Which Documents to Submit When Claiming One Hospital Bill From Two Insurers in India

For a second claim on the same hospitalization, submit the second insurer’s claim form, the itemized hospital bill and receipts, discharge and medical records, both policy details, and the first insurer’s settlement or rejection statement showing what it paid and deducted. If the first insurer is holding the originals, ask it for insurer-attested copies and an original certificate or settlement advice stating the amount paid. Tell the second insurer that the records are certified copies—not originals—and confirm its accepted format and policy deadline in writing.

This process is for coordinating two indemnity health policies in India. It is not permission to recover the same expense twice: the combined payment cannot exceed admissible hospitalization costs actually incurred, and the second insurer applies its own exclusions, limits and copayments.

What a “second claim” actually means

With two indemnity policies, you may choose which policy to use first. The insurer you choose becomes the primary insurer; the other can be approached for eligible expenses left unpaid after that settlement. The balance is assessed under the second policy, not calculated by simply reversing every deduction made by the first.

For example, if a hospital’s admissible bill is ₹2,00,000 and the first insurer pays ₹1,60,000, the second insurer may assess the remaining ₹40,000. It can still reject that amount if its own policy excludes an item, applies a sub-limit, deductible or copayment, or requires a condition that was not met. You cannot obtain more than the admissible expense incurred by submitting the same charge to both insurers.

This article concerns indemnity cover, where reimbursement is tied to actual medical expenses. A benefit-based policy—such as one that pays a defined amount on diagnosis or hospitalization—works differently: its fixed benefit may be payable by each insurer when the policy terms are satisfied.

Documents to send to the second insurer

Prepare one indexed file and a complete digital scan before sending anything. The exact list and submission channel depend on the policy wording and the insurer’s current claims instructions, but the following set addresses the questions a second insurer normally has: what was charged, what treatment occurred, what the first insurer paid, and what remains unpaid.

Document What it establishes How to provide it
Second insurer’s claim form Identifies the hospitalization, policy, claimant and amount being requested. Use the current form and disclose the other policy and the first claim. Sign every declaration and attach the claim reference if one was issued.
First insurer’s final settlement, partial-settlement or rejection letter Separates the original bill from amounts already paid, deductions and disallowed items. Prefer a statement that itemizes the sanctioned amount, deductions and any balance. Include the first claim number and settlement date.
Itemized final hospital bill Shows each room, procedure, medicine, investigation and professional charge instead of only a total. Submit the final bill with all charge breakups. If the original is retained elsewhere, use a certified copy and label it accurately.
Payment receipts Shows that the hospital charges were actually paid or that a documented liability remains. Include receipts, deposit adjustments and any refund note. Keep a copy of each page.
Discharge summary Confirms admission and discharge dates, diagnosis, treatment and outcome. Provide the complete document, including continuation pages and signatures where present.
Investigation reports and prescriptions Supports the medical necessity and timing of tests, medicines and procedures. Attach reports, prescriptions, operative notes and other records relevant to the treatment. The precise medical set varies by claim.
Both policy details Allows coordination of coverage, limits and prior disclosure. Include policy numbers, schedules, endorsements, insurer contact details and the employer or group-policy certificate if applicable.
Relevant correspondence Creates a traceable record of notifications, requests and decisions. Attach emails, letters, pre-authorisation decisions, query replies and acknowledgement numbers for both claims.
Certified copies when originals are with insurer one Gives insurer two reliable records without pretending that photocopies are originals. Request insurer-attested or otherwise certified copies of bills, receipts and medical papers, plus an original certificate or settlement advice stating the amount paid.

Some archived policy wordings hosted by the regulator expressly refer to verified photocopies attested by the other insurer or reimbursement provider and an original certificate of the amount paid. Another wording refers to certified documents and settlement advice. Those examples show why certification is useful; they do not impose one universal format on every current product.

When the first insurer has the originals

Hospitals commonly issue one original bill set, and a reimbursement claim may require the first insurer to retain it. Do not send an uncertified photocopy while describing it as an original. Instead, create a documented chain from the first claim to the second.

