Best Medicare Advantage Plans and Providers of 2025—Compared by What Matters Most

Direct answer: There was no single best Medicare Advantage plan nationwide in 2025. The right choice depended on the beneficiary’s ZIP code and county, doctors, hospitals, prescriptions, preferred pharmacies, budget, and eligibility for programs such as Medicaid or a Special Needs Plan. National providers—including Humana, UnitedHealthcare, Aetna, Blue Cross Blue Shield affiliates, and Kaiser Permanente—were reasonable places to start, but Medicare.gov’s personalized Plan Compare tool was the decisive source for actual 2025 availability, premiums, networks, formularies, benefits, and Star Ratings.

This guide covers the 2025 contract year. Medicare Advantage information changes annually, so readers enrolling or switching after 2025 should use the current-year information on Medicare.gov rather than simply updating the year in this article.

What “best” meant for Medicare Advantage in 2025

Medicare Advantage, also called Medicare Part C, is offered by private Medicare-approved insurers. Plans cover Medicare Part A and Part B services and usually include Part D prescription-drug coverage. In exchange for using the plan’s rules and network, a beneficiary may receive benefits that Original Medicare does not routinely include, such as some dental, vision, hearing, fitness, transportation, meal, or over-the-counter allowances.

Those benefits can be valuable, but they do not make one insurer universally superior. Two plans sold by the same company in neighboring counties may have different premiums, doctors, hospitals, drug formularies, copayments, supplemental benefits, and quality ratings. Even a plan with a $0 monthly premium can still involve the Medicare Part B premium, deductibles, copayments, coinsurance, drug costs, and substantial medical cost-sharing before reaching its annual out-of-pocket limit.

The most defensible way to use national provider lists is as a shortlist—not a final ranking. A plan is worth considering only after it passes five local tests:

  • Your primary-care doctor, specialists, preferred hospital, and other important facilities participate.
  • Your prescriptions are covered at acceptable tiers and your preferred pharmacy receives favorable pricing.
  • The premium and expected cost-sharing fit your budget.
  • The plan’s annual medical out-of-pocket maximum is acceptable for a bad year, not just an average year.
  • The plan’s supplemental benefits and rules are useful to you rather than merely attractive in an advertisement.

The 2025 Medicare Advantage market at a glance

Choice varied sharply by location. According to KFF’s 2025 market analysis, the average beneficiary could choose among 42 individual Medicare Advantage plans, including an average of 34 individual Medicare Advantage prescription-drug plans. These averages generally excluded employer or union group plans and excluded Special Needs Plans from the core comparison.

2025 market fact Why it matters
42 individual Medicare Advantage plans on average The average hides substantial county-to-county variation; your ZIP code may produce far fewer or far more choices.
34 Medicare Advantage prescription-drug plans on average Drug coverage is not interchangeable. Formulary tiers, prior authorization, quantity limits, and pharmacy networks can change the best-value choice.
Plans from about eight firms on average National brands may dominate some markets, while regional or provider-sponsored plans may be more competitive in others.
Nearly one-third of beneficiaries lived in counties with more than 50 individual plans A large menu makes personalized filtering more important; the most familiar brand is not necessarily the best local option.
No individual Medicare Advantage plans were offered in Alaska for 2025 National rankings cannot replace a location-specific availability check. A small number of other counties also had no individual MA offering.
$17.00 projected average monthly premium in 2025, including plans with prescription coverage This was a market average, not a quote for a particular beneficiary. The Medicare Part B premium generally still applied separately.
About 40% of MA-PD contracts earned an overall Star Rating of four stars or higher Star Ratings are useful for screening plan performance, but they do not confirm that your doctors, drugs, pharmacy, or preferred hospital are covered.

