The exact packet changes depending on whether you are replacing a Social Security card, changing a name, applying for retirement or Medicare, filing for disability, or handling survivor benefits.
Use the notice as the final authority for your case. If a document is missing, do not automatically cancel or delay the appointment; the Social Security Administration (SSA) generally allows missing evidence to be supplied later.

What to bring to any Social Security appointment
Start with a basic packet, then add records for the specific service you are requesting:
- Appointment notice: Bring the letter, email, or other confirmation showing the date, time, location, and any case-specific instructions.
- Current identity document: A U.S. driver’s license, state-issued non-driver identification card, or U.S. passport is usually the strongest choice.
- Original or issuing-agency-certified evidence: SSA generally requires original documents or copies certified by the agency that issued them for core identity, citizenship, age, immigration, and name-change evidence.
- Documents tied to the claim: Examples include a birth certificate, immigration document, military record, W-2, medical record, or workers’ compensation settlement.
- Any forms or worksheets named in the notice: A disability appointment may require the Medical and Job Worksheet, for example.
- A way to organize originals separately from copies: A simple folder can prevent you from accidentally leaving an original behind. This is a practical precaution, not an SSA requirement.
Bring documents in the name and status that apply to your case. If your current name differs from the name in SSA’s records, include the legal name-change evidence and, when necessary, identity evidence in the former name.

Match the packet to the reason for your visit
| Appointment purpose | Documents to prepare | Format and qualifications |
|---|---|---|
| Replace or correct a Social Security card | Current identity document; name-change evidence if applicable; citizenship or lawful-status evidence if SSA has not already established it | Originals or copies certified by the issuing agency are required for core evidence. Identity documents must be current and show your name plus identifying information such as date of birth or age. |
| Retirement or Medicare | Social Security number record or card; proof of age; citizenship or lawful-alien evidence when applicable; military service papers if you served before 1968; last year’s W-2 forms or self-employment tax return | SSA generally requires originals or certified copies for core proofs. Photocopies are accepted for W-2 forms and self-employment tax returns. |
| Disability claim or interview | Medical records in your possession; workers’ compensation information and settlement papers when applicable; spouse and minor children’s names and dates of birth; marriage and divorce dates; bank routing and account numbers for direct deposit; a contact person; Medical and Job Worksheet if you could not complete the online process | Bring what you have even if the packet is incomplete. The interview may be in person or by telephone and takes at least one hour. |
| Survivor benefits or another service | Appointment notice plus the identity and relationship records specifically requested for the claim | Requirements vary by the benefit and by what SSA already has on file. Follow the notice and confirm unusual cases with SSA. |
Identity documents for a Social Security card appointment
For card replacement, correction, or a first card, SSA says an acceptable identity document must be current, show your name and identifying information such as your date of birth or age, and preferably include a recent photograph.
Primary identity documents
- U.S. driver’s license
- State-issued non-driver identification card
- U.S. passport
These documents are usually the most straightforward because they combine a photograph with identifying information. Bring the physical original or an issuing-agency-certified copy when certification is available; an ordinary photocopy is not a substitute for required original evidence.
Alternative identity documents
If you do not have one of the primary documents, or cannot replace it within 10 days, SSA may consider a current employee identification card, school identification card, health-insurance card, or U.S. military identification card. A Medicare card is not treated as the health-insurance-card alternative for this purpose.
SSA’s standard is case-specific. A document can be genuine yet still fail to establish identity if it is expired, lacks identifying information, or does not reasonably connect to the person in the record. Take the strongest current document available and bring any additional evidence named in your notice.

Name-change appointments: bring both the legal proof and identity link
When you need a new card showing a different legal name, bring the document that created the change. SSA lists these examples:
- Marriage document
- Divorce decree
- Certificate of naturalization showing the new name
- Court order
If the name-change document does not adequately identify you in SSA’s records, also bring an identity document in your prior name. SSA says an expired identity document in the former name may be accepted in that situation, even though current identification is normally preferred.
Bring the complete document, not just a page that shows the new name. The issuing agency’s certification matters when SSA requires a certified copy.

