If you are considering a switch between Zepbound and Wegovy, ask your prescriber for an individualized stop-and-start plan. These are different medicines, their milligram strengths are not interchangeable, and current U.S. prescribing information does not provide a universal conversion dose or waiting period. Bring your exact product, strength, last injection date, treatment history, symptoms, other medicines and treatment goals to the appointment. Do not overlap or change doses on your own.
First, understand what is—and is not—known
Zepbound contains tirzepatide, while Wegovy contains semaglutide. Tirzepatide acts on both GIP and GLP-1 pathways; semaglutide is a GLP-1 receptor agonist. A 5 mg dose of one product is not equivalent to a 5 mg dose of the other, and the products should never be compared by milligrams alone.

The U.S. labels provide separate, product-specific escalation schedules. They do not establish a standard Zepbound-to-Wegovy or Wegovy-to-Zepbound conversion table, nor do they state one washout interval that applies to every patient. Only the prescribing clinician can select a starting dose and date after reviewing your prior dose, tolerance, treatment gaps and medical history.

| Point to compare | Zepbound | Wegovy |
|---|---|---|
| Active ingredient | Tirzepatide | Semaglutide |
| Receptor action | GIP and GLP-1 receptor agonist | GLP-1 receptor agonist |
| U.S. label context | Chronic weight management in adults with obesity, or overweight with a weight-related condition, alongside reduced-calorie eating and increased physical activity; also treatment of moderate-to-severe obstructive sleep apnea in adults with obesity | Formulation-specific indications and directions; the prescribed presentation determines the applicable instructions |
| Escalation | Usual starting dosage is 2.5 mg once weekly for four weeks; increases are 2.5 mg after at least four weeks, with the eventual dose guided by response and tolerability | Its own formulation-specific stepwise escalation schedule |
| Switching instructions | No universal conversion from or to Wegovy | The explicit switching section concerns Wegovy injection and Wegovy tablets, not a Wegovy-to-Zepbound or Zepbound-to-Wegovy conversion |
| Overlap | Label says coadministration with another tirzepatide-containing product or any GLP-1 receptor agonist is not recommended | Ask the prescriber how the previous medicine will be discontinued before this product begins |
These are label descriptions, not a recommendation for your switch. Your clinician may choose a plan that differs from a usual new-start schedule because of your prior exposure, side effects, treatment gap or another condition.

Writing this information down prevents an uncertain last-dose date from turning into an unsafe assumption.
Questions to ask during the appointment
1. “Why are we switching, and what are we trying to improve?”
Ask whether the change is driven by side effects, insufficient benefit, a changed health goal, supply, cost, insurance, convenience or another clinical reason. Then ask what outcome will define success and when it will be reviewed. The goal might involve weight-related health, glucose measures, sleep-apnea treatment, tolerability or another condition—not simply a number on the scale.

2. “What is my exact last dose, first new dose and start date?”
Request a written plan containing the final dose of the current medicine, the date it ends, the first dose of the new medicine, the strength, the injection day and any later increases. Ask how the plan changes if your last dose was late or if you have been off treatment for several weeks.

Zepbound’s usual new-start schedule begins at 2.5 mg once weekly for four weeks, with 2.5-mg increases after at least four weeks. Wegovy follows its own formulation-specific escalation schedule. Neither fact converts one medicine into the other. Do not select a dose by matching the milligram number or by copying an online schedule.
3. “Should I stop one before starting the other?”
Ask the clinician to state whether there will be a gap and, if so, the exact dates. Zepbound’s U.S. label says: “Coadministration with other tirzepatide-containing products or with any GLP-1 receptor agonist is not recommended.” Because Wegovy is a GLP-1 receptor agonist, do not intentionally overlap the products unless your prescriber gives explicit instructions.

Also ask what to do if you inject both by mistake, cannot remember which day you dosed, or receive conflicting instructions from a pharmacy. A particular waiting period cannot be recommended for everyone from the current labels.
4. “Which parts of my health history or other medicines change this plan?”
Ask the prescriber to review every medicine and supplement you take. Discuss medicines that lower glucose, because adding or changing therapy can alter the risk of low blood sugar in some treatment plans. Ask whether slowed stomach emptying or gastrointestinal symptoms could affect an oral medicine you rely on.
Both labels warn about thyroid C-cell tumors and contraindicate use in people with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2. Tell the clinician if either condition is possible in your family, rather than trying to interpret a vague family history yourself.

