and nipples tends to be the best approach.” Neither source says every gassy, fussy or spitty baby needs a new nipple or that changing nipples will resolve those symptoms.
When thickened feeds change the decision
Prescribed thickened feeds are a special medical situation. AAP guidance on oatmeal for children who need thicker food says to consult the pediatrician or feeding specialist about the appropriate consistency. It notes that a larger or faster-flow nipple may be needed when a child drinks prescribed oatmeal-thickened food from a bottle. This advice applies to clinician-directed thickened feeds; it is not a reason to move other babies to faster flow. Follow the treating clinician’s current instructions, particularly because that AAP page was last updated in 2016. Do not cut, cross-cut or enlarge a nipple opening yourself.

Separately, the CDC advises not to put cereal or other solid foods in a bottle to make a baby sleep longer because doing so could increase choking risk. That general sleep strategy is different from a thickened feed prescribed for a medical reason.

If you are buying a replacement, choose one intended for the exact bottle system you already use. A label or number describes the manufacturer’s category, not a flow rate that can be assumed to match another brand. The AAP’s bottle-feeding page does not recommend one bottle brand over another based on claims such as preventing gas or imitating breastfeeding. In David L. Hill, MD, FAAP’s words in that page, last updated in 2012: “I haven’t seen good scientific literature that supports these claims, so I can’t recommend one brand over another.”
