Long-term care (LTC) insurance helps pay for extended personal, custodial and supportive care when a policyholder cannot safely manage certain daily activities or has qualifying cognitive impairment. A policy may cover care at home, in adult day services, assisted living, a nursing home and other settings, but only after the contract’s benefit trigger and elimination period are satisfied. Daily and lifetime limits, exclusions and premiums can still leave substantial costs for the policyholder. Medicare is not a general long-term custodial-care benefit, while Medicaid may help eligible people under state-specific rules.
What is long-term care insurance?
Long-term care insurance is designed to pay for long-term services and supports rather than short-term treatment of an illness or injury. The Administration for Community Living (ACL), part of the U.S. Department of Health and Human Services, describes it this way: “Unlike traditional health insurance, long-term care insurance is designed to cover long-term services and supports, including personal and custodial care in a variety of settings such as your home, a community organization, or other facility.”

Traditional health insurance generally concentrates on acute medical care and skilled treatment. LTC insurance addresses the ongoing help a person may need with bathing, dressing, eating, toileting, transferring, continence or supervision because of cognitive impairment. The exact definition of covered care, the providers who may deliver it and the documentation required are controlled by the individual policy and the law in the state where it was issued.
Most policies are not unlimited. They usually specify a daily or monthly benefit, a total or lifetime maximum, an elimination period before payments begin and conditions that must be met before a claim is approved. A policy can therefore cover a meaningful portion of care without paying every bill.

What long-term care insurance covers
Comprehensive policies commonly cover several settings. A policy may reimburse eligible expenses or provide a stated cash amount when the insured qualifies. “Commonly” does not mean guaranteed: check the schedule of benefits, definitions and exclusions in your contract.
| Setting or service | What a comprehensive policy may pay for | Important contract question |
|---|---|---|
| Care at home | Skilled nursing, physical or other therapies, and personal-care assistance such as bathing and dressing. Some policies also provide homemaker services. | Is homemaker help available by itself, or only while personal-care services are being received? Are there minimum visit lengths or provider requirements? |
| Adult day services | Supervised daytime care, activities and assistance for people who return home at night. | Does the facility need a particular license or certification, and are transportation costs included? |
| Assisted living | Room, board and supportive services in an assisted-living community, subject to the policy’s covered-expense rules. | Which room-and-board charges and levels of assistance qualify, and are noncare amenities excluded? |
| Alzheimer’s or special-care facilities | Care in a facility specializing in Alzheimer’s disease or other cognitive conditions. | Does the policy require a separate cognitive-impairment diagnosis or a facility with a specified license? |
| Nursing homes | Skilled, intermediate or custodial services in a nursing facility, depending on the contract. | Which nursing-home levels and bed types are eligible, and is a licensed facility required? |
| Hospice | Hospice services, which may be delivered at home or in a facility, when the policy includes them. | How does the LTC benefit coordinate with Medicare hospice or other coverage? |
| Respite care | Short-term replacement care that gives an unpaid family caregiver a break. | Is respite limited to a number of days, a dollar amount or a separate annual allowance? |
Home benefits are often broader than a simple nursing visit. They can include help with activities of daily living, therapy and skilled nursing. Homemaker services—such as meal preparation or light housekeeping—may be conditional on the insured also receiving covered personal care. Supplies, equipment, transportation and room-and-board charges may be treated differently from hands-on care, so the policy’s definitions matter.
What must happen before an LTC policy pays?
The benefit trigger must be met
Needing help is not, by itself, enough. Most tax-qualified policies use a benefit trigger based on an inability to perform at least two of six activities of daily living (ADLs), or on qualifying severe cognitive impairment requiring substantial supervision. The six ADLs are bathing, dressing, eating, transferring, toileting and continence.

Definitions differ. For example, one contract may require hands-on assistance, while another may recognize substantial supervision because of dementia. Ask the insurer who performs the assessment, what records are needed and how often eligibility is reviewed.