The $17.00 average premium was down from $18.23 in 2024, according to CMS projections. The figure should not be treated as an estimate of total healthcare spending: premiums, medical copays, drug costs, and out-of-pocket exposure vary by plan and by how much care a person uses. See the KFF 2025 Medicare Advantage market analysis and CMS’s 2025 premium and program fact sheet for the underlying market figures.

Best Medicare Advantage providers by shopper priority

The categories below identify providers and plan types worth investigating for a particular need. They are not nationwide quality rankings. The actual plan contract available in your county must be evaluated separately.

Best starting point for broad availability and plan variety: Humana

Humana was one of the two largest Medicare Advantage organizations nationally in 2025 and offered plans in nearly 90% of U.S. counties. Its portfolio included HMOs, PPOs, private fee-for-service plans, and Special Needs Plans. That variety makes Humana worth checking when a shopper wants several plan structures, broad market availability, dental or vision packages, hearing benefits, or a possible SNP option.

Humana’s availability was not static: it entered some counties and exited others for 2025, and its plan count, provider network, premium, benefits, and Star Rating differed by location. Do not assume that a Humana plan found in one county is available—or has the same terms—in another.

Best starting point when a large national insurer and provider access matter: UnitedHealthcare

UnitedHealthcare was the largest Medicare Advantage parent organization by enrollment in 2025. Together, UnitedHealthcare and Humana accounted for nearly half of national Medicare Advantage enrollment, according to KFF. That scale can make UnitedHealthcare a logical comparison when a beneficiary wants a major national insurer or is evaluating multiple local plan designs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Scale is not proof that a particular doctor or hospital participates. UnitedHealthcare also changed its county footprint for 2025. Search for each important physician, hospital, specialist, rehabilitation facility, and pharmacy in the specific plan’s directory, and confirm participation with the provider before enrolling.

Worth comparing for supplemental benefits and CVS-linked options: Aetna Medicare

Aetna Medicare is part of CVS Health. Depending on the state and county, Aetna plans may offer combinations of dental, vision, hearing, fitness, prescription, and other supplemental benefits. Aetna can therefore be a reasonable comparison for people who place significant value on extra benefits or on care options connected to the CVS Health ecosystem.

Those features are plan-specific. A benefit may have an annual dollar limit, a participating-provider requirement, an authorization rule, an eligibility restriction, or a narrow definition of covered services. CVS pharmacy access or CVS-owned clinical assets also do not automatically mean that every provider or service is in-network. Verify the precise plan documents and provider directories.

Best local-network starting point in markets with a strong Blue plan: Blue Cross Blue Shield affiliates

Blue Cross Blue Shield is a federation of independent or separately operated affiliates, not one uniform national Medicare Advantage plan. A local Blue affiliate may be especially worth checking when it has established relationships with the doctors, specialists, and hospitals a beneficiary already uses.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Benefits, premiums, networks, formularies, and Star Ratings must be evaluated through the relevant local affiliate and plan contract. A national impression—or a rating associated with one Blue plan—should not be used to characterize every Blue Cross Blue Shield Medicare Advantage offering.

Best integrated-care option where available: Kaiser Permanente

Kaiser Permanente may appeal to beneficiaries who live in one of its service areas and prefer coordinated care within an integrated healthcare system. Its concentrated regional availability can be a strength for someone comfortable receiving care through Kaiser’s network and system.

It may be a poor fit for someone who wants broad access outside the Kaiser network, frequently travels, lives outside a Kaiser service area, or already relies on independent doctors who do not participate. Confirm service-area eligibility and the specific plan’s rules before treating Kaiser as an option.

Best potential fit for a specific county, hospital system, or eligibility group: Regional plans and Special Needs Plans

A smaller regional insurer or provider-sponsored plan can be a better local fit than a national brand if it includes the beneficiary’s preferred hospital, specialists, transportation benefits, care-management programs, or Medicaid coordination.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Special Needs Plans, or SNPs, are designed for eligible groups, including people who qualify for both Medicare and Medicaid, people with certain qualifying chronic conditions, and some people living in institutions. Eligibility, covered benefits, enrollment rules, and coordination with Medicaid must be confirmed directly. The average plan counts above generally excluded SNPs, so they should not be overlooked simply because they do not appear in the standard market averages. Medicare.gov explains the main plan options in its plan comparison guidance.

