The potential harm depends on the specific substance, its purity and formulation, the route of administration, the seller’s intended-use claims, and how much reliable human safety information exists. U.S. Food and Drug Administration (FDA) materials describe concerns ranging from immune reactions and aggregation to impurities, inadequate characterization of the active ingredient, contamination risks for injectables, and serious adverse-event signals for particular substances. An “FDA-approved,” “compounded,” or “research use only” label can describe very different legal and safety situations.
The practical rule is straightforward: an FDA-approved drug has undergone FDA premarket review; a compounded drug is not FDA-approved and is intended for a patient-specific medical need; and an online product that FDA regards as an unapproved new drug does not carry the same assurances of safety and effectiveness. FDA recommends discussing treatment with a clinician, obtaining a prescription when compounding is medically necessary, and using a state-licensed pharmacy.

First, separate the product categories
“Peptide” describes a type of molecule, not a single safety class. FDA’s compounding materials discuss substances one by one and often distinguish among routes such as intravenous, subcutaneous, or other administration. The same name on a vial or website therefore does not establish that two products have equivalent identity, strength, purity, sterility, or clinical evidence.

| Category | FDA approval status | What the category means for a reader |
|---|---|---|
| FDA-approved drug | Approved through an effective FDA application | FDA has reviewed the application’s evidence for the approved uses, including safety, effectiveness, quality, manufacturing, labeling, and other requirements. |
| Compounded drug | Not FDA-approved | A pharmacy or outsourcing facility prepares a medication under a separate legal and regulatory framework. It has not undergone FDA premarket review as an approved drug. |
| Online product FDA considers an unapproved new drug | No effective FDA application for marketing | Marketing claims, labels, websites, and other circumstances may show an intended use in people. A “research use only” disclaimer does not by itself settle that question. |
These categories are not a general safety ranking. A product’s actual risk still depends on its substance, route, manufacturing controls, labeling, and the patient’s circumstances.

Why FDA’s peptide concerns are substance- and route-specific
Immune reactions and immunogenicity
A peptide can prompt the immune system to recognize the molecule, an impurity, an aggregate, or another component as foreign. FDA’s October 29, 2024 Pharmacy Compounding Advisory Committee materials explain that peptide immunogenicity can cause serious harm, potentially including life-threatening anaphylaxis. That material addresses risks in peptide compounding; it does not mean every peptide product or every patient will have such a reaction. The possibility is one reason identity, purity, formulation, sterility, route, and clinical evidence matter.
Aggregation, impurities, and difficulty identifying the active ingredient
FDA’s page on bulk drug substances considered for compounding lists several substance-specific concerns: aggregation, peptide-related impurities, and difficulty characterizing the active pharmaceutical ingredient. Aggregated or chemically altered material may behave differently from the intended molecule. Impurities can arise from synthesis, purification, storage, or degradation. If the active ingredient cannot be adequately characterized, a label alone cannot establish what a patient will receive.
Route changes the risk
FDA sometimes reports information for one route but not another. A study involving intravenous administration cannot automatically establish safety for a different injectable route. Conversely, the absence of identified information for a proposed route is not evidence that the route is safe. Readers should treat the route named in an FDA assessment as an essential qualification, not a technical footnote.
Limited evidence is not evidence of safety
For some substances, FDA reports limited or no safety-related information for the proposed route and says it lacks enough information to know whether human use could cause harm. That is an evidence limitation, not a finding that the substance is harmless. There is no general, reliable harm-rate statistic for unapproved peptides that can be applied across products.

Serious signals FDA has reported for selected substances
FDA’s descriptions below are not a universal list of peptide risks. They are examples tied to particular substances, studies, routes, and levels of uncertainty.
| Substance | FDA-described signal | Important qualification |
|---|---|---|
| CJC-1295 | Serious adverse events listed include increased heart rate and a systemic vasodilatory reaction. | FDA notes that clinical data are limited. |
| Ipamorelin acetate | A study of intravenous administration for improving gastric motility reported serious events, including death. | FDA says safety information for certain other injectable routes was not identified. The intravenous study should not be treated as proof that every route has the same outcome. |
| AOD-9604 | FDA notes serious adverse events that may be associated with the substance. | Causality is unclear; the signal should not be presented as proof that AOD-9604 caused every reported event. |
These entries illustrate why a broad statement such as “peptides are safe” or “peptides are dangerous” is misleading. The relevant questions are which molecule, what formulation, what route, what dose or exposure, what patient population, and what evidence.
Why injectable products receive a particular warning
FDA’s March 31, 2026 warning letter to Gram Peptides explains the route-specific concern: “Injectable products are delivered directly into the body, sometimes directly into the bloodstream, and therefore, bypass some of the body’s key defenses against toxins and microorganisms that can lead to serious and life-threatening conditions.”
This is a quality and contamination issue as well as a pharmacology issue. A sterile injectable that is contaminated, incorrectly identified, incorrectly concentrated, or otherwise defective can expose a person without the protective barriers that apply to many non-injectable products. The warning does not establish that every injectable peptide is contaminated or that an adverse event will occur. It explains why FDA treats injectable claims and manufacturing conditions as high-consequence concerns.

