Prioritize two broad, adjustable padded straps, a padded back, useful compartments, and a close fit. Then check the fully loaded bag on the student before buying.
What to look for in a backpack
Grade level and advertised capacity are not enough to determine whether a backpack will fit. Compare bags by how they sit on the student and how much they weigh when empty, since a lighter bag leaves more of the recommended weight allowance for books and devices.
- Fit: Choose a size proportional to the student’s height and torso. Have them try it on with a typical school load.
- Straps and back panel: Look for two broad, adjustable padded shoulder straps and a padded back. More padding by itself does not guarantee less pain.
- Compartments: Multiple sections make it easier to organize and balance items. Pack the heaviest books or computer closest to the student’s back.
- Visibility: Reflective details may improve visibility during low-light travel.
- School and commute: Check locker access and school rules, as well as the route between home and school, before choosing a standard or wheeled bag.
A useful shopping category is a school backpack with padded straps and compartments. These features are selection criteria, not evidence that a particular model has been tested or will prevent pain.
How to check the fit
The American Academy of Pediatrics (AAP) recommends a backpack that sits close to the body and centered on the back. It should extend from waist level to about one or two inches below the shoulders, with the bottom resting in the curve of the lower back. It should not hang more than four inches below the waistline.

- Load the pack with the supplies the student normally carries.
- Have the student put it on using both shoulder straps; adjust them so the bag rests close and centered on the back.
- Secure a waist or chest strap if the backpack has one.
- Place the heaviest materials closest to the back and balance contents across compartments.
- Check that the student can stand and walk without leaning forward to support the pack.
Keep the loaded backpack within the weight guideline
The AAP’s HealthyChildren.org guidance, updated August 27, 2026, recommends that a child’s backpack weigh no more than 15% of their body weight. For example, the AAP says a 100-pound child should carry no more than 15 pounds. If the student leans forward to support the bag, the AAP says it is too heavy. Treat this as published guidance—not as proof that one backpack model prevents pain.
To manage the load, remove items the student does not need, clean out the backpack weekly, and use lockers or other school storage where available. If the usual load pushes the pack over the guideline, reducing what is carried is more useful than relying on extra padding or a different style of bag alone.

When a wheeled backpack makes sense
A wheeled backpack may work for a long trip on a smooth route, but its wheels and frame add weight. It can be awkward on stairs and rough ground. Consider the student’s actual commute and confirm that the school allows wheeled bags before choosing one.

Backpacks have not been shown to cause scoliosis, and a backpack should not be presented as a way to prevent or treat it. The AAP’s guidance instead focuses on habits that can reduce back strain and pain, including choosing a suitable fit, carrying the pack correctly, and limiting its load.
