Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Theranostics is a paired diagnosis-and-treatment approach that uses the same biological target to find disease and then deliver radiation to it. A low-dose radiotracer, usually imaged with PET, reveals whether a tumor carries the target. If it does, a chemically related radiopharmaceutical carrying a therapeutic isotope can be administered to irradiate target-positive cells throughout the body.
This makes the scan more than an anatomical picture: it is a biologic selection test. Uptake indicates that the matching treatment has a route to the disease, while absent or insufficient uptake can spare a patient an ineffective radioligand treatment. Theranostics is performed by licensed nuclear-medicine and oncology teams, not as a home treatment or consumer test.

What “theranostics” means
The term combines “therapy” and “diagnostics.” The Society of Nuclear Medicine and Molecular Imaging describes it as combining diagnostic imaging and targeted treatment into one process. The International Atomic Energy Agency similarly characterizes isotope-based theranostics as combining diagnosis and therapy so professionals can focus treatment on an individual patient’s disease biology.

The approach uses a targeting molecule—for example, a peptide, small molecule or antibody—that binds a receptor, transporter or other marker on tumor cells. For diagnosis, that molecule carries a radionuclide whose emissions can be detected by PET, SPECT or a gamma camera. For treatment, a chemically similar molecule carries a beta- or alpha-emitting radionuclide. The therapeutic isotope releases ionizing radiation close to cells displaying the target.

The targeting molecule and target remain broadly the same, but the isotope and purpose change. The diagnostic product maps target expression and disease distribution; the therapeutic product delivers a radiation dose to target-positive sites. Because the drug circulates through the body, it can reach both visible and microscopic target-positive deposits, although normal organs that take up or retain the agent can also receive radiation.
How a theranostic treatment works, step by step
-
Define the disease target and clinical question
The team first determines which marker is relevant to the suspected or known disease. The question may be whether a receptor is present, where all target-positive lesions are located, whether disease has spread, or whether a patient meets the criteria for a particular approved radioligand.
-
Administer the diagnostic radiopharmaceutical
A tracer is commonly injected into a vein. After an uptake interval, the patient undergoes PET or SPECT imaging. The scanner detects emissions from the tracer and creates a map of target expression. Imaging protocols, timing and preparation differ by product and institution.

Confirm target expression and disease distribution
Nuclear-medicine specialists review whether lesions take up the tracer and whether uptake is sufficient and widespread enough for the proposed therapy. The scan can identify target-positive metastases that are difficult to distinguish on structural imaging and can also reveal target-negative disease that the therapy would not be expected to treat.

-
Check eligibility and prepare the patient
Target uptake is necessary but not the only requirement. Clinicians review blood counts, kidney and liver function where relevant, prior treatments, performance status, concurrent medicines and agent-specific contraindications. Pregnancy status and the possibility of breastfeeding must be addressed because ionizing radiation can harm a developing fetus or expose an infant.
-
Give the therapeutic radiopharmaceutical
The treatment isotope is administered intravenously by a trained team in a facility equipped for radiopharmaceutical handling. Depending on the agent, the center may use premedication, intravenous fluids, amino-acid or other renal-protection measures, individualized dosimetry and radiation-monitoring procedures. The therapeutic compound travels to cells that display the target and irradiates them.
-
Monitor response and toxicity
Follow-up combines clinical assessment, laboratory testing and imaging. Teams watch for falling blood counts, kidney effects, liver or other organ effects, changes in symptoms and evidence of tumor response. The timing and type of follow-up are specific to the radiopharmaceutical and the patient’s condition.

How a PET scan determines whether therapy is possible
A PET scan for theranostics is interpreted as a target-expression study, not simply as a search for abnormal anatomy. The diagnostic molecule binds the same or a closely matched target used by the treatment molecule. Areas that accumulate tracer show that the molecular “address” is present and accessible to the drug.

