How to Compare COBRA With Marketplace Health Insurance After Losing a Job

By TheFinanceBase Team 11 min read

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare the actual COBRA offer with Marketplace plans available where you live, using the premium after any financial assistance, deductible and other out-of-pocket exposure, provider and hospital networks, prescription coverage, and the confirmed start date. COBRA usually preserves the employer plan for a limited period but shifts nearly all of its cost to you; a Marketplace plan may cost less when you qualify for savings, but its doctors, hospitals, drug coverage, and timing can differ.

Start by collecting your coverage-ending notice and COBRA election materials, then check Marketplace eligibility promptly. Do not cancel COBRA until you know when replacement coverage begins and whether ending it would create a Special Enrollment Period.

What you are actually choosing

After job-based insurance ends, the two principal routes identified by HealthCare.gov are continuing the employer plan through COBRA, if the offer and qualifying event make you eligible, or enrolling in a Marketplace plan during a Special Enrollment Period. Neither route is automatically the better deal. The right choice depends on your household, location, expected medical use, doctors, prescriptions, and the exact dates and prices in your notices.

COBRA continuation

COBRA generally keeps you in the same employer-sponsored plan, with its existing benefit design and provider network, for a limited period. Following job loss, HealthCare.gov describes the period as usually 18 months, but your plan administrator must confirm the actual duration and any conditions. You normally pay the full premium, including the portion your former employer previously paid, plus a small administrative fee. The amount on your election notice—not a former payroll deduction—is the number to compare.

Marketplace coverage

A Marketplace plan is a new individual or family policy. Its premium and cost sharing depend on the plans offered in your area and on the financial help for which your household qualifies. The application also checks for premium tax credits, additional savings, and possible Medicaid or CHIP eligibility. Because the plan is different, its network, formulary, deductibles, copayments, and out-of-pocket limit may not match the employer plan.

Protect the enrollment window first

Losing job-based coverage triggers a Marketplace Special Enrollment Period. HealthCare.gov says to apply within 60 days after the coverage loss. The general guidance is that Marketplace coverage can begin on the first day of the month after job-based coverage ends, but the date shown in your eligibility notice and final plan selection controls. Employment termination, the last day of active coverage, and the replacement policy’s effective date are not necessarily the same day.

  1. Record the coverage-ending date. Check the employer or insurer notice for the final day claims will be covered, not merely your last day at work.
  2. Open the Marketplace application promptly. Use the Marketplace for your state and complete the eligibility questions before assuming a plan will start immediately.
  3. Save the eligibility result. It will show the enrollment deadline, any requested proof, financial-help determination, and available effective dates.
  4. Submit documents quickly. Some applicants must document the qualifying event. Follow the notice’s instructions and keep submission confirmations.
  5. Verify the selected plan’s start date and first premium due date. Coverage is not active simply because an application was started; complete plan selection and any required payment.

Rules, plan availability, and subsidy eligibility can change. The official guidance linked above was accessed October 13, 2026; use the current notice and live Marketplace results for your case.

Get the real COBRA price and end date

Ask the employer or plan administrator for the COBRA election notice and request a complete written quote. A useful quote identifies the monthly premium, the administrative charge, the people covered, the first covered day, payment deadlines, and the date continuation ends. If the notice lists several coverage tiers, obtain the amount for the exact tier you need after job loss.

  • Premium: Use the full COBRA amount, including the former employer’s contribution, rather than your old employee payroll share.
  • Administrative fee: Add the fee stated by the plan. The official guidance describes it as small but does not establish one universal amount.
  • Duration: Job-loss continuation is usually 18 months, but the administrator’s notice governs your offer.
  • Existing spending: Ask how the plan treats deductible and out-of-pocket amounts already accumulated during the current plan year. Your plan documents, not a general rule, determine the answer.
  • Plan changes: Confirm whether the employer plan will change networks, formularies, deductibles, or premiums during the continuation period.

Do not infer the price from a former pay stub or from what a coworker pays. Employer contributions and coverage tiers differ, and the administrator’s quote is the evidence for your household.

Apply for Marketplace savings using the correct income

The Marketplace application evaluates your expected income for the entire calendar year. Include wages and other income received before the job ended, not just what you expect to earn after unemployment. The application uses that estimate to assess premium tax credits and other savings and to screen for Medicaid or CHIP.