The elimination period must run
After the trigger is satisfied, an elimination period—similar to a waiting period—usually applies. During it, the policyholder may have to pay the full cost of eligible care. The contract determines whether the period is measured in calendar days, days of receiving care or another method, and whether days in different settings count together. Some policies count services provided on a day; others require the insured to incur a covered expense. A longer elimination period can reduce premiums but increases the amount that must be self-funded at the start of a claim.
Care must meet the contract’s rules
Even after the trigger and elimination period, the service generally must be eligible under the policy. That can include requirements for a licensed facility, an approved home-care agency, a written plan of care, receipts or prior authorization. A claim can be delayed or denied if the provider, setting, diagnosis or documentation does not match the contract.
How benefits are paid—and where the limits apply
Reimbursement and cash-benefit designs
A reimbursement policy pays eligible expenses up to the policy’s daily or monthly limit after the insured submits documentation. If covered care costs less than the limit, the insurer generally pays the covered amount rather than the unused balance. A cash-benefit policy pays a stated amount for each qualifying day, often without requiring proof that the entire amount was spent on listed services. The policy determines whether cash can be used for informal caregivers, transportation, supplies or other expenses.

Many policies set a maximum benefit per day and a total pool of benefits. A lifetime maximum may be expressed as a number of covered days or as a total dollar amount. The duration depends on how quickly benefits are used: a high daily limit can exhaust a fixed pool sooner, while a lower limit may last longer but cover less of each bill.

Hypothetical example: If a covered service costs more than the policy’s daily cap, a reimbursement policy pays only up to that cap, assuming every claim condition is met. The policyholder pays the difference. The same person may also owe the entire bill during the elimination period and after the total benefit pool is exhausted.
Inflation protection changes the real value
Inflation protection increases the daily or total benefit over time according to the option selected in the contract. Without it, a benefit that looked adequate when the policy was purchased may cover a smaller share of future care costs. Inflation features can raise premiums, and the available choices differ by policy and state.
What costs may remain your responsibility?
- Care during the elimination period: You may pay eligible bills yourself until the required waiting period is completed.
- Amounts above the daily or monthly benefit: A reimbursement policy generally does not pay the portion of a covered bill above its stated limit.
- Care after the total benefit is exhausted: Once the lifetime or total pool reaches zero, the policy no longer pays covered claims unless the contract provides another benefit.
- Excluded services, supplies or charges: A policy may exclude certain equipment, transportation, noncare amenities, room upgrades, informal care or other items. The exclusion list controls.
- Care that does not satisfy the trigger: If an assessment does not show the required ADL limitations or cognitive impairment, benefits may not start even when assistance would be helpful.
- Premiums: Premiums are a continuing cost while a policy is in force. The ACL notes that policyholders typically are not expected to keep paying premiums while receiving long-term care, but a waiver-of-premium provision and its effective date must be confirmed in the contract.
- Rate increases: Premiums can rise if the assumptions used in pricing prove wrong and the insurer receives regulatory approval. A policy’s past rate-increase history is an important affordability check.
These are potential exposures, not a universal list of exclusions. One contract may cover an item another excludes, and state insurance rules can add requirements.
What happens when long-term care insurance runs out?
When the remaining daily or lifetime benefit reaches zero, the insurer generally stops paying under that benefit. The person then must fund ongoing care from income, savings, investments, family support or another source. If the policy has a residual, shared or restoration feature, its wording determines whether any additional benefit is available; do not assume that a new period of eligibility creates a new lifetime pool.

Before the maximum is reached, ask the insurer for a current benefit ledger showing the original pool, benefits paid, inflation adjustments and projected exhaustion date. Reassess care settings and public-program eligibility early rather than waiting for a final claim denial. Medicaid may become relevant for an eligible person, but its financial and functional rules are state-specific.
What does Medicare cover?
Medicare is not a general long-term custodial-care program. Under limited conditions, it may cover skilled nursing-facility care after a qualifying inpatient hospital stay and when qualifying skilled care is required. It may also cover medically necessary skilled home-health services when the program’s conditions are met.
Those benefits are intended for defined skilled-care situations, not indefinite help with bathing, dressing, eating, supervision or other custodial needs. Medicare’s limited coverage therefore should not be treated as a substitute for an LTC policy or for a plan to pay for extended personal care. Coverage rules and cost-sharing can change; use the current Medicare guidance or free State Health Insurance Assistance Program (SHIP) counseling for an individual case.
How Medicaid fits in
Medicaid, administered jointly by the federal government and the states, may pay for nursing-facility services and home- and community-based services for people who meet financial, functional and other eligibility rules. Service availability, income and asset standards, waiting lists and application procedures vary by state.

