Human-in-the-loop (HITL) AI is a way of operating an AI agent in which it pauses before a consequential action so a person can approve, reject, edit, or stop it. An agent should ask for approval when its next step could materially change records or permissions, affect another person, move money, expose sensitive information, create a legal or compliance commitment, or be difficult to undo. The right boundary depends on impact, reversibility, uncertainty, and whether mistakes are easy to detect—not on a blanket rule that every step needs approval or that the agent can approve itself.

What is human-in-the-loop AI?

Human-in-the-loop AI places a person at a decision point in an AI-supported process. The agent may gather information, reason about a task, and prepare a proposed action, but it pauses before carrying out an action that meets a defined approval threshold. The reviewer can approve the action as presented, reject it, edit it, or stop the run.

For example, an agent might draft a customer message and prepare a payment instruction. Drafting is not the same as sending or paying: the system can let the agent prepare the work while requiring a person to review the actual outgoing message or transaction. OpenAI’s agent documentation describes approvals as pauses before sensitive tool calls and side effects, including cancellations, edits, shell commands, and sensitive MCP actions. Its explanation distinguishes the agent’s decision that an action is needed from permission to execute it: the run pauses until approval or rejection. OpenAI agent documentation

HITL is not a guarantee that an action is safe. A person can be rushed, lack relevant context, or approve a misleading proposal. Approval is one control in a system that should also constrain what tools the agent can use, record what happened, monitor its behavior, and provide a way to stop it.

How does HITL differ from other oversight?

These labels describe where human oversight sits in relation to an action:

  • Human-in-the-loop: a person approves an action before it happens.
  • Human-on-the-loop: the system can act while a person monitors it and can intervene.
  • Human-out-of-the-loop: the system acts without human review of the action.

The UK National Cyber Security Centre uses “human-in-the-loop” for approval before actions occur. These arrangements are not interchangeable: monitoring after an action begins is not the same safeguard as requiring approval beforehand. UK NCSC guidance

When should an AI agent ask for approval?

Require approval when the proposed action has meaningful consequences or weak recovery options. A practical rule is to pause before an action that sends, publishes, approves, spends, changes permissions, updates important records, accesses sensitive data, or cannot readily be reversed—unless the expected impact is demonstrably low and undoing the action is trivial.

The Government of Canada’s guidance on agentic AI recommends review for actions such as sending, publishing, approving, spending, or updating records unless both low impact and easy reversibility are established. That is useful for financial workflows, where a draft payment, a payment approval, and the transfer itself are different steps with different consequences. Government of Canada agentic-AI guidance

Use these questions to assess an action

  • What changes? Does the action alter an account, customer record, payment, permission, public statement, or other system state?
  • Who could be affected? Could it influence a customer, employee, account holder, or other person?
  • Can it be reversed easily? A reversible draft is different from a sent message, completed transaction, or deleted record.
  • What information or access is involved? Sensitive data and broad permissions increase the need for a checkpoint.
  • How certain is the agent’s interpretation? Ambiguous instructions, incomplete evidence, or conflicting inputs favor escalation.
  • Would an error be easy to notice before harm occurs? If mistakes are hard to detect, a pre-action review is more important.
  • Is there a workable stop or recovery path? A fast, reliable external stop can reduce risk, but it does not replace approval for high-impact actions.

These factors work together. A low-value change may still deserve review if it exposes private data; a consequential action may be better bounded by a mandatory approval even when the agent appears confident. Approval thresholds should be set by system designers or operators, not improvised by the agent. The Canadian Centre for Cyber Security specifically recommends that designers or operators determine when approval is required. Canadian Centre for Cyber Security guidance

How much autonomy should an agent have?

