Why 30 Years of Deferred Savings-Bond Interest Can Affect Medicare IRMAA Two Years Later

Cashing long-held Series EE or I savings bonds can put decades of previously deferred interest on a single federal tax return. Medicare does not usually read the bond transaction itself; the Social Security Administration (SSA) later uses income from an IRS return—generally the return from two years before the premium year—to determine whether income-related Medicare adjustments (IRMAA) apply. A large redemption-year return can therefore affect a later Medicare premium year, although the scenario in the headline does not establish that the individual crossed an IRMAA threshold or was billed a higher amount.

The result depends on the bond series, whether interest was reported annually, the amount already included on earlier returns, filing status, tax-exempt interest and the exact premium year. Those details must be checked before treating a Form 1099-INT total or an SSA notice as conclusive.

The two-year chain from a bond redemption to Medicare

The mechanism is a sequence of separate tax and Medicare steps:

  1. Interest accumulates. A taxpayer who chose tax deferral generally does not include Series EE or I bond interest each year.
  2. The bonds are redeemed. For a cash-method individual, previously unreported interest is generally reported in the taxable year of redemption, to the extent it was not reported earlier.
  3. The redemption appears on an income-tax return. The bank or Treasury may issue Form 1099-INT, but the gross amount on that form is not automatically the amount newly taxable if earlier returns already included some interest.
  4. IRS data reaches SSA. SSA obtains the most recent federal tax information the IRS can provide, usually from two years before the Medicare premium year and, when newer information is unavailable, from three years before.
  5. SSA tests IRMAA MAGI. It compares the return’s modified adjusted gross income with the threshold for the beneficiary’s filing status and the applicable premium year.

For example, SSA’s policy example says 2026 premiums are generally based on 2024 tax returns. A redemption reported on a 2024 return could therefore be part of the data used for 2026 IRMAA, even though the bonds were cashed in an earlier calendar year only if that year was 2024.

SSA describes the governing rule in its Program Operations Manual System (POMS): The MAGI used to determine if the income-related monthly adjustment amount (IRMAA) applies is the most recent tax information that IRS is able to provide. The manual is available at SSA HI 01101.010.

Why decades of interest can appear on one return

Deferral is a reporting choice, not a tax exemption

For Series EE and Series I bonds, the difference between what was paid for the bond and its redemption value is generally taxable interest. IRS Publication 17 explains that an owner may elect to report that interest each year instead of waiting. If the owner used the deferral method, the accumulated amount generally becomes reportable when the bond is redeemed. The applicable rule can differ for other savings-bond series and for special situations, so the bond type and the owner’s prior reporting method matter. See IRS Publication 17 (2025).

The IRS savings-bond FAQ states: In general, you must report the interest in income in the taxable year in which you redeemed the bonds to the extent you did not include the interest in income in a prior taxable year. That “to the extent” language is critical. A 30-year holding period does not by itself prove that every dollar shown by a financial institution is newly taxable.

Annual reporting and redemption reporting produce different-looking returns

Reporting approach When interest generally enters income What to verify
Annual reporting As interest is earned each year Prior returns showing the amounts already included
Deferral until redemption Generally in the redemption year, for previously unreported interest Bond purchase records, redemption statement and the total previously reported

A taxpayer can also hold bonds acquired under different rules or report some bonds annually and others at redemption. Do not assume that every bond in a transaction follows one method merely because the bonds were cashed on the same day.

How to determine the amount that belongs on the tax return

Do not copy Form 1099-INT blindly

Form 1099-INT can show a gross amount of savings-bond interest. IRS Publication 17 cautions that the form may report more interest than must be included when some interest was already reported in earlier years. The correct taxable amount is generally the current-year interest that has not previously been included, subject to the rules for the particular bonds and taxpayer.

IRS Topic 403 also says taxable and tax-exempt interest must be reported even when no Form 1099-INT or Form 1099-OID was received. The topic is at IRS Topic 403, Interest received. Missing paperwork does not eliminate the reporting obligation, and receiving a form does not settle how much of the figure is includible.

Build a prior-reporting record

  1. Identify each bond’s series, issue date, purchase cost and redemption date.
  2. Locate prior federal returns and schedules where savings-bond interest may have been reported.
  3. Reconcile the redemption statement and Form 1099-INT with the interest already included in those returns.
  4. Report only the portion that remains includible under the IRS rules for the bonds and reporting method.
  5. Keep the calculation with the return in case the IRS or SSA later asks why the return differs from the form’s gross figure.

If earlier returns cannot be found, a tax professional can help reconstruct the reporting history from bond records and available IRS transcripts. The amount that affects AGI is the amount properly included on the return, not necessarily the largest number printed on an information form.

How that tax return becomes an IRMAA decision

SSA’s MAGI formula

For IRMAA, SSA defines modified adjusted gross income as adjusted gross income from Form 1040 line 11 plus tax-exempt interest from Form 1040 line 2a. A redemption-year interest amount can raise AGI and therefore MAGI, while tax-exempt interest can raise MAGI even though it is not included in AGI.

SSA normally uses the latest IRS return available for the premium year, usually the return filed two years earlier. If that return is unavailable, SSA may use a return three years before the premium year. The premium year, not the redemption date alone, determines which return is examined.