How to compare the actual 2025 plans available to you

1. Start with your ZIP code and county

Open Medicare.gov Plan Compare and enter your ZIP code. Confirm the county and enrollment circumstances when prompted. Plan availability, premiums, drug coverage, supplemental benefits, and networks are local, and a national provider’s website may display multiple plans that do not all serve your address.

Make a shortlist of plans rather than choosing the first $0-premium result. Save the plan name, contract or plan identifier when shown, monthly premium, deductible, medical out-of-pocket maximum, drug costs, Star Rating, and the documents associated with each finalist.

2. Decide whether the plan structure fits your provider habits

Medicare Advantage plans commonly use different network structures:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • HMO: Generally places stronger limits on using out-of-network providers and may involve referrals or a primary-care coordination process. Emergency and urgent-care rules are separate; read the plan’s terms.
  • PPO: Generally permits some out-of-network care at a higher cost, but the provider must still meet the plan’s rules for coverage. Out-of-network costs can be materially higher.
  • Private fee-for-service: May have different provider-acceptance rules. A provider’s willingness to treat a patient is not the same as automatic participation, so confirm before receiving care.
  • Special Needs Plan: Uses a plan structure designed for an eligible population and may coordinate Medicare and Medicaid or condition-specific care.

Do not rely only on the insurer’s general “national network” language. Search the directory for your primary-care physician, every regular specialist, preferred hospital, outpatient facility, rehabilitation facility, behavioral-health provider, and pharmacy. Then call the provider’s office and ask whether it accepts the exact plan name for the coming plan year. Directories can be incomplete or change, so retain the date and person you contacted.

3. Enter every prescription, dosage, and preferred pharmacy

Use Medicare.gov’s drug comparison feature to enter each regular prescription, including the dosage and frequency when requested. Check the plan’s formulary and drug tiers, but also examine:

  • Prior authorization requirements
  • Step-therapy rules
  • Quantity limits
  • Specialty-drug handling
  • In-network and preferred pharmacies
  • Mail-order pricing
  • Estimated annual drug spending

The 2025 Part D redesign capped out-of-pocket costs for covered Part D drugs at $2,000. That cap did not make every prescription free, did not necessarily include premiums or noncovered drugs, and did not remove formulary or utilization-management rules. A plan with the lowest premium can still be a poor choice if it places an important medication on an expensive tier or requires a difficult authorization process. The Medicare drug coverage guide provides the relevant 2025 framework.

4. Compare total medical exposure—not just the monthly premium

Look at the medical deductible, specialist and hospital copays, coinsurance, outpatient surgery costs, diagnostic imaging, emergency care, ambulance coverage, physical therapy, durable medical equipment, and the plan’s annual medical out-of-pocket maximum.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Medicare Advantage plans have a yearly limit for covered Medicare services. After the beneficiary reaches the plan’s applicable limit, the plan generally pays 100% of covered services for the rest of the calendar year, subject to the plan’s rules. PPOs may show different in-network and out-of-network limits, and not every service or premium belongs in the medical out-of-pocket calculation. Check the Summary of Benefits and Evidence of Coverage rather than assuming one number covers every possible expense. Medicare’s explanation of costs is available through its Medicare costs guidance.

A useful comparison is to model two scenarios for each finalist:

  1. Expected year: Add premiums, routine specialist visits, prescriptions, therapy, planned procedures, and recurring copays.
  2. Bad year: Test the plan using the annual medical out-of-pocket maximum, likely hospital or outpatient costs, drug exposure up to the Part D limit, premiums, and services that may fall outside the plan’s covered categories.