How FDA decides whether a marketed product is an unapproved drug
The basic marketing requirement
As a general rule, a new drug cannot be introduced into interstate commerce without an effective FDA application, subject to legal exceptions. FDA warning letters identify products and claims that the agency regards as unapproved new drugs, misbranded products, or both. A warning letter is directed to the firm and conduct described in that letter; it is not a finding that every product carrying a similar label has identical facts.
Intended use comes from the whole presentation
FDA can evaluate intended use through website statements, product names, labels, testimonials, social-media material, instructions, and other evidence. Claims that a peptide treats a disease, changes a body function, promotes weight loss, or produces a therapeutic effect can indicate intended use for people. The surrounding circumstances matter, not just one sentence printed on a vial.
Why “research use only” is not a complete shield
In its February 26, 2025 warning letter to USApeptide.com, FDA stated: “Unapproved new drugs do not carry the same assurances of safety and effectiveness as those drugs subject to FDA oversight.” The agency’s warning-letter approach makes clear that a “research use only” disclaimer does not itself determine whether a product is intended for human drug use. That does not mean every chemical sold with that phrase is automatically a drug; it means the disclaimer cannot override contradictory claims or circumstances.

| Question to ask | Approved drug | Compounded drug | Unapproved online product |
|---|---|---|---|
| Has FDA approved the drug? | Yes, for the approved labeling and conditions. | No. | No effective application for the marketed product, according to FDA’s determination in the relevant case. |
| Does FDA premarket-review safety, effectiveness, and quality? | Yes, as part of the approval process. | Not as an approved drug. | No FDA premarket review before marketing. |
| Is a patient-specific medical need relevant? | Approval covers the labeled use. | FDA recommends compounding only when the patient’s medical needs cannot be met by an FDA-approved drug. | A seller’s claims do not create a lawful compounding pathway. |
| What should a consumer verify? | Use the approved labeling and clinician guidance. | Prescription from a doctor and a state-licensed pharmacy. | Do not treat disclaimers, testimonials, or a website as proof of FDA oversight. |
What FDA says about compounded drugs
FDA describes compounding oversight as risk-based and emphasizes collaboration with states and other stakeholders. “Compounded” does not itself mean illegal, and the available FDA material does not provide a complete list of every condition or exception that may apply. It does mean the product is not FDA-approved and has not received the same premarket review as an approved drug.
FDA’s consumer guidance is specific: a compounded drug should be used only when a patient’s medical needs cannot be met by an FDA-approved drug. The patient should obtain a prescription from a doctor and have it filled by a state-licensed pharmacy. Those steps do not turn a compounded drug into an FDA-approved product; they provide the clinical and pharmacy oversight FDA recommends for situations in which compounding is medically necessary.
The GLP-1 alert is a bounded example, not a rule for every peptide
FDA’s alert on unapproved GLP-1 drugs concerns semaglutide and tirzepatide specifically. FDA says unapproved versions do not undergo FDA review for safety, effectiveness, and quality before marketing. The agency also warns about fraudulent compounded-product labels and reports of adverse events that may be related to use of amounts beyond FDA-approved labeling.
That alert should not be projected automatically onto every peptide or every compounded medicine. Its value is showing how FDA frames a concrete product category: the absence of premarket review, misleading labeling, and reports associated with use outside approved labeling are distinct concerns that must be evaluated in context. The alert includes a dated adverse-event figure as of May 31, 2026, but the numerical count is not reproduced here; no rate should be inferred from it.

A practical checklist before paying for or using a peptide product
Use the following questions for consumer literacy, not as a legal determination about a particular seller.
- Identify the category. Is the product an FDA-approved drug, a compounded drug, or an online product making drug-like claims without an effective FDA application?
- Read the claims, not only the disclaimer. Does the website, label, testimonial, or social-media material claim to treat a disease, alter a body function, or deliver a therapeutic result while also saying “research use only”?
- Check the clinical pathway. If compounding is proposed, is there a prescription from your doctor and a state-licensed pharmacy involved?
- Match evidence to substance and route. What does FDA say about this exact molecule and proposed route? Limited information should not be converted into a safety claim.
- Ask how identity and quality are established. FDA’s concerns include aggregation, peptide-related impurities, and difficulty characterizing the active ingredient. A marketing page cannot substitute for validated quality controls.
- Discuss the decision with a healthcare professional. Individual conditions, medicines, allergies, and treatment alternatives determine whether a product is appropriate.
- Do not use testimonials as surveillance data. Positive anecdotes cannot establish sterility, consistency, effectiveness, or population-level safety, and warning letters do not provide a general harm rate.
What FDA oversight can—and cannot—tell you
FDA oversight gives consumers meaningful distinctions, but it is not a guarantee that every approved treatment works for every person or that every compounded preparation will cause harm. Approval means FDA reviewed an application for the approved product and use. Compounding means a separate pathway without FDA approval. An FDA warning letter communicates the agency’s concerns about the facts and claims described in that letter.
There is also no single number that summarizes the danger of all unapproved peptides. FDA’s pages contain substance-specific evidence and, in the GLP-1 alert, dated adverse-event reporting, but those materials do not support calculating a general risk percentage. Treating an absence of published data as reassurance is especially inappropriate when FDA says it lacks enough information to know whether human use could cause harm.

Unapproved peptide risk is determined by the molecule, formulation, route, manufacturing quality, evidence, and intended use—not by the word “peptide” alone. FDA has identified potential immunogenicity, aggregation, impurities, characterization problems, and route-specific safety gaps, along with serious signals for certain substances. For injectables, contamination or toxins can bypass normal defenses. A research disclaimer does not override human-use claims, and compounded drugs are not FDA-approved. When an FDA-approved option cannot meet a documented medical need, FDA recommends a doctor’s prescription and a state-licensed pharmacy, with treatment decisions made alongside a healthcare professional.