Citizenship and lawful-status evidence
U.S. citizenship documents
If citizenship has not already been established with SSA, acceptable evidence can include:
Rank #3
- U.S. passport
- Certificate of Naturalization
- Certificate of Citizenship
- Certificate of Report of Birth
- Consular Report of Birth Abroad
SSA may not need these documents again if it already received and accepted the same evidence for an earlier claim. Check the appointment notice rather than assuming a previously submitted document must be resubmitted.
Documents for noncitizens
Noncitizens may need a current Permanent Resident Card (Form I-551), Employment Authorization Document (Form I-766), or Form I-94 together with an unexpired foreign passport or admission stamp. Immigration documents are valuable originals. Do not mail an irreplaceable document when SSA’s instructions say to bring it for examination at an office or appointment.
Retirement and Medicare appointments
For a retirement or Medicare claim, assemble the records SSA uses to verify your identity, age, work history, and eligibility:
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems- Your Social Security card or another record of your Social Security number
- Your original birth certificate or an issuing-agency-certified proof of age
- Citizenship or lawful-alien evidence when applicable
- Military service papers if you served before 1968
- Last year’s W-2 forms
- Your self-employment tax return if you were self-employed
SSA generally requires originals or certified copies for the birth, citizenship, immigration, and military evidence. It accepts photocopies for W-2 forms and self-employment tax returns. Keep tax paperwork complete enough to show the relevant year and your identifying information.
Rank #4
If SSA already received proof of age or citizenship for an earlier claim, the agency may not require it again. Bring the notice and ask if you are uncertain; carrying an unnecessary copy is preferable to leaving a required original at home, but do not mail irreplaceable originals without specific instructions.
Disability appointments: prepare information, not just identification
A disability interview covers medical history, work, family, payment, and contact information. SSA says the interview can take place at a local office or by telephone and will take at least one hour.
Medical and work information
- Medical records already in your possession, including provider names, treatment dates, test results, and medication information you can access
- Workers’ compensation information and settlement papers, if you received benefits or reached a settlement
- The Medical and Job Worksheet if you could not complete the online process
- A contact person who can help SSA reach you or clarify information, when appropriate
Family and payment information
- Names and dates of birth of your spouse and minor children
- Marriage dates and divorce dates
- Bank routing and account numbers if you want to arrange direct deposit
Starting the disability application online can cut the interview time in half, according to SSA’s starter-kit guidance. Bring the confirmation or any information you saved, but keep the appointment even if some answers or records are incomplete.
What to do when a document is missing
Do not postpone filing solely because you cannot obtain every record before the appointment. SSA states that you can submit the application and the documents you do have, then provide missing evidence later.
Best Value
- It can be a gift option
- Comes with secure packaging
- Helpful in various ways
- Attend the scheduled appointment and explain exactly which document is missing.
- Submit every available record so SSA can begin verifying the claim.
- Ask the local office what deadline or delivery method applies to the missing item.
- Request help identifying or verifying information with a state vital-records agency when a birth or other civil record is unavailable.
- For a disability claim, keep the appointment even when medical, work, or family information is incomplete.
Missing evidence can delay a decision, but canceling the appointment removes an opportunity to start the process and learn precisely what SSA still needs.
Originals, certified copies, and ordinary photocopies
Use the evidence format SSA specifies for the service:
- Original: The document issued by the government, court, school, employer, or other authorized organization.
- Issuing-agency-certified copy: A copy formally certified by the agency that issued the original. A scan or a photocopy you made yourself is not the same thing.
- Photocopy: Permitted for some items, including W-2 forms and self-employment tax returns in retirement or Medicare processing.
Separate originals from copies and ask the representative whether an original will be returned during the visit. Keep irreplaceable immigration, civil, and court documents with you unless SSA gives a specific reason and secure submission method for sending them.

SSA appointments may be by telephone or in person. You can schedule or change one online or by calling 1-800-772-1213; the TTY number is 1-800-325-0778. The appointment service lists availability in most U.S. time zones from Monday through Friday, 8 a.m. to 7 p.m.
When you call or use the online service, have your notice and Social Security number information available if requested. The notice remains the best source for an office address, interview type, arrival instructions, and any unusual evidence requirement.
Final packing checklist
- Read the appointment notice and highlight every requested item.
- Put a current photo ID in the front of your folder.
- Add originals or issuing-agency-certified copies for identity, age, citizenship, immigration, and name changes.
- Add service-specific records: tax and military documents for retirement or Medicare; medical, workers’ compensation, family, banking, and worksheet information for disability.
- Include photocopies only where SSA permits them, such as W-2 forms and self-employment tax returns.
- Write down the names and dates of birth of family members and the dates of marriages or divorces if the appointment concerns disability or another benefit claim.
- Keep a list of anything still missing and bring proof that you requested it, if available.
- Attend the appointment even if the packet is incomplete, then follow SSA’s instructions for supplying the remaining evidence.
Social Security card replacement limits
SSA’s 2026 replacement-card guidance states limits of three replacement cards in one year and 10 during a lifetime, subject to exceptions. Those limits apply to replacement-card requests, not to every type of Social Security office service. If your request involves a legal name change, correction, or another exception, explain the reason and follow the office’s instructions.