Autonomy should be proportional to the consequences of error and the quality of detection and recovery. Low-stakes, repeatable work that is easy to check can often proceed with monitoring and escalation. High-stakes choices call for mandatory review. Microsoft’s task framework examines repeatability, impact, error detectability, and time sensitivity; the Australian National AI Centre similarly describes proportional oversight, from automated monitoring in low-stakes applications to required human review for high-stakes decisions. Microsoft task framework Australian National AI Centre guidance

For personal-finance operations, an agent could be allowed to organize transaction records or draft an explanation for review, while a separate checkpoint protects actions that would transfer funds, change account access, or send a commitment to another party. The relevant distinction is not whether a task involves AI; it is whether the agent is preparing information or changing something consequential.

A five-level risk ladder

  1. Assist: Allow reading, summarizing, classifying, or drafting, with ordinary human review of the resulting work.
  2. Suggest: Let the agent prepare a plan or tool call, but require a person to approve before execution.
  3. Act with a checkpoint: Permit bounded work, then pause before sending, publishing, purchasing, changing permissions, accessing sensitive data, or taking an irreversible step.
  4. Monitored autonomy: Allow low-impact, reversible actions with alerts, logs, rate limits, and a tested stop control.
  5. Prohibited autonomy: Do not let the agent bypass policy, erase audit records, exfiltrate data, or expand its own permissions.

This ladder is a practical way to match controls to risk, not a universal legal standard or a claim that one level fits every system. The action scope, applicable policy, domain, and ability to recover all matter.

Balance risk against review capacity

More approval prompts do not automatically produce safer decisions. If reviewers face a long queue, receive too little context, or cannot distinguish routine actions from exceptional ones, they may approve without meaningful scrutiny or delay work that could safely proceed. Use stronger checkpoints for high-impact or uncertain actions and allow monitored execution only where the impact is low, mistakes are detectable, and recovery is straightforward.

Time pressure also matters, but it is not a reason to let the agent set its own threshold. A system owner can define an approved, bounded path for routine cases and an escalation path when the agent encounters ambiguity or an action outside that scope. The policy should specify what the agent may do while waiting and what happens if no reviewer is available.

What should an approval screen show?

A reviewer needs enough information to understand exactly what will happen, why it was escalated, and what approving it permits. A generic “Allow?” prompt encourages rubber-stamping; a useful prompt makes the action and its consequences legible.

Include the decision-critical context

  • Proposed action: State the requested operation in plain language.
  • Target: Identify the recipient, account, record, destination, or system that will be affected.
  • Tool and arguments: Show the tool being called and the material parameters it will use.
  • Expected effect: Explain what will change if the reviewer approves.
  • Evidence: Present the relevant information behind the proposal, including uncertainty or conflicting details that matter to the decision.
  • Reason for escalation: Identify the risk or policy trigger, such as a payment, sensitive-data access, or an irreversible update.
  • Controls: Offer clear approve, reject, edit, and stop options.

For example, an approval request for an outgoing payment should make the action, destination, relevant details, and policy reason visible before execution. The agent’s confidence alone is not a substitute for those details. Reviewers should be able to reject or edit a proposal without having to reconstruct its intent from a long conversation.

Keep authority outside the model

Approval logic belongs in the orchestrator or policy layer, not in the model’s informal judgment about whether it feels safe to proceed. High-risk triggers should be deterministic, and the system should explain why a run paused. Use read-only or draft permissions by default where possible, limit tool access to the least privilege needed, and define explicit action schemas so tools cannot silently receive broader authority than intended.

The Canadian Centre for Cyber Security’s advice is explicit that the approval threshold belongs to designers or operators, rather than the agent. The Government of Canada recommends “read-only by default” where possible. A separate pause or disable mechanism should exist outside the agent so an operator can stop it even if the agent misbehaves. Government of Canada guidance on read-only access and stop mechanisms

How should teams implement and maintain approvals?

Approval is part of an operating process, not just a button in an interface. Define the action boundary, implement it in enforceable policy, and test the system’s behavior when a request is rejected, interrupted, or only partly completed.