If you use oral contraceptives and are prescribed Zepbound, ask specifically about the label’s precaution: use a non-oral contraceptive method or add a barrier method for four weeks after starting Zepbound and for four weeks after each dose escalation. Your prescriber can determine whether that advice applies to your situation and discuss alternatives.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors5. “What side effects should I expect, and which symptoms need help?”
Ask which effects are common during initiation or dose increases and what home measures are appropriate. Discuss nausea, vomiting, diarrhea, constipation, abdominal pain and signs of dehydration. Ask how severe or persistent symptoms should be reported and which after-hours number to use.
The product labels include serious warnings and precautions, including pancreatitis and gallbladder-related problems. Rather than self-diagnosing from an internet symptom list, ask your clinician what symptoms in your history warrant urgent evaluation. Follow the individualized instructions you receive about contacting the office, urgent care or emergency services.
6. “Will the pen, dose preparation or missed-dose instructions be different?”
Ask which exact presentation will be dispensed and whether a nurse or pharmacist should demonstrate it. Device design, dose preparation, storage, needle handling and disposal instructions can differ. Do not reuse a pen, needle or dose step from one product with the other.
Have the clinician or pharmacist write the missed-dose rule for your exact prescription. Wegovy’s missed-dose directions are product-specific, and the correct action can depend on how many doses were missed. Ask what to do during travel, refrigeration problems or a delayed shipment.

7. “What if insurance will not cover it or I cannot obtain it on time?”
Ask who will check prior authorization, your expected out-of-pocket cost and local pharmacy availability. Request a contingency plan for a treatment gap, including whom to contact before taking a substitute dose or restarting.
Coverage and supply depend on your plan, prescription and location. Neither product label can promise insurance approval, a discount, a coupon or stock at a particular pharmacy. Get those details verified for you rather than relying on a general online claim.
8. “When do we follow up, and what should I track?”
Agree on a follow-up date and a way to reach the practice between visits. Ask which measures to record: weight-related goals, glucose or other relevant readings, appetite, sleep-apnea symptoms, gastrointestinal effects, injection dates and any changes in other conditions.
Ask what result would lead to holding an increase, changing the plan or investigating another cause. The schedule should reflect your indication, health history and response, not a generic calendar.

Possible thyroid-cancer history
Provide the exact diagnosis if you or a relative had medullary thyroid carcinoma or if multiple endocrine neoplasia syndrome type 2 is known in the family. These are label contraindications, so an uncertain history deserves clarification before a switch.
Compounded or nonstandard products
If your current medicine came from a compounding pharmacy, bring the vial, carton or pharmacy instructions. Ask the clinician to verify the exact product, concentration, dose, route and measuring device. The FDA has reported dosing errors and serious adverse events involving unapproved compounded GLP-1 products. Do not assume a compounded preparation and an FDA-approved product are interchangeable.
What a safe written switch plan should contain
Before leaving, check that the instructions answer each of these questions:
- What is the last dose and date of the current medicine?
- Is there an intentional gap, and what are the exact stop and start dates?
- What is the first dose and formulation of the new medicine?
- On which weekday and at what time should it be taken?
- When, if ever, will the dose increase, and what must be true before increasing?
- What should you do after a missed dose, vomiting, an uncertain dose or accidental overlap?
- Which symptoms require a same-day call, urgent care or emergency help?
- When is the follow-up, and who handles access or insurance problems?
Ask the office to put the plan in the after-visit summary or patient portal. A written instruction is safer than relying on memory, especially when the two products have different devices and schedules.
Why online conversion charts are risky
Switching evidence between GLP-1 receptor agonists remains limited. UK professional guidance from the NHS Specialist Pharmacy Service notes that limitation; it is not a U.S. dosing standard. A chart that assigns an equivalent dose or a fixed number of days between injections may ignore your last tolerated dose, escalation history, treatment gap, kidney or gastrointestinal concerns, other glucose-lowering medicines and the product presentation.
Current U.S. labels should be checked because indications, presentations, warnings and instructions can change. Use the label for the product actually prescribed and your clinician’s plan for the transition.
A concise appointment script
You can say: “I take [product and strength]. My last dose was on [date], and I have been on this strength for [time]. I want to switch because [reason]. I have had [symptoms or benefits], and I also take [medicines and supplements]. What is my exact stop date, first dose, start date, missed-dose plan, overlap plan and follow-up date?”
That wording gives the prescriber the facts needed to make an individualized decision without asking you to guess at dose equivalence.