5. Use Star Ratings as a filter, not a final verdict

CMS Star Ratings evaluate multiple dimensions of Medicare Advantage and prescription-drug plan performance, including areas such as care coordination, access to care, member experience, and prescription-drug performance. About 40% of MA-PD contracts offered in 2025 received an overall rating of four stars or higher.

A high Star Rating is a useful reason to investigate a plan. It is not a substitute for confirming your doctor, hospital, prescriptions, pharmacy, premium, and cost-sharing. Ratings can apply at the contract or plan level, and a plan’s rating may change for a later year. Review the details through CMS’s 2025 Medicare Advantage and Part D Star Ratings information.

6. Treat extra benefits as conditional value

Dental, vision, hearing, transportation, fitness, over-the-counter, meal, and allowance benefits can influence the decision—but only after you check their restrictions. For each extra, ask:

  • What exact service or product is covered?
  • What is the annual limit, allowance, or number of visits?
  • Must the provider participate in a separate benefit network?
  • Is prior authorization required?
  • Does the benefit apply to all members or only people who meet a health or eligibility criterion?
  • Does unused allowance expire?
  • Are there exclusions, waiting periods, or frequency limits?

For example, a dental allowance may not cover every procedure a dentist recommends, and a transportation benefit may apply only to approved medical appointments. Read the plan’s benefit documents and the relevant Medicare plan-cost guidance before assigning a dollar value to an advertised extra.

7. Compare Medicare Advantage with Original Medicare before enrolling

Medicare Advantage is not automatically the best form of Medicare coverage. Compare it with Original Medicare, potentially paired with a Medigap policy and a stand-alone Part D plan, if any of these priorities matter:

  • You want broader provider choice or care from specialists who do not participate in local Medicare Advantage networks.
  • You travel frequently or spend extended periods outside your plan’s service area.
  • You prefer Original Medicare’s provider-access rules over a plan’s network, referral, or authorization requirements.
  • You want to evaluate a different approach to predictable cost-sharing.

Original Medicare generally provides broader provider choice, but it does not have the same built-in annual out-of-pocket limit for covered services unless supplemental coverage is added. Medicare Advantage may provide an annual limit and bundled benefits, but it operates through plan networks and plan rules. The government’s Original Medicare versus Medicare Advantage comparison can help frame the trade-off.

Enrollment timing and the 2025 plan-change warning

The Medicare Annual Enrollment Period ran from October 15 through December 7, 2024, for coverage beginning in 2025. During the Annual Enrollment Period, eligible beneficiaries could generally join, switch, or leave Medicare Advantage or Part D coverage for the following year. Medicare’s open-enrollment guidance explains the applicable rules and dates.

Annual review mattered especially in 2025. Approximately 5% of people enrolled in individual MA-PD plans were in a plan terminated for 2025, and as many as 7% could have been affected by plan consolidation. A plan that continued under a similar name could still have changed its premium, formulary, network, benefits, or cost-sharing.

Read the plan’s Annual Notice of Change as soon as it arrives. Compare it with your medications, doctors, planned care, and budget. If a plan terminates or materially changes, do not assume that the replacement option from the same insurer is equivalent. Re-run the comparison on Medicare.gov and save the new plan documents.

  1. Eliminate network failures. Remove any plan that excludes a doctor, hospital, specialist, or facility you are unwilling or unable to replace.
  2. Eliminate drug failures. Remove plans that do not cover essential prescriptions acceptably or impose unmanageable authorization, tier, or pharmacy restrictions.
  3. Check eligibility. Confirm whether an SNP or Medicaid-coordinated plan is available and whether you qualify.
  4. Compare worst-case medical costs. Examine the applicable annual out-of-pocket maximum and high-cost services.
  5. Compare likely annual spending. Add premiums, medical cost-sharing, prescriptions, and recurring services.
  6. Use quality and extras as tie-breakers. Compare Star Ratings and supplemental benefits only after the fundamentals work.
  7. Confirm everything. Check the plan’s Summary of Benefits, Evidence of Coverage, provider directory, formulary, and the provider’s office before submitting an enrollment request.

