Your first step is to ask human resources or the plan administrator for the exact date your employer coverage ends and request that date in writing. If coverage ends, you may be able to continue the same plan through COBRA, enroll in another employer plan, buy a Marketplace plan, or apply for Medicaid or CHIP.
These choices have different deadlines, premiums, networks and effective dates. Compare them before an enrollment window closes rather than assuming COBRA is automatically the best or cheapest answer.

Find your actual coverage end date first
There is no single end date that applies to every employer health plan. Some plans end coverage on the last day worked, while plan documents may use another date. Ask HR or the plan administrator to confirm:

Compare the four main coverage routes
| Route | What it does | Deadline to check | Main cost and coverage issues |
|---|---|---|---|
| COBRA continuation | Temporarily continues the employer group plan for eligible workers and dependents. | Generally elect within 60 days measured from the later of the coverage-loss date or receipt of the election notice. | Usually the full group premium, plus an administrative charge of up to 2%; you retain the existing plan’s benefits and provider network. |
| Another employer plan | Allows special enrollment in a spouse’s or another eligible family member’s group plan when you lose eligibility. | Generally request special enrollment within 30 days of losing eligibility. | Compare the employee contribution, effective date, doctors, hospitals, prescriptions, deductible and out-of-pocket maximum. |
| Marketplace plan | Provides individual coverage, with financial assistance potentially available based on eligibility. | Generally select a plan within the 60-day special-enrollment period after losing job-based coverage. | Compare the net premium after any assistance, deductible, network, prescriptions and effective date. |
| Medicaid or CHIP | Public coverage for eligible adults and children; CHIP covers qualifying children. | Applications may be made at any time. | Eligibility and benefits depend on household circumstances and state program rules. |
This comparison reflects federal Department of Labor guidance. Your plan documents, state procedures and enrollment portal control the dates and eligibility in your case.
How COBRA continuation works
Who may qualify
COBRA is a federal continuation law for certain employer group health plans. It generally applies to plans sponsored by employers that had at least 20 employees in the prior year, subject to statutory and plan requirements. Job loss or a reduction in working hours can be a qualifying event. A worker fired for gross misconduct may not qualify under the Department of Labor’s job-loss guidance.
Eligible dependents can have continuation rights of their own. Review each family member separately instead of assuming that one person’s election automatically covers everyone who was on the plan.
Election window and duration
The usual COBRA election period is 60 days, counted from the later of the date employer coverage ends or the date you receive the election notice. The ordinary continuation period after job loss or reduced hours is up to 18 months. The Department of Labor describes conditional extensions that can produce periods of up to 29 or 36 months in particular qualifying-event or disability circumstances; those extensions are not automatic and do not apply to every former employee.
What COBRA costs
When you were employed, the company generally paid part of the group premium. Under COBRA, you generally pay both the employee and employer portions, and the plan may add an administrative charge of up to 2%. An employer may subsidize some of that amount, but it is not required to do so. Ask for the exact monthly premium, the first payment amount and the dates each payment covers.
Compare the total annual exposure, not just the monthly bill. Include the premium, deductible, copayments, coinsurance and out-of-pocket maximum. A higher COBRA premium can still be worthwhile if changing plans would mean paying new deductibles or losing coverage for an expensive medication.

Retroactive coverage and payment
COBRA is generally retroactive to the date employer coverage ended once you elect it and pay the required premiums. That feature can protect you from a gap, but it also means you may owe premiums for the entire intervening period even if you did not use medical services. Confirm the election notice’s payment deadline, amount due and the plan’s status before relying on retroactivity.

COBRA is temporary, not permanent insurance. It can end early in circumstances specified by law or the plan. Read the notice for termination conditions and keep proof of every payment. If you voluntarily end COBRA early, that decision generally does not by itself create a new Marketplace special-enrollment period outside open enrollment. A separate qualifying event may be required, so verify your situation before cancelling.
Joining another employer plan
Loss of job-based coverage can trigger special enrollment in a spouse’s or another eligible family member’s employer plan. The typical request period is 30 days from the loss of eligibility. Contact that plan’s benefits administrator immediately; do not wait for your COBRA notice if the other plan’s deadline could arrive first.
Ask for the plan’s effective date and compare:
- The employee and dependent premium contributions.
- Whether your doctors, hospitals and pharmacies are in network.
- Deductibles, copayments, coinsurance and the out-of-pocket maximum.
- Prescription formularies and any authorization requirements.
- Whether every household member can enroll or whether people need different coverage routes.
A lower payroll deduction does not necessarily mean lower total spending. Include expected medical use and any deductible you would have to satisfy again.
Using a Marketplace special-enrollment period
Loss of job-based coverage generally opens a 60-day Marketplace special-enrollment period. Marketplace plans are individual policies rather than a continuation of your former employer plan. Financial assistance, including premium tax credits or cost-sharing reductions, may be available depending on eligibility, household income, location and the plan selected.

