In the head-to-head SURMOUNT-5 trial, adults without diabetes lost an average of 20.2% of body weight with tirzepatide (Zepbound) and 13.7% with semaglutide (Wegovy) after 72 weeks. The randomized, open-label phase 3b study used maximum tolerated weekly doses and was funded by Eli Lilly. Those are group averages, not a guarantee for any individual.

There is no reliable like-for-like price winner in the available evidence. Your cost depends on insurance benefits, deductible status, dose, formulation, pharmacy and eligibility for any savings program. A current comparison requires checking the same dose and formulation on the same date. This guide explains what is established about effectiveness, safety, labeled uses and the practical steps for estimating your own out-of-pocket cost.

What are Zepbound and Wegovy?

Zepbound and Wegovy are prescription medicines used alongside reduced-calorie eating and increased physical activity. They are not the same molecule.

Medication Active ingredient and drug class Forms described in current labeling Key labeled uses described in the available labels
Zepbound Tirzepatide; GIP and GLP-1 receptor agonist Injection Chronic weight management in adults with obesity, or overweight with at least one weight-related comorbidity; treatment of moderate-to-severe obstructive sleep apnea in adults with obesity
Wegovy Semaglutide; GLP-1 receptor agonist Injection and tablet formulations are described in the FDA label surfaced for this comparison Weight-management indications; the injection also has an indication to reduce major adverse cardiovascular events in adults with established cardiovascular disease and obesity or overweight, plus an accelerated-approval indication for noncirrhotic MASH with moderate-to-advanced fibrosis. The tablet has labeled cardiovascular-risk-reduction and weight-management uses.

Formulations and indications can change as the FDA updates prescribing information. Confirm the current full prescribing information for the exact product and form being considered. A tablet and an injection are not automatically interchangeable, even when they contain the same active ingredient.

Why the distinction matters

Zepbound’s obstructive-sleep-apnea indication is specific to adults with obesity. Wegovy’s label includes cardiovascular indications that depend on established cardiovascular disease and, for the liver indication, the criteria in the accelerated approval. These distinctions affect whether a prescription is medically appropriate and whether an insurer may cover it.

Is Zepbound better than Wegovy for weight loss?

In the strongest direct comparison available here, tirzepatide produced greater average weight loss than semaglutide at 72 weeks. The result applies to the study population and conditions, not to every patient.

What SURMOUNT-5 tested

  • Randomized, open-label, phase 3b design.
  • Duration: 72 weeks.
  • Participants: adults with obesity, or overweight with at least one weight-related comorbidity.
  • People with type 2 diabetes were excluded.
  • Tirzepatide was used at a maximum tolerated weekly dose of 10 mg or 15 mg.
  • Semaglutide was used at a maximum tolerated weekly dose of 1.7 mg or 2.4 mg.

The published 2025 report stated that tirzepatide was superior for weight reduction and waist circumference in this study population. Because the trial was open label, participants and investigators knew which treatment was being given. The study was funded by Eli Lilly, the manufacturer of tirzepatide.

The 72-week result

Outcome Tirzepatide (Zepbound) Semaglutide (Wegovy)
Mean change in body weight at week 72 −20.2% −13.7%
Difference in mean percentage-point change 6.5 percentage points greater reduction with tirzepatide in this trial

For example, a percentage change is calculated from each participant’s starting weight; it is not a fixed number of pounds. A person who starts at a different weight, uses a different tolerated dose, stops early or responds differently can have a result far from the group mean.

How to use the result responsibly

The trial answers a narrow question: how two maximum tolerated weekly regimens performed over 72 weeks in adults without diabetes who met the study’s weight criteria. It does not establish that tirzepatide will work better for every person, that the difference persists indefinitely, or that either medicine is appropriate for someone outside those criteria. It also does not compare the total financial burden or access rules faced by a particular insurance plan.

Which has more side effects, Zepbound or Wegovy?

Both products commonly cause gastrointestinal symptoms. The available labels do not provide one single, directly comparable safety table that can be used to rank them across all doses and populations. Incidence changes with dose, formulation, trial design and the people enrolled.