Optional resources for additional help

A current general reference such as Medicare for Dummies 2025 can help a beneficiary or caregiver understand Medicare terminology before comparing local plans. It is educational background, not a substitute for Medicare.gov: premiums, networks, formularies, and benefits change annually, and a print book may not contain the current plan details.

Optional commercial help: After checking Medicare.gov, some readers may prefer to compare Medicare Advantage plans with a licensed agent through SelectQuote or use a Medicare Advantage marketplace such as GoHealth. These are private services, not official Medicare comparison tools. They may not represent every plan or carrier, and compensation may apply. Ask which insurers and plans are represented, whether the agent is licensed in your state, and whether the information matches the plan shown on Medicare.gov.

Beneficiaries and caregivers who need help with eligibility, Medicaid coordination, networks, or formularies can also look for free state health-insurance counseling or a credentialed Medicare plan counseling service. Distinguish government counseling from broker assistance and ask how the counselor is paid before relying on a recommendation.

Common mistakes to avoid

  • Calling a carrier “the best” nationwide: Medicare Advantage is county-specific.
  • Assuming a national network includes your doctor: Search for the exact plan and confirm with the office.
  • Comparing premiums without context: State the county, plan type, enrollment category, and whether drug coverage is included.
  • Treating a $0 premium as free coverage: Part B premiums and other cost-sharing generally still apply.
  • Assuming 2024 details carried into 2025: County availability, Star Ratings, formularies, networks, and benefits can change.
  • Choosing based on marketing alone: Broker and insurer claims are not independent evidence of quality or value.
  • Ignoring Original Medicare: Provider freedom, travel, specialist access, and predictable costs may justify a broader comparison.

Frequently Asked Questions

Is a $0-premium Medicare Advantage plan free?

No. A $0 plan premium does not eliminate the Medicare Part B premium or other possible costs, including deductibles, copayments, coinsurance, prescription-drug costs, and expenses up to the plan’s medical out-of-pocket maximum.

Which Medicare Advantage provider was best in 2025?

No provider was best for every beneficiary. Humana and UnitedHealthcare were broad national starting points; Aetna, Blue Cross Blue Shield affiliates, Kaiser Permanente, regional insurers, provider-sponsored plans, and Special Needs Plans could be better fits in particular counties or for particular needs.

Does a high Medicare Advantage Star Rating mean the plan is right for me?

Not necessarily. Star Ratings measure plan performance across several dimensions, but they do not confirm that your doctors, hospital, prescriptions, pharmacy, premium, or preferred benefits fit your circumstances.

Can I keep my doctors with Medicare Advantage?

Only if they participate in the specific plan or the plan’s rules otherwise cover their services. Search each provider in the plan directory and confirm participation with the provider’s office before enrolling. HMO plans generally impose stronger network restrictions, while PPO plans may offer out-of-network care at higher cost.

Should I choose Medicare Advantage or Original Medicare?

It depends on your priorities. Medicare Advantage may bundle drug coverage, an annual medical out-of-pocket limit, and supplemental benefits, but it uses plan networks and rules. Original Medicare generally offers broader provider choice, but it does not have the same built-in annual out-of-pocket limit unless supplemental coverage is added.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The Bottom Line

The best Medicare Advantage plan of 2025 was the one that worked in your county, included your doctors and prescriptions, provided acceptable access to hospitals and pharmacies, and kept both expected and worst-case costs within your budget. Humana, UnitedHealthcare, Aetna, Blue Cross Blue Shield affiliates, Kaiser Permanente, and regional or specialized plans were sensible starting points—not final answers. Use Medicare.gov to compare the exact local plans, read the Annual Notice of Change and plan documents, and verify providers and drugs before enrolling.
