It is not one universal statistic: in insurance it can mean no policy, insufficient limits or an excluded peril; in health it can mean missing essential services or unaffordable out-of-pocket costs; in social protection it can mean no benefit, ineffective access or an inadequate payment.
The people most exposed are generally those least able to absorb a loss and those excluded from the relevant scheme. Which group ranks as most at risk depends on the hazard, the country and the type of protection being measured.

What the protection gap means in different contexts
“Protection gap” is a descriptive term, not the name of a single global index. Always identify the domain before comparing figures. A household can have health coverage but still face a medical-cost gap; a homeowner can have insurance but an insured sum below the cost of rebuilding; and a worker can have a legal entitlement that is difficult to claim or too short-lived to prevent hardship.
Insurance protection gaps
In insurance, the gap is the difference between a loss that would be economically damaging and the protection a policy actually supplies. A person may have no policy at all, a policy with a limit below the likely loss, or a policy that excludes the event. A high deductible, coinsurance or other cost-sharing can leave a substantial amount payable after a claim. Insurance can also be unavailable or unaffordable in an exposed location, such as an area facing repeated floods, storms or earthquakes.

Health financial-protection gaps
Health protection has at least two separate dimensions. Service coverage asks whether people can obtain essential health services. Financial protection asks whether paying for care forces them to cut basic spending or otherwise suffer financial hardship. These measures overlap for some households but are not interchangeable, and neither is simply an insurance enrollment rate.
Social-protection gaps
Social protection includes cash and in-kind benefits such as maternity, unemployment, disability, old-age and employment-injury support. A gap can exist when a person is outside the legal rules, cannot receive a benefit in practice, receives it for too short a period, or receives too little to meet basic needs. The International Labour Organization distinguishes legal coverage from effective receipt and from the adequacy and duration of payments.
| Domain | What is measured | Typical causes of a gap |
|---|---|---|
| Insurance | Whether a policy and sufficient limit respond to a likely loss | No policy, unaffordable availability, exclusions, caps, deductibles or underinsurance |
| Health | Access to essential services and the financial burden of out-of-pocket payments | Missing services, point-of-care charges and insufficient prepaid or pooled financing |
| Social protection | Legal entitlement, effective receipt, adequacy and duration of benefits | Exclusion from a program, administrative barriers, short duration or inadequate payments |
How large is the health protection gap?
Service coverage: 4.6 billion people in 2023
The World Health Organization estimated that 4.6 billion people lacked coverage for essential health services in 2023. This is a service-coverage estimate: it indicates that people were not assured access to the essential services included in the measure. It should not be described as the number of people without health insurance, because the indicator covers service access rather than a particular insurance product.
Financial hardship: 2.1 billion people in 2022
WHO estimated that 2.1 billion people experienced financial hardship from out-of-pocket health spending in 2022. The figure captures the consequences of paying directly for care. A household can therefore appear to have access to services yet still experience a protection gap when the bill consumes money needed for food, housing, education or other essentials.

The two totals answer different questions
The 4.6 billion and 2.1 billion figures use different concepts and reference years. They must not be added together or presented as a single headcount. Some people may be in both populations, while others may lack services without incurring recorded spending or may receive services but face unaffordable payments. WHO periodically revises methods and indicators, so estimates from earlier editions may not be directly comparable with the revised 2025 presentation.
Who is most at risk?
People with the lowest incomes
Income is the clearest gradient in the health-financial-protection evidence. WHO reported that three in four of the poorest people experienced financial hardship from health-care costs in 2022, compared with one in 25 of the wealthiest. The hardship measure compares out-of-pocket spending with a household’s discretionary resources, so the same bill can be manageable for a high-income household and devastating for a low-income household.
Low-income households also tend to have fewer savings and less access to credit. A medical payment can therefore trigger debt, missed rent or mortgage payments, reduced food spending or withdrawal from school and work. The risk is greatest when a household has both poor service access and high charges for the care it can obtain.

1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsPeople paying at the point of care
Anyone who must pay a large share of treatment costs directly is exposed, even if they are formally enrolled in a health scheme. Deductibles, copayments, balance bills, medicine costs and transport can all create a gap. WHO describes financial protection as removing financial barriers to needed care and protecting people from payments that compromise basic needs or substantially reduce their ability to afford other goods and services.

