UnitedHealth’s Recovery From the Change Healthcare Cyberattack: What Was Restored and What Remained Unclear

UnitedHealth restored Change Healthcare services in stages after the 2024 ransomware attack, but its 2025 filing estimated about 190 million people were affected and reported continuing costs.

UnitedHealth reported that key Change Healthcare services—including pharmacy functions and electronic payments—were restored in stages after the February 2024 ransomware attack. But the company did not provide a single independently verified date showing that every service and downstream effect had returned to normal. Its later disclosures also showed that the incident’s consequences extended beyond the initial outage: a 2025 filing estimated about 190 million people were affected and reported substantial costs tied to disrupted care-management activities.

What happened in the Change Healthcare cyberattack?

In testimony to the Senate Finance Committee on May 1, 2024, UnitedHealth Group CEO Andrew Witty said the ALPHV/BlackCat ransomware group deployed ransomware inside Change Healthcare’s IT environment on February 21, encrypting systems and preventing access. UnitedHealth said it isolated the affected systems after detecting the attack and contacted the FBI.

Witty testified that the company had no evidence the attackers spread from Change Healthcare to Optum, UnitedHealthcare or UnitedHealth Group. That is the company’s stated assessment, not proof that no other systems or information were ever affected.

Which services came back, and when?

UnitedHealth described a phased restoration rather than one all-at-once reopening. The dates below are milestones the company announced; they do not establish that every customer, connection or downstream workflow was back to normal on those dates.

March 7, 2024 The company said pharmacy electronic prescribing, claim submission and payment transmission were functioning. It scheduled electronic-payment connectivity for March 15 and medical-claims testing and connectivity for March 18.
March 15, 2024 UnitedHealth later reported that its electronic-payments platform had been restored.
March 18, 2024 The company said medical-claims preparation software was beginning to roll out to customers in phases. It said third-party attestations were planned before operational use.
April 22, 2024 UnitedHealth said restoration of affected products and services continued to make strong progress, while its review of potentially exposed data remained underway.

In its March 18 update, UnitedHealth also said 99% of pharmacy-network services had been restored by March 7. That is the company’s measure of pharmacy-network services, not a claim that every pharmacy transaction or related business process was operating without disruption.

Did the outage affect provider payments?

Yes. Disruption to claims and payment systems created cash-flow pressure for providers awaiting reimbursement. By March 18, UnitedHealth said it had advanced more than $2 billion to providers through multiple initiatives, including no-cost assistance. The figure describes advances reported by the company at that point; it is not a measure of total losses or a guarantee that every affected provider received funding.

How many people were affected by the data breach?

UnitedHealth Group’s 2024 Form 10-K, filed February 27, 2025, estimated that approximately 190 million individuals were impacted by the Change Healthcare cyberattack. The filing also said the company was not aware of misuse of the information at that time. The estimate is the company’s reported figure; it does not mean that each person’s information was confirmed to have been misused.

Earlier, on April 22, 2024, UnitedHealth said targeted sampling had found files containing protected health information (PHI) or personally identifiable information (PII). The company said its review would take months and that the announcement was not yet an official breach notification. It offered impacted people two years of free credit monitoring and identity-theft protection through a dedicated call center. The April announcement did not establish that every potentially affected person had already received an individual notice.

What were the longer-term costs and effects?

In the same 2025 filing, UnitedHealth reported approximately $640 million in medical costs associated with temporarily suspended care-management activities. This is a company-reported cost tied to those activities, not a complete measure of all financial, operational or personal harm caused by the attack.

The disclosures show why a service-restoration milestone is not the same as a full recovery. Claims processing, payment transmission, pharmacy workflows, data analysis, consumer notifications and care-management activity can have different timelines and consequences. The available company reports do not give one independently verified date on which every Change Healthcare service and downstream effect returned to baseline.

What oversight followed the attack?

The Senate Finance Committee held a hearing titled “Hacking America’s Health Care: Assessing the Change Healthcare Cyber Attack and What’s Next” on May 1, 2024, with Witty testifying for UnitedHealth Group. His prepared testimony addressed the attack, the decision to isolate affected systems, contact with the FBI and the company’s stated commitment to strengthen its defenses.