When comparing plans, enter your expected household income accurately and check the effective date shown during enrollment. Verify each doctor, hospital and prescription before selecting a plan; a familiar insurer name does not guarantee that your current providers or medications are covered. Compare the net premium after any eligible assistance with the deductible, copayments, coinsurance and out-of-pocket maximum.
Do not cancel COBRA merely to manufacture a Marketplace enrollment opportunity. Voluntary early termination generally does not create a new special-enrollment period by itself. If you need to switch, confirm that another qualifying event or an open-enrollment period permits the change.
Medicaid and CHIP may be available at any time
Medicaid and the Children’s Health Insurance Program (CHIP) are alternatives for people who meet program requirements. The Department of Labor’s FAQ states that eligible applicants can apply at any time, rather than waiting for a limited special-enrollment window. Eligibility, covered services and enrollment procedures vary with household circumstances and state rules.
Apply through the appropriate state Medicaid or CHIP agency and ask when coverage would begin. A child may qualify for CHIP even when an adult in the household uses COBRA, an employer plan or Marketplace coverage, so evaluate each person independently.
How to choose without missing a deadline
- Get the termination date in writing. Ask HR or the plan administrator when employee and dependent coverage ends.
- Collect the notices. Save the COBRA election notice, plan summary, premium quote and any notice from a spouse’s employer plan or the Marketplace.
- Build a household list. Record who needs coverage, current doctors, regular prescriptions and upcoming treatment.
- Mark every applicable deadline. Use the 60-day COBRA and Marketplace windows and the 30-day employer-plan window shown above; Medicaid and CHIP applications can be submitted at any time.
- Price total exposure. Add premiums to expected deductibles, copayments, coinsurance and the out-of-pocket maximum for each option.
- Check continuity. Confirm provider networks, hospitals, prescriptions, prior authorizations and the effective date.
- Submit the election or application. Use the plan’s stated method and keep confirmation, receipts and copies of forms.
- Resolve gaps immediately. If a bill is denied while an election is pending, tell the provider and plan administrator that continuation or new coverage is being processed and ask what documentation is required.
Common mistakes and how to avoid them
Assuming insurance ends on your last day
The plan may use a different termination date. Acting on an assumption can make you miss an enrollment window or pay for coverage you did not need. Obtain the date from the administrator.

Choosing by premium alone
COBRA’s full premium can look expensive beside a Marketplace quote, while a low-premium Marketplace plan can carry a high deductible or exclude a needed provider. Compare the complete annual cost and benefits.
Ignoring dependents’ separate rights
A spouse or child may have a different continuation or enrollment opportunity. List each person’s eligibility, deadline and preferred coverage before making one household decision.
Letting notices go to an old address
Update your mailing and electronic contact information with the former employer and plan administrator. The election clock is tied to the coverage-loss date and notice rules, so missing mail can create avoidable confusion.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteEnding COBRA without checking future enrollment rights
Voluntary early termination generally does not open a new Marketplace special-enrollment period. Confirm another qualifying event or open-enrollment availability before ending continuation coverage.
Failing to keep payment evidence
Save confirmation numbers, cancelled checks, electronic receipts and correspondence. They can establish that you elected and paid on time if claims processing is delayed.

Example decision paths
You need the same doctors immediately
Start with COBRA because it continues the existing group plan and may be retroactive to the coverage-end date. Price the full premium and compare it with any Marketplace plan that includes the same providers. If the cost is manageable, continuity may outweigh a lower headline premium elsewhere.
Your household income falls sharply
Check Marketplace financial assistance and Medicaid or CHIP eligibility rather than assuming the former employer plan is affordable. Apply within the relevant Marketplace window and ask the state program about start dates.
Your spouse has employer coverage
Request special enrollment in that plan within its stated period, typically 30 days after losing eligibility. Compare the spouse plan’s contribution and network with COBRA before electing.
What the federal guidance does—and does not—decide
The Department of Labor’s COBRA materials describe temporary continuation, election timing, premium responsibility and conditional extensions. They do not set one universal employer coverage-end date or determine your household’s Marketplace, Medicaid or CHIP eligibility. Plan documents, the COBRA notice, the Marketplace and your state agency provide the case-specific answer.
The Department of Labor’s COBRA page summarizes the purpose plainly: “COBRA provides temporary coverage to you and your dependents–usually up to 18 months.” Guidance retrieved October 13, 2026, also cautions that rules and procedures can vary by plan, state and individual circumstances. Use the official notices and administrator contacts for final dates and amounts.
