Do not stop, ration, or replace the prescription on your own. Call the number on your Medicare plan card to identify exactly what changed, ask whether a temporary transition fill is available, and contact the prescriber the same day. The prescriber can request a coverage determination or formulary exception; if the plan denies it, you can request a redetermination and, when the problem concerns a future plan year, compare other plans during an applicable enrollment period.
Start by finding out what “stopped covering” means
A pharmacy rejection does not always mean the drug has been removed completely. A Part D plan or Medicare Advantage plan with drug coverage may have changed the drug’s formulary status, moved it to a higher cost-sharing tier, added prior authorization, imposed a quantity limit, required step therapy, or excluded it for the condition being treated. Each reason leads to a different remedy.

Call the member-services number on your plan card and ask for the precise coverage reason, the date it takes effect, and the written notice that controls your rights. Ask the representative to confirm:
- Whether the exact medication, strength, dosage form, and quantity remain on the current formulary.
- Whether a higher tier, prior authorization, quantity limit, or step-therapy rule applies.
- Whether the plan considers the drug non-formulary or excluded for your diagnosis.
- Whether the change is effective now or begins with a coming plan year.
- Whether a transition fill, coverage determination, tiering exception, or formulary exception is available.
- How your prescriber should submit supporting information and where to send an appeal if needed.
Formularies are plan-specific and can change under Medicare rules. The plan’s current formulary and your actual notice are more reliable than an old drug list, a pharmacy website, or an informal online post.

Ask the pharmacy about a transition fill
Medicare describes a one-time 30-day transition fill when coverage begins for a drug that the plan does not cover or subjects to prior authorization or step therapy. It is designed to prevent an immediate interruption while the member and prescriber resolve the coverage issue.
This is not a promise of continuing coverage and should not be treated as a universal entitlement for every midyear formulary change. Ask both the plan and the pharmacy whether the transition rule applies to your circumstances, whether the fill is limited to 30 days, and what you must do before that supply runs out.
If the pharmacy cannot process the prescription, ask the pharmacist for the rejection or notice that explains how to contact the plan. Keep the notice; it may identify the applicable rule and provide instructions for requesting a determination.

Work with the prescriber on a coverage determination
You, your prescriber, or an authorized representative may request a Part D coverage determination. The request can address whether the drug should be covered, a formulary or tiering exception, the amount you must pay, a quantity or dose limit, step therapy, or prior authorization.

When a formulary exception is appropriate
A formulary exception asks the plan to cover a non-formulary drug or waive a coverage rule. The prescriber’s supporting statement should explain why the plan’s covered alternatives would not be as effective, would cause adverse effects, or would be inadequate or likely to be less effective or harmful because of a required dose limit or step-therapy alternative. The explanation should relate to your medical circumstances rather than simply state that you prefer the requested drug.
What to give the prescriber
- The plan’s written denial or formulary-change notice.
- The drug name, strength, dosage form, directions, and quantity prescribed.
- The date the current supply will run out and whether the pharmacy has denied a fill.
- Any previous treatments, adverse effects, contraindications, or treatment failures that matter to the exception request.
- The plan’s submission instructions and any case or reference number from your call.
Ask the prescriber whether a covered alternative is clinically appropriate, but do not change medicines, skip doses, split tablets, or ration a supply unless the prescriber gives specific instructions. A plan’s preferred alternative may not be safe or effective for your condition.
Know the decision deadlines
The applicable deadline depends on the type of request and whether waiting could seriously jeopardize your health. Tell the plan if a delay could cause a serious deterioration, loss of function, or other urgent medical risk, and ask the prescriber to support expedited handling.
Recommended Free Tools
| Request | Expedited decision | Standard decision | Important qualification |
|---|---|---|---|
| Initial benefit coverage determination that does not involve an exception | Within 24 hours | Within 72 hours | The clock applies to the plan’s receipt of the request under CMS guidance. |
| Formulary or other exception | Within 24 hours | Within 72 hours | The decision period starts when the plan receives the prescriber’s supporting statement. |
| Part D redetermination (first-level appeal) | No later than 72 hours | No later than 7 calendar days | Use the instructions in the denial notice; expedited treatment requires a health-related reason. |
If the plan says information is missing, ask exactly what is needed and when the decision clock will resume. Record the date and time of every submission and request confirmation that the plan received it.