- Positive, sufficiently intense uptake: supports the possibility that the matching radioligand can reach those lesions.
- Heterogeneous uptake: may indicate that some lesions are target-rich while others are target-poor. The team must consider whether untreated target-negative disease could still drive progression.
- Absent or inadequate uptake: generally argues against using that target-specific therapy, because the drug would have too few binding sites.
- Unexpected distribution: can change staging, reveal additional disease or identify normal organs that naturally take up the tracer and therefore require dose and safety consideration.
The exact uptake threshold, image-reading rules and approved companion product depend on the therapy. A scan does not by itself guarantee benefit; eligibility also depends on organ function, marrow reserve, previous therapy and the prescribing label for the agent.
Established examples of theranostics
| Theranostic example | Diagnostic selection | Therapeutic use and schedule | Key qualification |
|---|---|---|---|
| Lutetium Lu 177 dotatate (LUTATHERA) | Imaging demonstrates somatostatin-receptor expression. | Radiolabeled somatostatin analog for adults and children aged 12 years and older with somatostatin-receptor-positive gastroenteropancreatic neuroendocrine tumors (GEP-NETs). The U.S. prescribing label specifies 7.4 GBq (200 mCi) every eight weeks for four doses. | Intravenous administration and handling by providers trained in radiopharmaceutical safety; kidney and blood-count monitoring are required. |
| Lutetium Lu 177 vipivotide tetraxetan (PLUVICTO) | PSMA PET using Locametz (gallium Ga 68 gozetotide) or another approved PSMA PET product demonstrates PSMA-positive disease. | For specified PSMA-positive metastatic prostate-cancer populations, the 2025 U.S. label lists 7.4 GBq (200 mCi) every six weeks for six doses for the mCRPC indication. | Intravenous radioligand therapy with warnings for myelosuppression and renal toxicity; patient selection must follow the current label. |
| Radioactive iodine | Thyroid cells’ natural ability to take up iodine provides the biologic targeting mechanism. | Long-established targeted nuclear-medicine treatment for selected thyroid diseases. | Use, dose and precautions vary with the thyroid condition and local regulations. |
LUTATHERA received initial U.S. approval in 2018, and PLUVICTO received initial U.S. approval in 2022. On July 31, 2026, the U.S. Food and Drug Administration announced an additional PLUVICTO indication in combination with androgen-receptor pathway inhibitor therapy for PSMA-positive metastatic prostate cancer that is androgen-pathway-modulation naïve or sensitive. The precise eligibility criteria and sequencing must be checked in the current prescribing information.
Is theranostics the same as nuclear medicine?
No. Nuclear medicine is the broader medical specialty that uses radioactive substances for imaging and treatment. It includes diagnostic scans such as conventional bone scans, PET and SPECT, as well as therapies such as radioactive iodine and other radiopharmaceutical treatments.
Theranostics is a strategy within nuclear medicine in which the diagnostic and therapeutic products are deliberately linked by a shared biological target. A routine nuclear-medicine scan may show anatomy or physiology without having a matching treatment. A theranostic scan is intended to answer whether a corresponding targeted radiotherapy is likely to reach the patient’s disease.

Which cancers can theranostics treat?
Established use is concentrated in cancers for which a validated target, an approved radiopharmaceutical and a reliable imaging test exist. Current examples include:
- Gastroenteropancreatic neuroendocrine tumors: somatostatin-receptor-positive disease can be treated with lutetium Lu 177 dotatate in the approved age groups and settings.
- Metastatic prostate cancer: PSMA-positive disease can qualify for lutetium Lu 177 vipivotide tetraxetan under the applicable U.S. indication and treatment line.
- Selected thyroid diseases: radioactive iodine exploits iodine uptake by thyroid tissue and represents one of the earliest targeted nuclear therapies.
Many other targets and cancer types are being studied, but investigation does not equal approval. Availability, reimbursement, eligibility rules and isotope supply differ by country and by center. A patient should ask whether a proposed use is on-label, available through a clinical trial or being offered under another lawful access pathway.
What radiation does the treatment use?
Diagnostic tracers use radionuclides selected for detectable emissions and a relatively low administered activity. PET detects pairs of photons produced after positron emission; SPECT and gamma cameras detect gamma rays. These scans are designed to create images while limiting diagnostic radiation to the amount needed for the study.
Therapeutic agents use isotopes that release particulate radiation, usually beta particles or alpha particles. Beta radiation travels farther through tissue and can treat a small cluster of nearby cells. Alpha radiation travels a much shorter distance but deposits very high energy over that path. The choice affects range, tumor-cell killing, normal-organ exposure, dosimetry and facility requirements. The isotope, not merely the targeting molecule, determines much of the radiation behavior.

