Treat an AI-generated workers’ compensation form as an unverified draft. Before signing or filing it, match the draft to the current form and instructions for the state handling the claim, verify every factual entry against original records, and confirm who must complete each section, where the form goes, and how to keep proof of submission.
1. Identify the state and the purpose of the form
Workers’ compensation forms and filing routes are state-specific. First identify the state agency handling the claim and the exact document being prepared. A claimant form, employer report, and filing to a workers’ compensation board may have different sections, recipients, and procedures. Do not assume that a form number or filing method used in one state applies elsewhere.
For example, California’s guidance concerns an injured worker’s claim form, Georgia identifies Form WC-14, Massachusetts describes a route for an unrepresented worker to self-file, and Pennsylvania distinguishes among forms and electronic data interchange requirements. Consult the agency that handles this claim rather than relying on an AI-generated explanation of another state’s process.
2. Match the draft to the current official form
Find the form on the responsible agency’s website and compare its title, form number, revision, and instructions with the AI draft. Check for changed fields, required attachments, and current filing directions before transferring any information. Iowa warns that using the wrong agency form for certain filings may result in rejection. New York periodically updates forms and may announce when older versions will no longer be accepted.

3. Verify every factual entry against original records
Do not check one AI-generated passage against another. Compare each entry with reliable records, such as the worker’s own documents, employer records, claim correspondence, or insurer information. Pennsylvania’s agency specifically advises filers to double-check details including employee name and address, Social Security Number, employer name and address, insurance information, injury date, and injury description.
- Worker details: Check the legal name and current address against the worker’s records.
- Employer details: Confirm the employer’s name and address from reliable employment or claim records.
- Insurance and claim identifiers: Verify carrier information and enter the correct kind of claim number in the field that requests it. Pennsylvania distinguishes its bureau claim number from the carrier’s claim number; its guidance says to put the carrier number only where the form asks for it. - Injury details: Check the date and description against original records and the worker’s account. Do not accept an AI’s added detail or changed wording as evidence.
If a required fact is unknown or disputed, do not guess or let AI fill the gap. Follow the form instructions or ask the responsible agency or a qualified representative how to proceed.

4. Confirm who completes each section
Check the form’s instructions to make sure the right person completed the right parts. A form may separate information for the worker, employer, insurer, or representative. For instance, California tells the injured worker to complete only the employee section of its claim form and send it to the employer. That is California-specific guidance, not a nationwide rule.
5. Check signature, date, recipients, attachments, and filing method
Before submission, compare the completed draft with the agency’s directions for that form. Confirm any required signature and date, the destination, who must receive copies, and whether supporting records are needed. Filing procedures differ by state and document.

- California tells workers to sign and date the claim form, return it promptly to the employer, and keep a copy. It suggests certified mail if mailing so the mailing and receipt dates can be documented.
- Georgia says Form WC-14 is filed with the State Board and a copy is sent to the employer and insurer.
- Massachusetts’s instructions for its self-filed claim route describe copies and supporting documents.
- Pennsylvania describes upload, electronic, and paper routes for various forms; its guidance also explains that electronic data interchange alone does not satisfy every form-filing requirement.
These examples illustrate why the agency’s instructions for the particular state and form control. Do not transfer one state’s recipients, signature practice, or filing route to another.
6. Inspect the saved or printed form before sending it
Open the final file and check that the entries are visible, legible, and present on every relevant page. New York warns that entered data may not transmit unless the completed form is printed to PDF and saved, and provides instructions for opening a saved form. Iowa’s instructions for its forms say to use Adobe Acrobat for electronic completion and to flatten completed PDF forms before e-filing. Apply these steps only where the relevant agency directs them.

- Verify that the saved or printed PDF contains every entry and page.
- Check that text is not clipped, missing, or obscured by a form field.
- Keep a copy of the final version and any proof of filing or delivery, as the applicable agency instructions allow or require.
7. Pause when AI changes facts or interprets legal meaning
An AI-generated explanation is not proof that an event occurred or that a legal requirement applies. Do not adopt a proposed factual change, waiver, or legal interpretation merely because the system suggested it. If the official instructions do not resolve the question, ask the responsible agency or a qualified representative. The cited agency materials establish state-specific filing instructions; they do not establish a general AI-specific legal standard for reviewing workers’ compensation forms.
