The Best Pacifiers for Babies: How to Choose Safely by Age

No evidence-backed “12 best” pacifier ranking is established. Compare two Philips models with verified age labels and learn how to choose, use, and inspect pacifiers safely.

There is no verified “12 best” ranking supported by comparative testing here. Only two models have confirmed manufacturer specifications: Philips Avent Soothie SCF190/01, labeled for ages 0–3 months, and Philips Avent Super Soothie SCF192/10, labeled for 3–18 months. They are examples to compare—not proven winners. Choose by the package’s age range, construction, cleaning directions, and whether your baby accepts it; no pacifier shape is established as best for every baby.

What to look for in a pacifier

Start with the age range on the current package and use the product only as directed. Then compare the features that affect fit, handling, and care. Babies may accept different nipple shapes, so preference can be individual rather than a sign that one design is superior.

  • Construction and material: Check whether the pacifier is one piece or assembled and what material the manufacturer specifies.
  • Shield: Look for a shield designed to prevent the whole pacifier from being swallowed and to provide ventilation, as required by U.S. safety rules.
  • Care: Follow the manufacturer’s cleaning and replacement instructions, including any expiration date.
  • Fit and acceptance: Use an age-appropriate model and do not force a pacifier on a baby who refuses it.

When to offer a pacifier

The American Academy of Pediatrics (AAP) recommends offering a pacifier at naps and bedtime. Its safe-sleep guidance reports a protective effect even if the pacifier falls out after the baby falls asleep. For a breastfed baby, wait until breastfeeding is going well—usually around 3–4 weeks—before introducing one, according to HealthyChildren’s AAP guidance, updated January 20, 2026.

  • Do not use a pacifier to replace a feed or postpone feeding when your baby is hungry.
  • Do not force it if your baby refuses it.
  • Do not attach it to a crib, your baby’s neck or hand, or a cord. U.S. rules prohibit selling pacifiers with specified cord-like attachments.

How to check pacifier safety

Inspect the pacifier regularly and follow its package instructions. HealthyChildren’s pacifier safety guidance advises replacing one if it is damaged, torn, or has a change in material, and not using it beyond a manufacturer-set expiration date. Never use a bottle nipple as a pacifier.

The U.S. Consumer Product Safety Commission explains that pacifier shields must be designed to prevent swallowing the whole pacifier and provide ventilation. Do orthodontic pacifiers prevent dental problems?

No reliable basis in the American Academy of Pediatric Dentistry (AAPD) policy establishes orthodontic-labeled pacifiers as better than conventional designs for preventing malocclusion. The AAPD says evidence is insufficient to recommend one design over the other for that purpose. Treat “orthodontic” as a product description, not a guarantee of dental protection. The AAPD’s Policy on Pacifiers, revised in 2024 and included in its 2026–2027 Reference Manual, emphasizes that duration of use matters.

When to discuss stopping

Guidance describes increasing concerns with prolonged use, but it is not a single universal deadline. The AAPD says use after 12 months may raise the risk of acute otitis media, use beyond 18 months may influence developing oral structures, and it encourages discontinuing nonnutritive sucking by 36 months. HealthyChildren notes that strong sucking beyond roughly ages 2–4 may affect mouth shape or tooth alignment. These are potential risks, not a prediction that every child will develop a problem. Ask your child’s pediatrician or pediatric dentist about the timing that fits your child’s circumstances.