Why the full-year estimate matters

A job ending late in the year can leave substantial prior wages even if your current monthly income is zero. Conversely, a new job, severance, unemployment benefits, or other income can change the estimate after you apply. Enter the information requested by the application and update it if circumstances change. Do not assume that no current paycheck automatically means zero annual income.

Separate household decisions

Family members do not have to use the same coverage path. One person may continue COBRA while another qualifies for a Marketplace plan or a public program. Compare eligibility and cost for each person, and make sure the application accurately identifies who needs coverage and whose income is counted.

Marketplace results show the premium after any estimated assistance. Use that net premium in the comparison, but treat the estimate as conditional on the information supplied and the final eligibility determination.

Compare total financial exposure, not just premiums

A low monthly premium can be expensive when care is frequent, while a higher premium can buy lower cost sharing or preserve a valuable network. Put the figures from your COBRA notice and the specific Marketplace plan documents side by side.

Item COBRA Marketplace How to verify
Monthly premium Full amount plus the stated administrative fee; exact figure not stated in general guidance. Plan premium after any eligible tax credit or other savings; exact figure varies by location and household. COBRA election notice and current Marketplace plan listing.
Deductible Use the continuing employer plan’s current individual and family deductibles. Use the selected plan’s individual and family deductibles. Summary of Benefits and Coverage or plan details.
Copayments and coinsurance Check primary care, specialist, urgent care, emergency, hospital, and therapy costs. Check the same services, including whether charges differ in and out of network. Plan documents and insurer member materials.
Out-of-pocket limit Record the plan-year limit and what expenses count toward it. Record the Marketplace plan’s limit and covered-service rules. Plan documents; do not substitute a premium for this limit.
Prescription cost Check each medication’s tier, deductible treatment, quantity limits, and pharmacy requirements. Check the new formulary and the pharmacies available in network. Current formularies and a pharmacist or insurer confirmation.
Doctors and facilities Confirm current physicians, specialists, hospitals, and preferred facilities remain available. Search the exact plan network and confirm with each provider. Insurer directory plus direct provider confirmation.
Effective date Confirm the first day of continuation in the election materials. Confirm the date displayed after eligibility and plan selection. Written notices; never assume the dates align.
End date and later options Use the administrator’s stated termination date and rules. Review the plan term and future enrollment opportunities. COBRA notice and Marketplace notices.

Estimate expected-year cost

For a useful comparison, calculate each option’s annual premiums, then model a low-use year and the care you realistically expect. Add the deductible, visit charges, coinsurance, and medication costs you could incur, while keeping the out-of-pocket limit visible as a worst-case covered-care ceiling. This is a planning exercise, not a promise of what you will spend; claims, exclusions, and network status control the final bill.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the same assumptions for both options. If you expect surgery, pregnancy, regular specialist care, expensive prescriptions, or recurring therapy, verify those services before relying on a premium-only comparison.

Check doctors, hospitals, and prescriptions before enrolling

Continuity is COBRA’s practical advantage when the employer plan already works for your care. Confirm that your physicians and facilities are still participating rather than assuming the network never changes. An employer can change plans, and providers can leave a network during a continuation period.

For a Marketplace plan, search the exact plan name and network—not just the insurer’s general directory. Call the physician’s office and the hospital billing or contracting department to confirm participation for that specific plan year. Direct confirmation matters because directories can lag behind contract changes.

Build a medication check

  1. List every prescription, strength, dose, and monthly quantity.
  2. Check the COBRA plan’s current formulary, tier, deductible rules, prior-authorization requirements, and preferred pharmacies.
  3. Check the Marketplace plan’s formulary and pharmacy network for the same details.
  4. Ask the prescriber whether an equivalent covered medicine is medically acceptable if a drug is excluded or placed on a high tier.
  5. Obtain any transition or prior-authorization instructions before the first refill under the new policy.

Do not compare a drug’s list price or a generic office copay in isolation. The deductible, specialty-drug rules, quantity limits, and pharmacy network can change what you pay.

Prevent a coverage gap when dates do not match

Coverage generally does not start on the day you submit a Marketplace application. HealthCare.gov’s general guidance points to the first day of the month after job-based coverage ends, subject to the eligibility notice and plan selection. COBRA may be available for the period between those dates, but confirm the continuation start and payment deadline.

