Use the tax year named in your Social Security Administration (SSA) notice when checking an existing IRMAA determination; SSA usually uses tax information from two years before the premium year, but may use information from three years before.
Calculate IRMAA MAGI from Form 1040
For this Medicare calculation, MAGI is not a broad tax-planning definition. SSA specifies a two-line calculation:
IRMAA MAGI = Form 1040, line 11 AGI + Form 1040, line 2a tax-exempt interest
- Find the Form 1040 for the tax year SSA used, or says it used, in its notice.
- Copy the AGI from line 11.
- Copy tax-exempt interest from line 2a.
- Add the two amounts. The result is the MAGI to compare with the threshold for the applicable year and filing status.
SSA’s IRMAA MAGI guidance defines the calculation, and the current Form 1040 referenced in SSA-44 uses those lines. Do not substitute a MAGI formula from another tax or benefits program. The reviewed SSA guidance does not establish a general exclusion for one-time income items: the calculation starts with reported AGI, then adds tax-exempt interest.

Use the tax year SSA used
SSA uses the most recent IRS tax information it can obtain. The standard lookback is two years: for example, SSA says 2026 premiums generally use 2024 tax-return information. In some cases, when newer information is unavailable, it may use information from three years before the premium year. For an existing determination, follow the tax year shown in the SSA notice rather than assuming your latest filed return is the one used.
When reviewing or comparing IRMAA calculations, keep these details together:

- the premium year and tax year SSA used;
- the filing status on that return;
- line 11 AGI;
- line 2a tax-exempt interest; and
- the resulting MAGI compared with the threshold for that year and filing status.
Compare MAGI with the 2026 threshold
SSA’s policy table lists the following initial MAGI thresholds for 2026:

| Married filing jointly | $218,000 |
| All other filing statuses | $109,000 |
These are the starting thresholds above which IRMAA applies, not the adjustment amount itself. The adjustment varies by income tier and coverage. Use SSA’s current IRMAA table and rules for premium amounts and details. Married filing separately has special rules, so consult SSA’s detailed table instead of assuming the general “all other” threshold tells the whole story.

Ask SSA to review IRMAA after a qualifying life event
If an IRMAA notice reflects older income and your income has fallen after a qualifying life-changing event, you can ask SSA for a new initial determination using Form SSA-44. Events listed by SSA include:
- marriage, divorce, or annulment;
- death of a spouse;
- work reduction or stoppage;
- loss of income-producing property;
- loss of pension income; and
- receipt of an employer settlement payment.
Form SSA-44 and its instructions ask for MAGI and filing-status information for a more recent tax year, as well as evidence of the income and life-changing event. The appropriate year depends on when income dropped and whether it will be lower in the premium year or the preceding year. The instructions explain when an estimate may be used and when a filed return is later needed as evidence. Follow the form instructions for your circumstances or contact SSA.