In the context described by the ACL, Medicaid does not pay a person’s rent, mortgage, utilities or food. A person receiving home- and community-based services may therefore still need income or other resources for ordinary living expenses. Contact the Medicaid office in your state for current eligibility and covered-service information.
Partnership-qualified policies
Some states participate in the Long-Term Care Partnership program. A partnership-qualified policy may provide an asset disregard tied to the benefits the policy has paid, allowing an eligible applicant to retain specified assets when applying for Medicaid. The amount disregarded, qualifying policy features and Medicaid treatment are governed by state rules. Confirm partnership status before buying and before relying on it in an application.
Policy features to compare before buying or keeping coverage
There is no universally best configuration. A higher benefit or longer protection period can preserve more assets but usually costs more. Compare offers using the same assumptions and ask how each feature affects both claims and premiums.

|---|---|---|
| Benefit trigger | How many ADLs must be impaired? How is cognitive impairment defined? Who certifies eligibility? | Determines whether a claim can begin and what evidence is required. |
| Elimination period | How many days are required, and do calendar days or service days count? Does the period apply once or per episode? | Sets the amount you must fund before benefits start. |
| Eligible settings and services | Are home care, adult day, assisted living, special-care facilities, hospice, respite and nursing homes included? Which providers qualify? | A broad benefit is useful only if it follows the care setting you are likely to use. |

| Daily or monthly amount | What is the initial limit, and how does inflation protection increase it? | Shows how much of a current and future bill the policy can cover. |
| Total or lifetime benefit | Is the maximum measured in dollars, days or another pool? Is it shared across settings? | Determines how long benefits can last and when they end. |
| Inflation protection | What increase method and rate apply, and can you reduce or decline the option later? | Protects purchasing power but affects premiums. |
| Premium affordability and history | What is the current premium, how often can it change, and what rate increases has this policy form experienced? | A policy that cannot be kept in force cannot protect future care costs. |
| State Partnership status | Is the policy partnership-qualified in your state, and what asset disregard would apply? | May affect future Medicaid planning, subject to state rules. |
Putting older cost statistics in context
An ACL cost page reproduces statistics from 2007: an average annual policy cost of $2,207, an average covered-benefit duration of 4.8 years excluding the 20 percent who elected lifetime coverage, and an average daily benefit of $160. The same page reports that 20 percent elected lifetime coverage. The page was last modified February 18, 2020, and these figures are historical—not current market averages. They should not be used to estimate today’s premium, benefit amount or expected claim duration.
A practical contract-review checklist
- Read the definitions of each ADL, cognitive impairment, covered care and provider.
- Ask the insurer to explain exactly how the elimination period is counted and when premium waiver begins, if available.
- Confirm whether the policy reimburses invoices or pays a cash benefit, and list the documents required for a claim.
- Write down the current daily or monthly limit, total or lifetime maximum, inflation option and any shared-benefit rules.
- Request the policy’s exclusions and limitations, including rules for room and board, supplies, transportation, informal caregivers and respite care.
- Ask for the insurer’s premium rate-increase history and whether the proposed premium remains affordable under a higher-rate scenario.
- Check whether the policy is Partnership-qualified in your state and what conditions apply.
- Compare policies without pressure. A state-licensed LTC insurance agent, broker or financial planner can explain contracts, but the decision should fit your budget, health situation and other resources.
- Keep the insurer’s claims department, your state insurance department, SHIP and state Medicaid office contact information with the policy.
Policy language controls every claim. Before relying on a benefit, obtain the answer in writing from the insurer and verify current state rules with the appropriate public agency.