  1. Ask for a document-return or certification letter. Request copies of every submitted bill, receipt and medical record that the second insurer may need. Ask for an original certificate or claim settlement advice showing the amount paid, deductions and the status of each major item.
  2. Obtain written confirmation of the acceptable format. Send the second insurer a list of the records held by insurer one and ask whether insurer-attested copies, scanned certified copies or another substitute is acceptable. Keep the reply with your claim file.
  3. Match each copy to the settlement statement. Number the pages and identify which charges were paid, reduced or left unpaid. This prevents the second insurer from treating a paid item as an outstanding expense.
  4. Disclose the arrangement on the claim form. State that originals were submitted to the primary insurer and identify the attached certified copies and settlement advice. Never conceal the existence of the first policy.
  5. Preserve your own evidence. Scan the complete set before submission and retain courier receipts, portal upload confirmations, email delivery records and acknowledgement numbers.

If the first insurer refuses to return or certify records, ask it to state that refusal in writing and forward that response to insurer two. Ask insurer two, also in writing, what alternative evidence it will accept. A verbal assurance is difficult to prove if the claim is later queried.

How the two-policy rules affect payment

Indemnity policies coordinate; they do not create double recovery

Regulatory consumer guidance allows a policyholder with multiple indemnity policies to select the policy used first. It states: “The Insurer of that chosen policy shall be treated as the primary Insurer.” If the primary policy’s available cover is less than the admissible amount, the primary insurer should seek details of the other policy and coordinate settlement of the balance, subject to the policies’ conditions.

Coordination does not turn every hospital charge into a payable charge. Each insurer applies its own definition of hospitalization, exclusions, room-rent limits, disease sub-limits, deductibles, copayments, waiting periods and documentation rules. The second insurer may therefore pay less than the arithmetic difference between the bill and insurer one’s payment.

Benefit-based cover is different

A benefit policy pays a defined sum when its trigger is met, rather than reimbursing each rupee of the hospital bill. If both policies are benefit-based and their terms are satisfied, each defined benefit may be claimed. Read the schedule and wording before applying indemnity coordination rules to a fixed-benefit product.

What “balance” means

For a second indemnity claim, “balance” means an expense that remains unpaid after the first settlement and is admissible under the second policy. It does not automatically include every deduction made by insurer one. A non-payable consumable under policy two, for instance, remains your responsibility even if policy one also deducted it; conversely, an item allowed by policy two may still require proof that it was actually incurred and not already reimbursed.

Choosing which policy to use first

You can generally choose the primary policy, so make that choice before submitting a claim when circumstances permit. Compare the practical effect of each contract rather than choosing solely by insurer name.

Decision factor Questions to answer Why it matters
Available sum insured How much remains after earlier claims, and is restoration or a family floater limit involved? A policy with insufficient remaining cover may leave a larger balance for coordination.
Cashless network Is the hospital in-network, and can pre-authorisation be obtained? Cashless access can reduce the amount you must pay upfront, but it does not waive policy exclusions.
Sub-limits Are there room-rent, procedure, disease or ambulance caps? A lower cap can create deductions that the second insurer may or may not cover.
Copayment and deductible What percentage or fixed amount is your share? These contractual shares reduce the first settlement and may remain payable under policy two.
Exclusions and waiting periods Does either policy exclude the diagnosis, treatment, item or pre-existing condition? The second insurer will apply its own wording; it is not bound by insurer one’s approval.
Notification and claim deadlines When must hospitalization and reimbursement be notified, and through which channel? Missing one policy’s notice requirement can create a separate coverage issue.
Records and coordination Which insurer can provide certified copies and a detailed settlement advice promptly? Good documentation can prevent avoidable queries when originals are held by the primary insurer.

Notify both insurers within their respective policy timelines. Notification to one insurer should not be assumed to satisfy the other policy’s notice condition.

