What to Check About Healthcare Access Before Retiring to a Small Town

Before buying a home in a small town, verify three things for your household: the care you use is available nearby, your Medicare or retiree coverage will pay for it there, and you have a realistic route to emergency and specialty services. A clinic pin on a map is only a lead. Call providers, hospitals, pharmacies and your insurer to confirm new-patient availability, appointment waits, services, network status, drug supply, transportation and backup destinations.

The checks below are U.S.-focused. Provider capacity, road conditions, plan networks, pharmacy inventory, broadband and hospital services can change by address, so make the final decision using the exact home location and current written answers from each organization.

Start with the care your household actually needs

Healthcare access is personal. A town that works for a healthy couple may fail someone who needs weekly physical therapy, dialysis, oncology visits, behavioral-health care or regular imaging. Make a one-page inventory before comparing towns.

  • List every ongoing condition, medication, refill frequency and routine appointment.
  • Note the specialists, laboratory tests, imaging, rehabilitation, home-health or durable medical equipment you use now.
  • Ask current clinicians which services must continue without interruption and which could be provided by telehealth.
  • Identify foreseeable needs over the next five to 10 years, including mobility support, post-surgical rehabilitation or help with daily activities.

Give the list to each candidate town’s providers. Their answers, rather than a general population estimate, determine whether the move is practical.

Map the complete local care network

Primary care and Rural Health Clinics

Find family-practice, internal-medicine and geriatric clinicians, then identify any Rural Health Clinic (RHC). Medicare describes RHCs as outpatient facilities in non-urbanized areas with provider shortages that deliver primary and preventive services. Their presence does not prove that they are accepting patients or can meet your needs; confirm by phone at Medicare’s RHC services page and with the clinic itself.

Record each practice’s new-patient policy, next available appointment, clinician credentials, hours, after-hours advice and referral process. Ask whether visits, blood draws and basic diagnostics occur in the town or require a trip elsewhere.

Hospitals, emergency departments and specialists

Map the hospital, freestanding emergency department (if any), urgent-care sites and the facilities that provide cardiology, surgery, obstetrics, oncology, dialysis, behavioral health and other services on your inventory. A hospital name does not establish that it has an intensive-care unit, 24-hour imaging, a surgeon on call or inpatient beds. Ask the hospital for its current service lines and where it transfers patients when a service is unavailable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Diagnostics, rehabilitation and home-based care

Confirm locations for laboratory work, X-rays, CT or MRI scans, physical and occupational therapy, skilled nursing, hospice and home-health visits. Ask whether referrals from a local clinician are required and how often mobile services visit. A nominally local service that operates one day a week may not meet a frequent-care need.

Pharmacies and medical supplies

List every retail, independent, mail-order and specialty pharmacy serving the address. Ask about hours, delivery to your road, controlled-substance rules, refrigeration, shortage procedures and transfer of existing prescriptions. Check suppliers for oxygen, diabetic equipment, walkers and other recurring items before you move.

Call providers instead of relying on directories

Online maps and insurer directories can be outdated. Use a call log with the date, employee’s name and exact answer. For every priority provider, ask:

  1. Are you accepting new patients with my conditions and age group?
  2. What is the next available routine appointment and the typical wait for follow-up?
  3. Which services are performed at this location, and which require referral or travel?
  4. Which emergency department or specialist receives your referrals?
  5. Do you accept my exact Medicare Advantage plan or Medicare assignment? If you are a pharmacy, are you in my Part D network?
  6. What are your hours, holiday coverage and after-hours instructions?

Request written confirmation of network participation and any referral or prior-authorization rules. Recheck immediately before enrollment or closing because contracts and staffing can change.

Assess hospital capability and a backup destination

Rural hospital capacity is not static. The Health Resources and Services Administration (HRSA) reports that 152 rural hospitals closed between 2010 and 2025, including 52 Critical Access Hospitals; the figure appears on a page last reviewed in September 2026 and describes national history, not a prediction for a particular town. HRSA notes that “Rural hospitals often struggle to maintain healthcare services.” Read the current Rural Hospital Programs information, then verify the candidate hospital directly.

Ask the hospital:

  • Is the emergency department staffed 24/7, and what ambulance service normally brings patients there?
  • Which inpatient beds, operating-room services, imaging modalities and laboratory capabilities are available today?
  • Which conditions trigger transfer, and to which regional facility?
  • How are transfers arranged when weather, staffing or bed shortages affect the preferred destination?

Identify a second destination for services the local hospital cannot provide. Call both facilities about acceptance, referral requirements and driving directions. Ask local emergency-management officials or ambulance providers about dispatch arrangements and whether air transport is available; national figures do not establish a response time for your address.

Measure travel by road, not by the map pin

Calculate routes from the actual house, not the town center. Record normal and winter driving times to primary care, the local emergency department, the backup hospital, the nearest specialist, pharmacy and rehabilitation site. Include mountain passes, ferry schedules, flooding, snow, construction and the availability of a driver. Straight-line mileage is not a substitute for road time.

There is no universal travel-time limit supported by national Medicare or HRSA guidance. Set your own limit based on the seriousness and frequency of care, mobility, climate, vehicle reliability and whether a spouse or friend can drive. A 45-minute trip may be manageable for quarterly dermatology visits but unacceptable for twice-weekly dialysis.