Benefits and limits of the approach
Why clinicians use it
- The scan can select patients whose tumors display the target, reducing exposure to a treatment unlikely to bind.
- Systemic circulation allows treatment of multiple target-positive lesions, including sites too small or numerous for local therapy.
- Imaging can reveal biologic heterogeneity and help plan treatment, rather than relying only on tumor size.
- Repeated imaging and laboratory follow-up provide a way to assess both disease response and treatment toxicity.
What it cannot guarantee
- Tracer uptake does not prove that every lesion will respond or that the response will be durable.
- Target-negative or low-target disease may progress despite treatment of target-positive sites.
- Normal organs can express the target, metabolize the drug or receive circulating radiation, creating dose-limiting toxicity.
- Access depends on an appropriately licensed center, trained personnel, isotope availability and the rules in the patient’s jurisdiction.
Side effects, monitoring and radiation safety
Theranostic radiopharmaceuticals are prescription medicines with both drug-related and radiation-related risks. The specific profile depends on the molecule, isotope, cumulative dose and the organs that take up or clear it. Blood tests are used to detect marrow suppression, and kidney-function tests help identify renal injury or determine whether another dose is safe. PLUVICTO labeling includes warnings for myelosuppression and renal toxicity; LUTATHERA labeling also requires trained handling and ongoing monitoring.
Radiation-safety instructions are individualized. A center may ask the patient to drink fluids and urinate frequently to reduce residence time in the bladder, follow temporary distance or contact limits, use careful hygiene and follow instructions for handling bodily fluids. Restrictions can be especially important around children and pregnant people. The nuclear-medicine team should provide written instructions that account for the administered activity, isotope, home circumstances and national regulations.
- Tell the team about pregnancy, possible pregnancy or breastfeeding before imaging or treatment.
- Provide a complete medication and treatment history, including prior chemotherapy, radiation and other radiopharmaceuticals.
- Keep scheduled blood-count and kidney-function appointments; a normal feeling does not rule out laboratory toxicity.
- Report fever, unusual bleeding, severe fatigue, reduced urination or other urgent symptoms according to the center’s instructions.
- Do not alter contact precautions, hydration plans or medication instructions without speaking with the treating team.
What happens on a treatment visit?
Although protocols vary, a visit commonly includes identity and eligibility checks, review of recent laboratory results, pregnancy screening when relevant, placement of an intravenous line, administration of supportive medicines or fluids, delivery of the radiopharmaceutical and a period of observation. Staff use shielding, contamination controls and dosimetry procedures appropriate to the isotope. Some centers measure radiation levels before discharge or provide a card documenting the treatment.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesAt home, the patient follows the center’s written precautions for the specified period. The duration is not universal: it depends on the isotope, administered activity, clearance, living arrangements and local rules. A patient should obtain explicit instructions rather than relying on generic internet advice.
How theranostic options should be compared
Two radioligand treatments cannot be compared safely by dose alone. The clinically meaningful questions are:
| Comparison axis | Why it matters |
|---|---|
| Target and uptake threshold | Determines which lesions can bind the drug and whether the scan meets the treatment-selection rule. |
| Diagnostic isotope and modality | PET, SPECT or gamma-camera imaging has different resolution, availability and tracer characteristics. |
| Therapeutic isotope and radiation type | Beta or alpha emissions differ in tissue range, energy deposition and normal-organ exposure. |
| Approved disease and treatment line | An agent approved for one cancer or prior-treatment setting is not automatically appropriate in another. |
| Dose and schedule | For example, the U.S. labels specify 7.4 GBq (200 mCi) every eight weeks for four LUTATHERA doses and 7.4 GBq (200 mCi) every six weeks for six PLUVICTO doses for the cited indications. |
| Kidney and marrow safety | Baseline function and cumulative exposure can determine whether treatment starts, pauses or stops. |
| Dosimetry and radiation protection | Facilities need trained staff, licensed handling, waste procedures and patient-release protocols. |
| Availability | Access depends on an appropriately licensed center, local approval, isotope supply and payer or program rules. |
Questions to ask a nuclear-medicine team
- What exact target is being measured, and what uptake result is required?
- Will the scan show all disease sites, or could some lesions be target-negative?
- Is the proposed treatment approved for my cancer, age and prior-treatment setting?
- Which blood tests and kidney tests are required before every cycle?
- What hydration, renal-protection and anti-nausea measures will be used?
- How long will contact, travel, work or childcare precautions last in my circumstances?
- Whom should I call for fever, bleeding, severe weakness, reduced urination or other urgent symptoms?
- How will response be measured, and when will follow-up imaging occur?
Bottom line
Theranostics links a target-specific scan to a target-specific radiotherapy. The diagnostic tracer shows whether the disease carries the molecular address; the therapeutic radiopharmaceutical uses that address to deliver beta or alpha radiation to target-positive cells. It is a personalized nuclear-medicine process with established applications in receptor-positive neuroendocrine tumors, PSMA-positive metastatic prostate cancer and selected thyroid diseases. Eligibility, dose, monitoring and radiation precautions must be determined by a qualified team under the current product label and local regulations.