Recommended filing sequence

  1. Review both active wordings and schedules. Check the version in force on the hospitalization and claim dates, remaining sum insured, exclusions, sub-limits, copayments, deductibles and notice instructions.
  2. Notify both insurers and disclose both policies. Record the date, method, acknowledgement number and contact person for each notification.
  3. Select and complete the primary claim. Use cashless or reimbursement procedures as applicable, and answer questions about other insurance accurately.
  4. Request the primary settlement package. Obtain the final settlement or rejection letter, itemized deductions, payment confirmation and certified copies of records retained in original.
  5. Calculate the potential balance. Reconcile the itemized bill against the first payment. Remove amounts already reimbursed and flag items that may fall under policy two’s exclusions or limits.
  6. Confirm insurer two’s document format and deadline. Ask for a written checklist, especially where only certified copies are available.
  7. Submit the second claim. Attach its form, both policy details, the first settlement advice, itemized bills and receipts, medical records, and the certification explaining why originals are unavailable.
  8. Keep proof of delivery. Save portal receipts, courier tracking, emails, uploaded files and every subsequent query or response.

What the second insurer may still question

  • Whether the expense was already paid: the first settlement advice should identify sanctioned amounts and deductions line by line.
  • Whether the treatment is covered: approval by insurer one does not override insurer two’s exclusions, waiting periods or definitions.
  • Whether the amount was actually incurred: a hospital estimate or undifferentiated total may not establish payment; provide final bills and receipts.
  • Whether the records are authentic: identify every photocopy as a copy and obtain attestation or certification from the insurer holding the original.
  • Whether a policy condition was met: late notice, missing pre-authorisation, a room-rent limit or an undisclosed co-insurance term can affect the second assessment.

IRDAI consumer guidance says claims should be assessed against policy terms and that, ordinarily, documents not listed in the policy should not be treated as necessary except in suspected-fraud situations. It also says necessary documents should be called for together rather than piecemeal. Keep that principle in context: the active policy and the insurer’s written claim instructions control your particular file.

Deadlines, assessment and delayed payment

Do not rely on a generic “30-day filing rule.” The applicable notification and submission period varies by policy, product and the event that starts the clock. Read both wordings and obtain written confirmation from each insurer if the original documents are with the other company.

IRDAI’s health-insurance FAQ states that an insurer should settle or reject a claim within thirty days after receiving the last necessary document. The same FAQ describes interest at 2% above the bank rate for a delay beyond that period, measured from receipt of the last necessary document until payment, subject to the regulatory terms and exceptions in force. Verify the current regulatory text and your policy before relying on those figures in a dispute. The thirty-day period is not a universal deadline for filing the second claim; it concerns the insurer’s decision after its required documents are received.

Common mistakes and how to correct them

Calling copies “originals”

Incorrectly labelling a photocopy can trigger an authenticity query. Mark it as an insurer-certified copy, identify where the original is held and attach the certification or settlement advice.

Assuming every first-insurer deduction is payable by insurer two

Prepare an item-by-item reconciliation, then apply policy two’s exclusions, limits and cost-sharing rules. Submit only the unpaid, potentially admissible amount.

Sending originals without a retained set

Scan or photocopy every page before dispatch. Keep the dispatch proof and ask the receiving insurer to acknowledge the page count.

Using an archived wording as a universal rule

Archived examples can illustrate certified-copy handling, but products and versions differ. The schedule and wording active on your hospitalization or claim date take priority.

Failing to disclose the second policy

List both policies on each claim form and in correspondence. Non-disclosure can delay coordination and create a separate policy-compliance problem.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Waiting for insurer one before notifying insurer two

Notify both within their respective timelines, even if the final amount paid by the primary insurer is not yet known. Update the second insurer with the settlement advice later.

Accepting a verbal document instruction

Ask for the checklist and any approval of certified copies by email or letter. Written instructions provide a clear record if the claim is later escalated.

If the second claim is delayed or rejected

  1. Request the decision in writing. Ask for the exact policy clause, itemized deductions and the list of documents still considered necessary.
  2. Compare the decision with the wording. Check the policy schedule, exclusions, sub-limits, copayment, deductible and any condition cited by the insurer.
  3. Supply a focused response. Send the missing certification, settlement reconciliation or medical record with a covering note that maps each document to the query.
  4. Use the insurer’s grievance process. Keep the complaint number, submission date and all replies. If unresolved, proceed to the appropriate regulator or insurance-ombudsman channel then available in India, checking the current contact route before filing.

Maintain one chronological file containing notifications, claim forms, bills, certifications, settlement advice, queries, responses and proof of delivery. It is the clearest way to show that the second request concerns an unpaid balance rather than a second recovery of the same expense.