If the plan denies the request, file a redetermination
A denial of a Part D coverage determination can be appealed first to the plan sponsor through a redetermination. The usual filing period is 65 calendar days from the date on the coverage-determination notice. The notice should state where to send the request, what form or documents to use, and the case-specific deadline. If you did not receive the notice or cannot understand the deadline, call the plan immediately and ask how to preserve your appeal rights.
Build the appeal record
- Read the denial notice and identify the exact rule the plan applied.
- Submit the redetermination using the plan’s stated channel and keep proof of delivery.
- Include the prescription, the denial, the formulary or restriction information, and relevant clinical history.
- Ask the prescriber for a statement addressing why covered alternatives are ineffective, unsafe, or unsuitable for you.
- Request expedited review if the delay could seriously threaten your health, and ask the prescriber to explain that risk.
- Write down the submission date, representative’s name, reference number, and the date by which the plan says it will decide.
The plan’s response should arrive within 72 hours for an expedited redetermination or within 7 calendar days for a standard redetermination. If the plan misses its stated deadline, contact the plan and ask what escalation or next-level appeal instructions apply.
Decide whether this is a temporary problem or a reason to compare plans
A coverage dispute for the current plan year may be resolved through a transition fill, exception, or appeal. If the change begins with a future plan year, comparing plans during a valid Medicare enrollment period may be more practical than repeatedly seeking an exception.

Compare the complete coverage, not just the drug name
| Comparison point | What to verify |
|---|---|
| Formulary status | The exact drug, strength, dosage form, and quantity for the relevant plan year. |
| Coverage rules | Prior authorization, step therapy, quantity limits, tier placement, and exception requirements. |
| Pharmacy access | Whether your pharmacy is in network and whether a preferred pharmacy changes your cost. |
| Total cost | Premium, deductible, monthly cost sharing, and expected annual spending for all your prescriptions. |
| Enrollment timing | Whether you are currently in an enrollment period that permits the change and when new coverage would begin. |
A plan that lists your medication may still impose restrictions or cost more overall. Confirm the details with the plan before enrolling because formularies and utilization rules differ and can change.

Coverage disputes often involve several calls and documents. Keep one folder—paper or electronic—with:
- The original prescription and every revised prescription.
- Formulary pages or screenshots showing the drug’s status and restrictions.
- Pharmacy rejection messages, transition-fill information, and written notices.
- Dates, times, names or identification numbers of representatives, and reference numbers.
- Copies of the prescriber’s supporting statement and confirmation of submission.
- Appeal forms, delivery receipts, and every decision letter.
When you call, summarize the issue in one sentence, ask the representative to repeat the next required step, and write down the deadline before ending the call. If a representative gives information that conflicts with a written notice, ask for clarification from the plan’s coverage-determination or appeals department.
Common mistakes that can jeopardize access
Waiting until the last dose
Exception and appeal requests require documents and prescriber involvement. Start as soon as you receive a change notice or a pharmacy rejection so there is time to request a transition fill and submit supporting information.

Assuming a higher tier means no coverage
A higher tier usually changes your cost-sharing obligation rather than removing the drug. Ask whether a tiering exception is available and what your actual out-of-pocket amount would be.
Assuming a transition fill solves the dispute
The Medicare-described transition fill is one-time and limited to 30 days when the transition rule applies. It does not establish ongoing coverage or eliminate the need for an exception, authorization, or appeal.
Switching drugs without clinical advice
Do not substitute a generic, a different strength, or another medicine solely because it appears cheaper or preferred by the plan. Ask the prescriber to evaluate any alternative.

The standard Part D redetermination filing period is 65 calendar days from the coverage-determination notice. Treat the date on the notice as controlling and seek instructions immediately if the notice was delayed or lost.
A practical decision path
- Pharmacy rejects the prescription: obtain the rejection reason and notice, then call the plan.
- Plan confirms a restriction or non-formulary status: ask about a one-time transition fill and the coverage-determination process.
- Prescriber says the drug remains medically necessary: have the prescriber submit the supporting statement and request expedited review when delay threatens health.
- Plan approves: confirm the authorization period, quantity, pharmacy requirements, and your cost before the next fill.
- Plan denies: file the redetermination described in the notice within 65 calendar days, requesting expedited review when medically justified.
- Change is for a future year: compare complete plan costs and restrictions during an applicable enrollment period, with Medicare or SHIP assistance if needed.
The correct remedy depends on the plan’s rule, the effective date, and your medical circumstances. The safest approach is to keep the prescription clinically supervised while you use the plan’s determination and appeal procedures.