Thresholds are year- and filing-status-specific

SSA’s 2026 POMS table lists these IRMAA MAGI thresholds:

2026 filing status MAGI threshold shown in the 2026 SSA table
Married filing jointly $218,000
All other filing statuses $109,000

These are threshold figures, not premium amounts. They apply to the 2026 table and should not be substituted for another premium year’s limits. Whether a person pays an income-related surcharge depends on total MAGI, filing status and the bracket in force for that year. There is no automatic dollar-for-dollar conversion from a bond’s interest to a Medicare premium.

What the headline scenario does and does not prove

The fact pattern supplies neither the bond records nor the tax and Medicare documents needed for a personal determination. It does not establish:

  • Whether the bonds were Series EE, Series I or another series;
  • Whether the owner elected annual interest reporting or deferred it until redemption;
  • How much interest had already appeared on earlier returns;
  • The redemption year and the premium year;
  • Filing status, other income or tax-exempt interest;
  • The MAGI shown on the return SSA used; or
  • Whether an SSA notice actually imposed a higher IRMAA amount.

Accordingly, the defensible explanation is that deferred interest could have produced a one-year income spike that SSA later used for IRMAA. It is not a finding that the person in the title definitely owed a surcharge.

How to check an SSA notice and the underlying return

When a notice appears after a large bond redemption, work from the documents rather than from the 1099-INT alone.

  1. Read the notice’s tax year and premium year. Confirm which federal return SSA says it used and which Medicare year the decision covers.
  2. Compare SSA’s MAGI with the filed return. Start with Form 1040 line 11 and add line 2a tax-exempt interest, following SSA’s IRMAA definition.
  3. Recheck the bond calculation. Compare the interest included on the return with the gross Form 1099-INT amount and the amounts reported in earlier years.
  4. Check filing status and every other income item. A threshold test uses total MAGI, not bond interest in isolation.
  5. Correct the IRS return if it is wrong. If a return omitted previously reported interest, included too much, or otherwise contains an error, obtain tax advice about an amended return and retain proof of the correction.
  6. Contact SSA with the notice and evidence. SSA’s public instructions explain how to request a new determination when corrected tax information is available and how to seek reconsideration or appeal.

When SSA can lower or revisit IRMAA

Qualifying life-changing events

SSA-44 is used to request a lower IRMAA after certain life-changing events that reduced household income. SSA lists events such as marriage, divorce, death of a spouse, stopping work, reducing work, loss of income-producing property, certain pension losses and an employer settlement. The request requires supporting documentation and an estimate or evidence of the reduced income, as described at SSA’s request-to-lower-IRMAA page.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A one-time savings-bond redemption is not listed as a life-changing event in that guidance. The resulting income may be unusual or nonrecurring, but unusual income alone does not turn the redemption into a qualifying event. The listed event must have occurred and must satisfy SSA’s evidence requirements.

Corrected tax information and appeals

If the IRS return used by SSA was amended or corrected, SSA has procedures for a new determination based on the updated tax data. If the beneficiary believes SSA applied the rules or figures incorrectly, reconsideration and further appeal rights are available. The appeal process is described in SSA HI 01140.001. Follow the deadline and submission instructions on the notice; an appeal is different from a life-changing-event request.

Could the education savings-bond exclusion remove the interest?

Some or all interest from qualifying Series EE or Series I bonds issued after 1989 may be excluded when the owner pays qualified higher-education expenses in the same year. The exclusion is conditional: the bonds, expenses, taxpayer, filing status and income must all satisfy the program rules. Form 8815 is used to calculate the exclusion; see IRS information about Form 8815.

For 2025 returns, IRS Publication 550 states that the exclusion begins to phase out above modified AGI of $149,250 for joint returns and $99,500 for other returns. Those figures are specific to 2025 returns and should not be carried to another tax year without checking that year’s instructions. See IRS Publication 550 (2025).

Paying tuition in a different year, using nonqualified expenses, holding bonds issued before the qualifying date or exceeding the income limits can prevent the exclusion. It should be tested before filing, not assumed after an IRMAA notice arrives.

Practical preparation for a future redemption

  • Keep a bond inventory with issue dates, purchase prices and registration details.
  • Save every return that reported savings-bond interest and note which bonds were covered.
  • Before redeeming a large group, estimate the tax-year income effect and the later Medicare look-back year.
  • Ask a tax professional whether annual reporting, redemption-year reporting or an available education exclusion fits the facts; changing reporting treatment has technical rules.
  • When Medicare coverage is near, retain the final return, Form 1099-INT, calculation of previously reported interest and any amended-return acceptance evidence.

The central distinction is between a legitimate one-year income increase and an incorrect income figure. SSA can use a valid high-income return even when the increase came from a single redemption. If the return or the SSA data is wrong, documentation and the appropriate correction or appeal route—not the redemption’s age—determine what can be changed.

Frequently Asked Questions

Does SSA calculate IRMAA from the Form 1099-INT sent by the bank?

SSA generally uses MAGI from the federal tax return it obtains from the IRS. The 1099-INT is useful for checking the return, but its gross figure is not itself the IRMAA calculation.

Can a one-time bond redemption automatically qualify me for a lower IRMAA?

No. SSA’s listed life-changing events do not include a savings-bond redemption by itself. A request must fit a qualifying event or rely on corrected tax information, and it must include the evidence SSA requires.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What if I reported some bond interest in earlier years?

Only interest not previously included should generally be reported again. Reconcile prior returns with the redemption statement and Form 1099-INT, and obtain tax advice if the records or return need correction.