Common reactions listed for both

  • Nausea
  • Diarrhea
  • Vomiting
  • Constipation
  • Abdominal pain and related gastrointestinal discomfort

Those symptoms can affect eating, hydration and the ability to continue treatment. Report persistent or severe symptoms to the prescribing clinician rather than changing the dose independently.

Additional reactions listed for Zepbound

Zepbound’s label also lists injection-site reactions, fatigue, hypersensitivity reactions, eructation (burping), hair loss and gastroesophageal reflux disease among common reactions.

Additional reactions listed for Wegovy

Wegovy’s label lists dyspepsia, headache, fatigue, dizziness, abdominal distension, eructation and hair loss among reactions in addition to the gastrointestinal effects shared above.

Serious warnings and precautions shared by both

Both labels carry a boxed warning about thyroid C-cell tumors observed in rodents. The relevance of those findings to humans is unknown. Both medicines are contraindicated for people with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2.

Both labels also warn about serious gastrointestinal adverse reactions, pancreatitis, gallbladder disease, hypersensitivity reactions, hypoglycemia in certain situations, delayed gastric emptying and pulmonary aspiration during anesthesia or deep sedation. Tell every treating professional that you use one of these medicines, particularly before a procedure requiring anesthesia or deep sedation.

Warnings that differ by label

Wegovy’s label additionally includes warnings about increased heart rate and diabetic retinopathy complications. Zepbound’s label includes a warning not to share its KwikPen between patients. Pregnancy warnings and counseling instructions are not identical, so the exact product label and clinician instructions control.

Stopping treatment because of adverse events

Eli Lilly reported that 6.1% of participants receiving tirzepatide and 8.0% receiving semaglutide discontinued because of adverse events in its detailed SURMOUNT-5 results announcement. This is the manufacturer’s report from that study; it should not be treated as proof that one medicine is universally better tolerated. The trial’s open-label design, dose escalation and participant characteristics all affect how the figures should be interpreted.

How dosing and administration differ

What the direct trial tells us

SURMOUNT-5 compared weekly injections at the maximum tolerated doses used in the protocol: 10 mg or 15 mg for tirzepatide and 1.7 mg or 2.4 mg for semaglutide. “Maximum tolerated” means participants could remain at the highest dose they could manage under the study protocol; it does not mean every patient should receive that dose.

Following the current prescription

Actual starting doses, escalation intervals, maintenance choices, missed-dose instructions and injection technique must come from the current prescribing information and the prescriber. Do not convert a Zepbound dose to a Wegovy dose by matching the number of milligrams: the molecules and dose scales are different. The Wegovy label surfaced for this comparison describes both injection and tablet formulations, so the administration instructions depend on which formulation is prescribed.

Practical administration checks

  • Verify the exact product name, formulation and strength on the prescription and pharmacy label.
  • Ask when to increase, hold or reduce the dose and what to do after a missed dose.
  • Keep a written list of all medicines, especially drugs that can lower blood glucose or affect digestion.
  • Tell the anesthesia team about treatment before surgery, endoscopy or deep sedation because delayed gastric emptying and aspiration are label warnings.
  • Never share an injection device; Zepbound’s label specifically warns against sharing the KwikPen between patients.

How much do Zepbound and Wegovy cost with insurance?

A current, comparable price cannot be stated from the available evidence. The sources used for this comparison do not establish current list prices, cash prices, typical insured out-of-pocket costs or savings-card eligibility for either medicine. Publishing one figure without a date, dose, formulation and eligibility terms would be misleading.

Why two patients can pay very different amounts

  • Benefit design: Some plans cover anti-obesity medicines, while others exclude them or require a separate employer benefit.
  • Utilization rules: Prior authorization, step therapy, quantity limits and documentation of a qualifying diagnosis can affect approval.
  • Deductible status: A deductible can make the first fills cost more than later fills in the same year.
  • Dose and formulation: Different strengths, injection versus tablet and package sizes may have different prices or coverage tiers.
  • Pharmacy network: The negotiated amount can vary by in-network pharmacy, mail order and specialty-pharmacy rules.
  • Eligibility for savings: Manufacturer programs, when available, have their own eligibility, insurance and renewal conditions.