People missing social-protection benefits
ILO figures illustrate distinct uncovered groups. An estimated 85 million women with newborns lacked a cash maternity benefit; 2.3 billion workers lacked employment-injury protection; and 157 million unemployed people lacked unemployment cash benefits. These are separate populations and must not be added as though they were mutually exclusive. A person can also face a duration or adequacy gap even when a benefit exists on paper.
People with severe disabilities who receive no disability benefit are another exposed group identified by the ILO. The scale and eligibility rules vary by country, so a global ranking across disability, maternity and unemployment risks would be misleading without national program data.
People in catastrophe-prone places without affordable cover
Natural catastrophes can create a protection gap where insurance is unavailable, excluded or priced beyond household and business budgets. The International Association of Insurance Supervisors treats catastrophe protection gaps as a financial-stability and supervisory concern because repeated uninsured losses can weaken households, firms, insurers and public finances. Exposure differs sharply by hazard and location; the broad global evidence does not establish one country or community as universally most at risk.

People whose policies look adequate but are not
Nominal coverage can conceal underinsurance. A building policy may use an outdated rebuild value, a contents policy may apply a low sublimit to valuables, or a liability policy may have a limit below a plausible judgment. Exclusions for floods, earthquakes, wear and tear, pandemics or gradual damage can remove the very event a household fears. The gap becomes visible only when the loss occurs and the policy responds for less than the economic damage.

How protection gaps are measured
WHO’s health-financial-hardship indicator
For Sustainable Development Goal indicator 3.8.2, WHO’s revised 2025 definition counts people in households with positive out-of-pocket health expenditure exceeding 40% of the household’s discretionary budget. It is a burden indicator, not a test of whether a person holds insurance. WHO data can be disaggregated by residence, income or consumption quintile, age and sex of the household head, and household age structure. Read the service-coverage measure alongside this financial indicator to avoid treating access and affordability as the same outcome.
An insurance diagnostic
There is no universal protection-gap score for every insurance risk. Assess the questions separately:
- Is there any policy? Check whether the person, asset or activity is insured for the relevant peril.
- Is the limit adequate? Compare the insured sum with a realistic replacement cost, liability exposure or income loss.
- What is excluded or capped? Read exclusions, sublimits, waiting periods and geographic restrictions.
- What must the policyholder pay? Record the deductible, excess, coinsurance and any cost-sharing formula.
- Can the cover be obtained and renewed? Availability and affordability in the exposed location matter as much as the wording.
Social-protection measurement
For a benefit program, separate three tests. Legal coverage asks whether the rules include the person. Effective coverage asks whether the person can actually enroll and receive payment, including whether documentation or administrative procedures create barriers. Adequacy and duration ask whether the amount and payment period protect living standards for as long as the risk lasts. A program can pass the first test and fail the other two.
Why having coverage does not always mean being protected
Participation in a scheme is only the starting point. Protection depends on the amount paid, the timing of payment and the costs left with the household. In health care, pooled and prepaid financing can reduce exposure to sudden bills, but a plan with narrow provider networks or high cost-sharing may leave patients delaying care. In insurance, a policy can respond to a claim yet still leave a large shortfall through depreciation, a deductible or an inadequate limit. In social protection, a benefit can be legally available but inaccessible to informal workers or too small to cover essential expenses.

This is why closing a protection gap cannot rely on private insurance alone. GFIA President Susan Neely has said: “Closing the gaps will require action from policymakers to create environments in which risks can be managed and mitigated. These actions will help keep risks insurable and insurance protection affordable.” Public financing, regulation, risk reduction and well-designed benefits all influence whether protection is available and sustainable.
A practical checklist for households
Use the checklist below to identify the gap relevant to your own situation. It does not replace country-specific advice, because eligibility, exclusions and prices differ by jurisdiction.
- Name the loss. Write down the event that could threaten your finances: illness, disability, job loss, death, property damage or liability.
- Estimate the financial consequence. Include immediate bills, lost income, replacement costs, debt payments and any care or transport expenses.
- List every existing source of protection. Include public benefits, employer programs, insurance policies, savings and legal entitlements.
- Check the conditions. Note eligibility, waiting periods, exclusions, documentation, provider restrictions, deductibles and payment limits.
- Measure the remaining shortfall. Compare the likely cost with the amount and timing of help actually available.
- Test affordability and continuity. A plan that cannot be maintained, renewed or accessed where you live is not dependable protection.
- Prioritize high-severity gaps. Address losses that could threaten housing, food, health or income before minor losses.
Country-level data should replace global context when making a personal decision. Benefit rules, public-service availability, catastrophe exposure and insurance terms can change the answer even for households with similar incomes.
The bottom line on risk
No single group is universally “most at risk.” The strongest cross-domain pattern is that people with low incomes and little capacity to absorb a shock face the greatest consequences. In health, the poorest experience dramatically higher financial hardship; in social protection, hundreds of millions of people lack specific maternity, employment-injury or unemployment benefits; and in insurance, households in exposed locations or with inadequate policy terms can remain vulnerable despite nominal coverage. Identify the domain, separate access from affordability and adequacy, and then measure the specific shortfall rather than relying on a single global number.
