No shampoo is established by the evidence here as a reliable way to regrow hair lost to pattern hair loss. Ketoconazole shampoo is the most defensible shampoo to consider, but only as a possible adjunct with limited evidence—not as a proven regrowth treatment. Its cited U.S. 2% label is for dandruff and seborrheic dermatitis. If you are comparing products, focus on the labeled purpose, active ingredient, directions and scalp tolerance rather than a promise to regrow hair.
Can shampoo regrow hair?
Shampoo cleans hair and the scalp. The evidence reviewed here does not establish a shampoo as a dependable stand-alone treatment for common pattern hair loss. A product may help manage a scalp condition, but that is different from treating the cause of thinning or restoring lost hair.
That distinction matters when shopping: a product marketed for “hair loss” is not necessarily supported by clinical evidence for regrowth. The sources cited here do not establish a credible set of shampoos that can be ranked by regrowth effectiveness.
Does ketoconazole shampoo help with hair loss?
Ketoconazole has some human-study signals for androgenetic (pattern) hair loss, but the evidence is small and weak. A 2020 systematic review by Fields and colleagues screened 47 papers and included seven in its qualitative synthesis: five human studies involving 318 participants and two animal studies. The human studies reported increased hair-shaft diameter; one reported an increased pilary index, and some reported clinical improvement based on photographs or subjective assessments. The authors described topical ketoconazole as promising but concluded that “Randomized controlled trials are needed.” Read the systematic review.

An older 1998 study compared 2% ketoconazole shampoo with unmedicated shampoo, with or without 2% minoxidil. It reported improvements in hair density, size and the proportion of hairs in the growth phase in the ketoconazole and minoxidil regimens; the authors said the clinical significance awaited larger controlled study. That study alone cannot establish that ketoconazole shampoo reliably regrows hair or works as well as minoxidil. See the 1998 study.
The Japanese hair-loss guideline gives topical ketoconazole a C1 recommendation, meaning it is permissible, and characterizes the evidence as weak. It summarizes a non-randomized study in men and case series; it reports no clinical trials for female-pattern hair loss. See the Japanese guideline.

What the cited ketoconazole shampoo is labeled to do
The U.S. DailyMed label cited here is for ketoconazole shampoo 2% used for dandruff or seborrheic dermatitis—not hair regrowth. Follow the label for the specific product you buy: strength, availability and directions can differ. Check the 2% product label.
For the labeled scalp conditions, 11 double-blind trials involving 264 patients reported increased normal hair loss and irritation each in fewer than 1% of patients. Post-marketing reports also include alopecia and other skin or hair reactions. These safety figures concern use for the labeled conditions; they do not show that the shampoo treats hair loss. Stop using a product and seek advice from a clinician or pharmacist if you develop a concerning reaction.

How to choose a shampoo when hair is thinning
Use a shampoo for the job its label supports. For a scalp-care product, compare the following rather than relying on a broad “regrowth” claim:
- Active ingredient and labeled purpose: distinguish a medicated anti-dandruff shampoo from a cosmetic cleanser. An ingredient list alone does not establish hair-regrowth effectiveness.
- Strength and directions: check the actual package or product label; do not assume all ketoconazole products have the same concentration or instructions.
- Evidence for the claim: ketoconazole’s evidence for pattern hair loss is limited and should not be treated as equivalent to evidence for established hair-loss medicines.
- Scalp tolerance: the cited 2% label lists possible irritation, dryness, itching and other reported reactions. Consider whether the product suits your scalp and follow its directions.
- Whether it fits the likely cause: persistent or patterned thinning may need diagnosis. A dandruff shampoo is not a substitute for evaluating ongoing hair loss.
What treatments have stronger support for pattern hair loss?
Treatment depends on the type and cause of hair loss. The American Academy of Dermatology (AAD) identifies topical minoxidil as an FDA-approved treatment for male-pattern hair loss. It is applied to the scalp, not used as shampoo. The AAD says it can reduce loss, stimulate growth and strengthen existing strands; it may take six to twelve months to see results, and “you’re unlikely to see full regrowth.” Those statements concern minoxidil and male-pattern hair loss, not shampoo or every cause of shedding. When should you get hair loss evaluated?
Consider a medical evaluation if thinning is persistent, follows a pattern, or concerns you. Hair loss can have different causes, and a treatment that fits one diagnosis may not fit another. A clinician can help identify the cause and discuss options suited to your situation rather than relying on a shampoo’s marketing claim.
FDA adverse-event reports are also worth interpreting carefully. By 2016, the FDA had received more than 1,400 reports concerning certain cleansing conditioners, including reports of significant hair loss, breakage, balding, itching and rash. The agency commissioned a Columbia University study to investigate possible connections. Reports are signals for monitoring, not proof that a particular product caused an effect, and this information should not be generalized to every shampoo or cleansing product.
