- Reactivation: Select this to request reactivation and provide the requested reason or explanation.

What information does HQP-PFF-416 ask for?
Complete the member-identification fields, which include:
- Pag-IBIG MID number
- Name
- Email address
- Cellphone number
- Date of birth
It also includes a member certification, signature and date fields, as well as sections reserved for Pag-IBIG Fund use. Follow the form’s labels and complete the member sections relevant to your request.

How to prepare and submit the form
- Download HQP-PFF-416 from the official Pag-IBIG Fund PDF.
- Print and complete two copies. Pag-IBIG’s instructions say to accomplish the form in two copies and print entries in block or capital letters.
- Fill in your details and request. Enter the requested member information, select unlocking, deactivation, or reactivation, and provide the requested reason or explanation.
- Sign and date the form in the member certification section.
- Include a photocopy of one valid ID and submit the completed form to any Pag-IBIG Fund branch. The form’s instruction reads: “Submit the duly accomplished form together with photocopy of one (1) valid ID to any Pag-IBIG Fund Branch.”
What the form does not establish
HQP-PFF-416 specifies branch submission, two copies, and a photocopy of one valid ID. It does not state a processing time, fee, online-submission route, approval guarantee, or additional ID requirement.
