Grade 12 Work Immersion Journal: What to Write for Days 1–10

A practical Day 1–10 structure for recording Grade 12 work immersion tasks, learnings, issues, resolutions, evidence, and reflections.

Your Grade 12 work immersion journal should be a dated, first-person record of the work you actually performed, what you learned, what you produced, any difficulty you encountered, and how you responded. Write one entry for each immersion day, then add the weekly or end-of-period reflection required by your school. Keep the journal with your attendance records, authorized evidence, evaluations, and other work-immersion portfolio documents.

What DepEd expects in a work immersion journal

Current Department of Education guidance lists a “Daily Journal / Learning Log” as a written output during immersion. The K to 12 Work Immersion Curriculum Guide says the learner “records the daily tasks performed” and identifies a portfolio component called a “weekly diary (narrative/ account of learnings and achievements, issues faced and corresponding resolutions).”

These records support assessment evidence such as attendance and punctuality, verified accomplishments, supervisor evaluation, and application of workplace skills. Your school’s current form and partner agreement control if they differ from an older template.

Use this structure for every day

  1. Day and date: Label the entry Day 1 through Day 10 and write the actual date. Add time in and time out if required.
  2. Workplace or department: Identify the assigned unit without naming confidential clients, patients, students, account holders, or internal projects.
  3. Tasks performed: Describe the sequence of duties with concrete verbs such as assisted, encoded, sorted, inspected, prepared, observed, tested, or documented.
  4. Skills, tools, or procedures learned: Connect the activity to your strand or specialization. Mention a tool or procedure only when disclosure is permitted.
  5. Output or accomplishment: State what you completed, submitted, organized, checked, or demonstrated.
  6. Issue or difficulty: Record a genuine obstacle, uncertainty, error, delay, or safety concern without assigning blame.
  7. Response or resolution: Explain the instruction, correction, collaboration, escalation, or process change that addressed it.
  8. Reflection: State what the experience taught you and what you will apply next.
  9. Supervisor check: Leave room for a signature, initials, or comment when the school or partner requires one.

Day 1 to Day 10 writing prompts

Use the prompts below to decide what to emphasize. They are prompts, not events to copy: replace them with what actually happened at your placement.

Day Focus for the entry Evidence to name
Day 1 Orientation, workplace rules, role, safety, confidentiality, and your first assigned task. Orientation notes, completed first task, or supervisor instruction.
Day 2 Repeating a basic process accurately and recording feedback or corrections. Completed batch, form, checklist, or corrected work.
Day 3 Explaining one concept from school that appeared in workplace practice. The task where you applied the concept and the result.
Day 4 Communication, teamwork, or customer/service interaction while protecting personal identifiers. Your communication role, handoff, message, or documented procedure.
Day 5 First-week synthesis: strongest learning, recurring difficulty, and adjustment for the next week. A short weekly reflection and any week-one accomplishment list.
Day 6 A new task or increased responsibility and the guidance you received. New duty, instructions, and the work you completed with them.
Day 7 Evidence of improvement in speed, accuracy, organization, or technical skill. Specific correction avoided, process improved, or standard met.
Day 8 A problem-solving moment and the safe escalation path you used. Problem, person or procedure consulted, action taken, and outcome.
Day 9 A work habit or professional value that became clearer through practice. Behavior showing reliability, initiative, respect, precision, or safety.
Day 10 Accomplishments, remaining questions, skills to improve, and connection to a postsecondary or career goal. Final output, self-assessment, and questions for your supervisor or teacher.

How to turn notes into a strong daily narrative

Start with verifiable action

Write “I sorted incoming documents by category and entered the approved fields in the department log” rather than “I helped in the office.” Name the order of work and the result, but do not invent quantities or performance scores.

Link practice to learning

Explain the connection between a classroom idea and the task: for example, how a bookkeeping, information-technology, hospitality, health, or technical-vocational procedure guided your work. Describe what you personally understood or applied.

Show a problem and a safe response

A useful entry can say that you were unsure about a form, stopped before submitting it, asked the assigned staff member, corrected the field, and checked the revised version. Do not conceal mistakes, disclose restricted information, or portray an unsafe workaround as initiative.

End with a next action

Make the reflection practical: identify the habit, procedure, or question you will carry into the next shift. “I will verify the reference number before saving the record” is more useful than “I learned a lot.”

Weekly diary and final reflection

If your school requires a weekly diary, combine the daily entries into a short account of learnings and achievements, issues faced, and corresponding resolutions. A five-day synthesis should identify one major learning, one recurring difficulty, the adjustment you made, and the goal for the following week. A final reflection can add how the placement influenced your preferred course, occupation, or further training.

Assemble the journal in your portfolio

  • Daily journal or learning-log pages, in date order.
  • Attendance or Daily Time Record materials showing presence and punctuality.
  • Authorized photographs or captions, with no confidential information.
  • Sample written outputs, only when the workplace and school permit them.
  • Supervisor evaluations, signatures, comments, or verified accomplishment records.
  • Weekly reflections and the final reflection or self-assessment.
  • Other documents listed on your school’s current portfolio checklist.

Keep originals secure and submit copies or digital scans in the format your teacher specifies. A photograph of a workplace document is not automatically permitted evidence; ask before capturing or sharing it.

Choosing a notebook or template

Option Advantages Checks before using it
School-issued template Usually includes required fields, reflection space, and supervisor-check areas. Confirm whether every page must be signed, stamped, or submitted in a particular format.
Blank notebook Portable and flexible for detailed notes, sketches, or daily observations. Ensure it does not omit required fields, privacy controls, signatures, or portfolio page requirements.
Purpose-built work immersion journal May provide structured prompts, notes, and goal-setting sections. Verify that its layout matches your school’s form and leaves enough space for actual entries.

A commercially marketed “work immersion journal notebook” can be a convenience, but no notebook automatically satisfies a school’s printing, signature, or portfolio rules. Use the required school form when one is issued.

Hours, phases, and which rule applies

Older DepEd curriculum materials commonly describe Work Immersion as an 80-hour semester subject. DepEd’s 2025 pilot guidance instead describes a 10-hour pre-immersion phase, a 400- or 620-hour immersion phase for that pilot, and a 10-hour post-immersion phase. Those figures are not a universal replacement for every school arrangement. Follow the current written plan from your school and partner institution; Department Order No. 30, s. 2017 remains the baseline official guideline page for implementation.

Privacy and accuracy checklist

  • Use first person and the actual date of each entry.
  • Record what you did, not what you think an ideal intern would do.
  • Use initials, role labels, or general descriptions when names or records are confidential.
  • Do not copy client, patient, student, payroll, account, login, or proprietary data into the journal.
  • Separate observation from completion: write “observed” when you watched a task and “completed” only when you performed it.
  • Ask your supervisor how to document restricted tasks or attach evidence.
  • Review spelling, dates, time entries, and signatures before submission.

Does every Grade 12 student use the same number of immersion hours?

No. Older curriculum materials cite 80 hours, while the 2025 pilot specifies different phase and hour arrangements. Your school and partner institution’s current written plan controls.

Can I use a blank notebook instead of the school form?

Only if your school permits it and the notebook still provides every required field, reflection space, privacy protection, and signature or supervisor-check area.