  1. Inventory the agent’s actions. List the tools it can call and what each can read, change, send, approve, or delete. Separate preparation from execution wherever possible.
  2. Classify impact and recovery. For each action, assess impact, sensitivity, reversibility, ambiguity, detectability of mistakes, and time pressure. Identify actions that must pause every time and narrow exceptions, if any.
  3. Set approval rules outside the model. Encode high-risk triggers in orchestration or policy logic. Tell the reviewer why a prompt appeared and prevent the agent from bypassing the rule through a different tool path.
  4. Constrain permissions. Start with read-only or draft access, grant only the tools and scope needed, and keep particularly sensitive operations behind a checkpoint.
  5. Build a useful review and recovery path. Show the action, target, parameters, evidence, and effect; provide approve, reject, edit, and stop controls; define what happens to any partial work when a proposal is rejected or execution is interrupted.
  6. Record the decision and result. Log the reviewer, decision, timestamp, action actually taken, and outcome. Retain relevant prompts, tool calls, approvals, and overrides so operators can investigate a failure or review the control’s operation.
  7. Test realistic failure cases. Exercise ambiguous instructions, worst-case inputs, prompt injection, rejection, partial completion, and stop or recovery behavior. Verify that an untrusted instruction cannot persuade the agent to evade a checkpoint.
  8. Reassess after changes. Review approval rules when the model, tools, data, permissions, scope, or policy changes. A previously low-risk workflow can become different when its capabilities or operating environment change.

Logging and monitoring support accountability, but they do not replace a pre-action checkpoint when one is required. Likewise, a stop control is valuable only if it is independent enough to work when the agent is behaving unexpectedly and has been tested under realistic conditions.

What can go wrong with human approval?

A review step can fail if it is designed as a click-through rather than a decision. The main safeguards are to make the action boundary explicit, provide the information needed to evaluate the proposal, keep approval requirements predictable, and preserve technical limits even after a person approves.

  • Rubber-stamp approvals: Reviewers may approve quickly if prompts are frequent, vague, or stripped of context. Escalate proportionally and show the reason for each pause.
  • Overbroad access: A reviewer’s approval does not justify giving the agent more access than the task needs. Use least privilege and separate read or draft work from sensitive execution.
  • Untrusted content influencing tool use: Treat instructions found in external or user-provided content as potentially untrusted. Test whether such content can trigger actions or evade policy checkpoints.
  • Unclear accountability: Keep a record of who decided, what was approved, what action occurred, and what outcome followed.
  • Unsafe partial completion: A rejected action may follow earlier changes. Define how the system identifies, halts, or recovers from partial work rather than assuming rejection rolls everything back.
  • Stale policy: Changes to tools, data, permissions, or scope can change the risk profile. Revisit the threshold rather than relying indefinitely on an old approval design.

No single approval threshold works for every agent or domain, and the guidance described here does not establish a universal legal requirement. Set the threshold from the system’s actual impact, applicable policy, and recovery options, then preserve monitoring and technical controls around it.

Frequently Asked Questions

Can an AI agent approve its own actions?

No. The agent may identify that a checkpoint is needed and prepare a proposal, but the approval rule should be determined and enforced by system owners or operators. Giving the agent authority to decide when its own high-risk actions need review defeats that boundary.

Does human approval make an AI agent safe?

No. A reviewer may lack context or approve a misleading proposal, so approval must sit alongside limited permissions, monitoring, logs, and an independent stop mechanism. The quality of the review depends on showing the actual action and relevant evidence.

Should an agent ask for approval before every tool call?

Not necessarily. Requiring a person to approve every low-impact, reversible step can create a bottleneck and make prompts less meaningful; reserve mandatory review for actions that meet the system’s risk criteria and monitor bounded low-risk work.

What should happen if no reviewer is available?

The agent should not silently proceed with an action that requires approval. Define a safe waiting, cancellation, or escalation path in advance, including how any already completed work is handled.