  • Keep the written final day of employer coverage.
  • Keep the written effective date for COBRA, if elected.
  • Keep the Marketplace eligibility notice and selected-plan confirmation.
  • Pay the first premium by the required deadline and retain the receipt.
  • Ask the insurer how claims submitted during a transition will be handled if enrollment is pending.

If you have a scheduled procedure, infusion, delivery, or medication refill near the transition, call the plan before the appointment. A provider may need the new member ID or authorization even when the effective date is retroactive under the plan’s rules.

Can you switch from COBRA to the Marketplace?

HealthCare.gov explains that losing COBRA because it expires or is no longer available can create a Marketplace Special Enrollment Period; its current guidance describes a 60-day enrollment window. A voluntary decision to end COBRA early, or simply stopping premium payments, generally does not create a Special Enrollment Period outside Open Enrollment.

That distinction makes the COBRA end date a planning deadline. If you want a Marketplace plan later, check whether your proposed switch is tied to COBRA expiration, another qualifying event, or Open Enrollment. Do not cancel first and hope a Marketplace window will open.

What if Medicaid or CHIP may cover you?

If the Marketplace application indicates possible Medicaid or CHIP eligibility, HealthCare.gov advises waiting for a final eligibility decision before ending COBRA. Keep COBRA active while the determination is pending unless the plan administrator and the public program give you clear, written instructions that prevent a gap.

Choose a path using your priorities

COBRA may fit when continuity is worth the premium

  • Your doctors, hospital, and medications are difficult to replace.
  • You are in active treatment and changing networks could delay care.
  • The employer plan has already met much of its deductible or out-of-pocket limit.
  • You need coverage only until a known later event and the quoted premium is manageable.
  • The Marketplace alternatives available to your household have materially different benefits or networks.

These are decision factors, not guarantees that COBRA will cost less. Verify the plan-year rules and the actual quote.

Marketplace coverage may fit when savings or flexibility dominate

  • The net premium after eligible assistance is substantially below the COBRA quote.
  • Your doctors and facilities participate in a suitable local plan.
  • The plan’s deductible, drug coverage, and out-of-pocket limit fit your expected use.
  • You need coverage beyond the COBRA continuation period.
  • Household members qualify for different Marketplace, Medicaid, or CHIP options.

Do not select a plan solely because its displayed premium is lowest. A narrow network, high drug deductible, or unfavorable hospital coverage can outweigh monthly savings.

A worksheet you can complete before deciding

  1. Coverage dates: Write the last covered day under the employer plan, the COBRA start and end dates, and each Marketplace plan’s confirmed effective date.
  2. COBRA price: Record the full monthly premium, administrative fee, coverage tier, payment deadline, and continuation duration from the notice.
  3. Marketplace price: Record the full premium, estimated tax credit or other savings, resulting net premium, and any first-payment requirement.
  4. Cost sharing: Record individual and family deductibles, office and hospital charges, coinsurance, prescription tiers, and out-of-pocket limits.
  5. Care map: List every essential doctor, hospital, facility, prescription, and pharmacy, then mark whether each is confirmed in network or covered.
  6. Risk scenarios: Calculate annual premiums for each option and separately consider routine care, a high-use year, and the plan’s covered out-of-pocket ceiling.
  7. Fallback plan: Note the COBRA termination date, Open Enrollment timing, and what event would create a future Special Enrollment Period.
  8. Evidence file: Save notices, screenshots of plan details, provider confirmations, application submissions, eligibility decisions, and payment receipts.

Common mistakes and how to fix them

Using the old payroll deduction as the COBRA price

Fix: Request the complete COBRA quote. The former employer’s contribution normally no longer reduces your bill.

Waiting until COBRA ends to investigate Marketplace coverage

Fix: Review Marketplace options during the initial job-loss enrollment window and calendar the COBRA end date. Expiration can open another window, but you still need time to apply and provide documents.

Assuming a Marketplace plan starts on the last day of work

Fix: Use the effective date in the eligibility notice and plan confirmation. Arrange interim coverage if the dates do not touch.

Ignoring full-year income

Fix: Include income earned before the job ended and other information requested for the calendar-year estimate. Update the application when circumstances change.

Trusting an insurer directory without confirmation

Fix: Verify the exact plan with each provider and facility, and check the current formulary for every medication.

Forgetting family members may have different eligibility

Fix: Compare each person’s available plans and public-program eligibility instead of forcing the entire household into one option.