How to calculate a personal annual estimate

  1. Read the plan’s formulary for the exact product and formulation, not merely the drug class.
  2. Call the insurer and ask whether the medicine is covered for the diagnosis being treated and whether prior authorization or step therapy applies.
  3. Ask for the tier, deductible treatment, copay or coinsurance method, quantity limit and preferred pharmacies.
  4. Request the insurer’s estimated member cost for the prescribed strength after applying your current deductible status.
  5. Ask the pharmacy for its cash price for the identical strength and quantity as a fallback comparison.
  6. Check any official savings terms directly and confirm that your insurance type and diagnosis qualify before relying on a quoted amount.
  7. Add related costs such as clinician visits, required monitoring and travel to the pharmacy when comparing total household spending.

A simple annual worksheet is: monthly medicine payments multiplied by the number of fills, plus any remaining deductible or coinsurance, plus visit and monitoring costs. Record the date, strength, formulation and pharmacy for every quote because prices and coverage can change.

Questions to ask before filling the prescription

  • “Is this exact product and strength covered for my diagnosis?”
  • “What documentation is required for prior authorization, and how long does approval last?”
  • “Does the plan require me to try another treatment first?”
  • “What will I pay before and after I meet my deductible?”
  • “Which pharmacies are in network, and are refills restricted to a specialty or mail-order pharmacy?”
  • “If coverage is denied, what is the appeal process and what records does my clinician need?”

Which medication may make sense for a particular clinical situation?

This is a comparison framework, not a personal treatment recommendation. A clinician has to match the indication, contraindications, other medicines, medical history and ability to obtain the product.

When the labeled indication is the deciding factor

For an adult with obesity and moderate-to-severe obstructive sleep apnea, Zepbound has a specific labeled OSA indication. For an adult with established cardiovascular disease and obesity or overweight, Wegovy injection has a labeled indication to reduce major adverse cardiovascular events. The Wegovy label also describes an accelerated-approval use for noncirrhotic MASH with moderate-to-advanced fibrosis. These indications do not mean one product is generally superior; they identify situations in which the label addresses a particular condition.

When the evidence question is average weight loss

The SURMOUNT-5 average favors tirzepatide at 72 weeks, but the study excluded people with type 2 diabetes and used specific maximum tolerated doses. Someone outside that population, unable to tolerate escalation or facing different coverage may have a different practical outcome.

When tolerability and safety dominate

Both medicines share important gastrointestinal and serious safety warnings. A history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 is a contraindication for both. Other issues, such as retinopathy risk, heart-rate concerns, gallbladder disease, pancreatitis history, pregnancy and planned anesthesia, require product-specific counseling.

When household budget dominates

Do not assume the medicine with the larger average weight reduction is cheaper. The available sources do not establish a current price comparison. Coverage approval, deductible timing, refill rules and savings eligibility can make the lower-cost option different for each household.

A concise decision checklist

  1. Identify the treatment goal and confirm that the diagnosis fits the selected product’s current FDA indication.
  2. Review contraindications, pregnancy considerations, gastrointestinal history, gallbladder or pancreas history, eye complications and upcoming procedures with the prescriber.
  3. Compare the direct evidence using the correct population and dose limits rather than treating the 72-week mean as a promise.
  4. Obtain a written insurance estimate for the exact formulation and strength.
  5. Ask how dose changes, missed doses and adverse reactions will be handled.
  6. Recheck coverage and savings conditions before each major change in prescription or insurance.

What the evidence can—and cannot—answer

The clearest head-to-head finding is the 72-week mean weight change in SURMOUNT-5: −20.2% with tirzepatide versus −13.7% with semaglutide in adults without diabetes. The study’s open-label design and Eli Lilly funding belong alongside that statistic. Both medicines have substantial gastrointestinal effects and serious label warnings, with some warnings unique to each product. Labeled uses also differ, including Zepbound’s obesity-related OSA indication and Wegovy’s cardiovascular and liver-related indications described in its current label.

There is no responsible universal answer to “which one costs less with insurance?” without a dated, same-dose comparison using the reader’s plan. The most useful next step is to obtain insurer and pharmacy quotes for the exact prescription, then weigh that confirmed cost against the indication, safety profile and treatment plan established with a clinician.
