Do not stop a blood-pressure medicine on your own. FDA recalls for nitrosamine contamination involve specific lots of some valsartan, losartan and irbesartan products—not every bottle of those drugs. A separate October 31, 2025 report described voluntary recalls involving more than 580,000 bottles of prazosin, but the affected prazosin lots and current status must be confirmed in FDA records. Read the exact label on your bottle and have a pharmacist or prescriber check it.
What the FDA recalls actually cover
The cancer concern involves nitrosamine impurities, including NDMA and NDEA. FDA describes these chemicals as probable human carcinogens. They can enter a medicine through the chemistry used to manufacture its active pharmaceutical ingredient or through reuse of solvents.
FDA’s established recall activity covers selected lots of the angiotensin II receptor blockers (ARBs) valsartan, losartan and irbesartan. A recall is tied to an identified manufacturer or labeler, lot number and expiration window. It does not automatically apply to every product containing the same active ingredient.

Recall records can be ongoing or terminated, so the status needs to be checked when you are reviewing your medicine.

Why a contamination warning mentions cancer
Long-term exposure to nitrosamines above FDA’s acceptable intake limits may increase cancer risk. FDA also says that daily exposure at or below those limits for 70 years is not expected to increase cancer risk.

That figure is a worst-case estimate for the highest daily valsartan dose and a defined four-year exposure. It is not a prediction that an individual patient will develop cancer, and it does not mean every valsartan, losartan or irbesartan product contains NDMA.
How to check whether your bottle is recalled
- Read the physical label. Record the exact active ingredient, strength, manufacturer or labeler, lot number and expiration date. On a blister pack, inspect the foil and outer carton; on a bottle, look for the lot and expiration fields printed near the label or cap.
- Check the current FDA recall listing. Compare every recorded detail, not just the medicine’s name. A matching active ingredient without a matching lot and manufacturer is not enough to establish that your bottle is recalled.
- Ask a pharmacist to confirm the match. Give the pharmacist the bottle or a clear photograph of all sides. The pharmacist can check whether the record is current, ongoing or terminated and identify an unaffected lot when one is available.
- Contact the prescriber if a replacement is needed. The prescriber can decide whether to use an unaffected lot or select another medicine based on your indication and medical history.
What to do if your lot is recalled
FDA’s patient instruction is direct: “Patients taking prescription medications with potential nitrosamine impurities should not stop taking their medications.” Stopping an ARB or another prescription blood-pressure medicine without a replacement plan can leave the underlying condition untreated.

- Call the dispensing pharmacy or prescriber as soon as you confirm a lot match.
- Continue following the prescribed plan until a pharmacist or prescriber tells you how to transition or supplies a replacement.
- Obtain the unaffected lot or clinician-selected alternative before disposing of the recalled supply, unless a pharmacist gives different handling instructions.
- Return unused recalled medicine to the pharmacist after you have received the replacement. Do not pass it to another person.
Stay on an unaffected lot or switch medicines?
There is no single substitute that is appropriate for everyone. FDA says pharmacists and prescribers should make the replacement decision. They should weigh the following factors together:

| Impurity and measured level | If FDA has published a specific impurity and level, that information helps the clinician assess the recall notice rather than treating every product as equivalent. |
| Reason for treatment | The choice may differ when the medicine is being used for hypertension, heart failure or another indication. |
| Clinician-selected substitute | Another product must fit your dose, kidney function, other medicines and patient-specific contraindications. |
| Continuity and shortages | A replacement that is difficult to obtain can interrupt treatment; the pharmacy can identify available unaffected stock. |
What is known—and not yet established—about the prazosin report
The October 31, 2025 Associated Press report is evidence of a reported voluntary recall, not a substitute for matching your bottle to an FDA enforcement record. It identified Teva Pharmaceuticals USA and Amerisource Health Services, more than 580,000 bottles and possible nitrosamine contamination, and reported a Class II classification.

Because the specific FDA lot numbers, bottle details and current status are not established here, do not assume that every prazosin prescription is affected. Take the package to a pharmacist and ask for the current FDA record before changing therapy.
