Chase’s app instructions describe a prefilled direct-deposit form, not a digital voided check. To get the form, sign in to Chase Mobile, open the account for deposits, swipe up, tap “Set up direct deposit form,” review the details, and tap “Create form.” You can then download, print, or email it.
Get Chase’s direct-deposit form in the app
- Open Chase Mobile and sign in.
- Tap the checking or savings account where you want deposits sent.
- Swipe up and tap “Set up direct deposit form.”
- Review the account information and update it if needed.
- Tap “Create form.”
- Download, print, or email the form, following the recipient’s submission instructions. Chase describes this as a prefilled direct-deposit form for giving an employer your deposit information. Chase’s mobile instructions explain the flow.
The app labels can change. If you do not see the option, check Chase’s current instructions or contact Chase for help.

Find account and routing numbers without creating a form
In Chase Mobile, sign in, tap the account tile, and choose “Show details.” On Chase’s website, sign in, open the account name, and click “Account & routing number.” Confirm you have selected the account intended to receive the deposit before sharing or entering its details. Chase provides these lookup steps.
If the employer asks specifically for a voided check
Do not assume the app form will satisfy a request that explicitly calls for a paper check. Ask the employer or other recipient whether it accepts Chase’s direct-deposit form, a signed authorization form with the numbers entered manually, or a voided deposit slip. The recipient decides which documents it accepts. Chase also offers a printable direct-deposit authorization form as an alternative, but it is not a digital voided check. Voiding a physical check
If the recipient requires an actual check and confirms no alternative is acceptable, write “VOID” across the front without covering the routing and account numbers. A voided check still displays sensitive identifying and account information, so keep it secure and shred it when it is no longer needed. See Chase’s guidance on voiding a check.