Confirm how Medicare pays at the destination

Medicare Advantage

Medicare Advantage plans must cover services that Original Medicare covers, but routine care is governed by the plan’s network and rules. Ask the plan to verify the exact doctors, hospital, laboratory, imaging center and pharmacy at the candidate address. Out-of-network care may cost more; emergency and urgent care remain available when needed even if the facility is out of network. Details are explained in Medicare’s guide to getting services.

Original Medicare

With Original Medicare, ask each provider whether it accepts Medicare assignment and charges the Medicare-approved amount. Coverage is governed by federal and state law, national coverage decisions and local coverage decisions, so moving does not make an excluded service covered. Medicare lists long-term care, dentures and routine physical exams among examples of services Original Medicare generally does not cover. Review the limits in What Original Medicare covers and check any Medigap policy separately.

Typical cost-sharing questions

Medicare says most preventive services cost nothing when eligibility and provider requirements are met. For covered Part B services, patients generally pay 20% of the Medicare-approved amount after the Part B deductible, although the exact bill depends on the provider’s assignment, facility type, other insurance and location. An RHC’s rural status does not by itself remove deductible or coinsurance rules.

Coverage arrangement What to verify before moving Potential consequence
Medicare Advantage Exact network providers and facilities, referral and authorization rules, urgent and emergency terms Out-of-network routine care can produce higher cost sharing or no plan payment
Original Medicare Provider accepts assignment; supplemental policy terms; local coverage rules for planned services You may owe the applicable deductible and coinsurance, and some services are excluded
Part D prescription coverage Local and mail-order pharmacy network, formulary status, preferred-pharmacy pricing and stock Out-of-network fills may require full payment or cost more
Employer or union retiree coverage Coordination with Medicare, creditable drug status and effects of changing coverage Changing Medicare or drug coverage can affect other retiree benefits

Protect prescription and retiree-benefit continuity

Give the pharmacist a complete medication list, including dose and formulation. Confirm that each drug is on the plan’s formulary, that the local pharmacy participates and that inventory or ordering times will support your refill schedule. Preferred pharmacies usually offer the lowest Part D cost; an out-of-network pharmacy may require you to pay the full amount and seek reimbursement.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If you have employer or union retiree insurance, contact the benefits administrator before changing anything. Ask how the plan coordinates with Medicare, whether Part A and Part B enrollment is required, whether prescription coverage is creditable and whether enrolling in or changing a drug plan could terminate other retiree benefits. Read the plan booklet and keep the administrator’s written response. Medicare’s guidance is available at Retiree insurance and Medicare.

Use telehealth as a supplement, not a substitute

Ask each clinician and your plan which virtual services are available in your state and plan. Medicare Part B covers eligible virtual check-ins, generally brief real-time communications of 10 minutes or less, and e-visits requested through an online patient portal. Medicare describes a general 20% share of the Medicare-approved amount after the Part B deductible for both, subject to your other coverage and circumstances. See virtual check-ins and e-visits.

Telehealth can reduce routine travel for medication reviews, follow-ups and some behavioral-health appointments, but it does not create local in-person capacity for examinations, imaging, procedures or emergencies. Confirm that your home has reliable internet, a suitable device and a private space; the availability and quality of broadband must be checked at the specific address.

HRSA’s Medicare Telehealth Payment Eligibility Analyzer says many flexibilities were extended through December 31, 2027, but warns that the analyzer is not updated to reflect those flexibilities. Use current HHS telehealth policy plus your plan and provider’s written eligibility decision rather than treating the analyzer as definitive.

Compare towns with one consistent scorecard

When two or more towns remain, use the same household-specific evidence for each. A simple table prevents an attractive housing price from hiding a care gap.

Decision axis Questions to answer for every town
Service breadth Are primary care, required specialties, diagnostics, emergency care, pharmacy and post-acute services available?
Access in practice Are providers accepting new patients? What are appointment waits, hours and referral destinations?
Travel burden What is realistic road time in normal and adverse weather to routine, emergency and specialty care?
Coverage fit Are the exact providers, facilities and pharmacies in your plan? What would out-of-network care cost?
Continuity and resilience Is there prescription supply, a backup hospital, dependable transport and a workable telehealth option if local services change?

Mark an item “unverified” rather than assuming it is available. A town should not pass your comparison if a single essential service has no confirmed provider, payment route or backup plan.

Final pre-move verification

  1. Reconfirm new-patient acceptance and appointment dates with every priority clinician.
  2. Ask the insurer to validate network status using the exact plan name, county and provider identification number.
  3. Transfer prescriptions only after the receiving pharmacy confirms participation, stock and refill timing.
  4. Obtain current hospital service and transfer information, then save the backup facility’s address and route.
  5. Review retiree-benefit responses and Medicare enrollment requirements with the benefits administrator.
  6. Save call notes, names, dates, plan documents and referral instructions in both paper and digital form.

For state-specific rural-health programs, contact your state Office of Rural Health. Medicare also directs people with retiree coverage to their benefits administrator and identifies the State Health Insurance Assistance Program (SHIP) for free Medigap counseling. Treat every confirmation as time-sensitive: repeat the calls if the move is delayed or your coverage changes.
