Leaving a job before 65 usually ends your employer health plan, but it does not leave you without choices. In the United States, you can generally use a Marketplace Special Enrollment Period, elect COBRA, apply for Medicaid or CHIP, or join an eligible spouse’s or family member’s employer plan. Losing job-based coverage generally gives you 60 days to enroll in a Marketplace plan, while another employer plan may give you only 30 days. Compare the effective date, total costs, doctors, prescriptions and eligibility before choosing.
What changes when your job-based coverage ends
Your employment status and your insurance status are related but not identical. Your employer plan normally ends on the date set by the plan documents, which may be your last day of work or the end of that month. Ask human resources or the plan administrator for the exact termination date and a written notice of your rights. The date coverage ends controls several enrollment deadlines.

Quitting and being fired generally create the same Marketplace enrollment opportunity: HealthCare.gov says loss of job-based coverage opens a 60-day Special Enrollment Period even when the loss follows a voluntary resignation. An application can also screen you for premium tax credits, cost-sharing reductions, Medicaid and CHIP. Coverage can start the first day of the month after your employer coverage ends, depending on when you select a plan and complete enrollment.

Your four main coverage routes
1. Marketplace coverage
A Marketplace plan is individual insurance purchased through the federal or a state Marketplace. After losing job-based coverage, you generally have 60 days to select a plan. The application asks about household size, income and current coverage and then displays plans available in your location. It also determines whether you may qualify for premium tax credits or reduced cost sharing.
Coverage options, networks and premiums vary by county and household circumstances. Check the plan’s effective date before assuming a prescription or doctor visit will be covered. If your employer plan ends on June 30, for example, a Marketplace plan may begin July 1 when you enroll on time; the Marketplace will show the date that applies to your application.
2. COBRA continuation
COBRA can let an eligible former employee and covered dependents continue the same employer group health benefits temporarily. The federal Department of Labor generally gives you 60 days to elect COBRA, measured from the later of the date coverage ends or the date the election notice arrives. Once elected, coverage is usually retroactive to the date your prior coverage ended, provided you pay the required premiums.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesThe usual federal continuation period is up to 18 months, with possible extensions in certain circumstances. You generally pay the entire premium rather than the employee share that your employer previously paid. A plan may charge the group rate plus a 2 percent administrative fee, so the monthly bill can be substantially higher than your payroll deduction.
COBRA is often valuable when you need the same doctors, hospitals, prescription formulary or deductible. It can also protect you when you have already accumulated significant spending toward the plan’s annual out-of-pocket maximum. Compare the full COBRA premium with the expected total cost of a replacement plan instead of comparing premiums alone.

3. Medicaid or CHIP
Medicaid and the Children’s Health Insurance Program are public programs whose eligibility depends on income, household facts, age, disability, pregnancy and state rules. A drop in income after leaving work can make screening worthwhile even if you did not qualify while employed. You can apply for these programs at any time; they are not limited to the Marketplace’s 60-day window.
The Marketplace application can screen for Medicaid or CHIP and route your information to the appropriate agency. Because states set many eligibility rules and income thresholds, no general article can determine whether you qualify. Follow the state agency’s instructions and wait for the eligibility decision and effective date before ending other coverage.
4. A spouse’s or family member’s employer plan
If a spouse or another eligible family member has employer coverage, loss of your job-based plan may trigger a special enrollment right. The Department of Labor says the request generally must be made within 30 days after you lose eligibility for the original coverage. This window is shorter than the Marketplace period, so contact the plan administrator immediately and ask which documents are required.
The other employer plan may have a different deductible, network, prescription list and payroll contribution. Confirm when coverage will begin and whether dependents must enroll at the same time. A plan administrator, rather than your former employer, can provide the controlling deadline and effective-date rules.
Deadlines and dates to put on your calendar
| Action | General federal timing | What to verify |
|---|---|---|
| Marketplace enrollment after loss of job-based coverage | Generally 60 days after the qualifying loss | Your coverage termination date, plan selection deadline and effective date shown by the Marketplace |
| COBRA election | Generally 60 days from the later of coverage loss or receipt of the election notice | Election notice date, premium amount, payment deadline and continuation period |
| Special enrollment in another employer plan | Generally 30 days after losing eligibility for prior coverage | The plan administrator’s deadline, required proof and start date |
| Medicaid or CHIP application | Eligible people may apply at any time | State eligibility decision and effective date |
| COBRA continuation | Usually up to 18 months, with extensions in some circumstances | Whether an extension applies to your specific situation |
Keep the termination letter, election notice, proof of loss of coverage and every confirmation number. If a deadline falls on a weekend or holiday, ask the administering agency how it handles submissions rather than assuming the date moves.

How to compare the real cost of each option
The premium is only one part of your exposure. Build a side-by-side comparison using the same expected medical use for every option.
- Start date: Identify the first day the plan pays claims and whether a gap exists between your job plan and the replacement.
- Monthly premium: For COBRA, use the full amount including any permitted administrative fee, not your former payroll deduction. For Marketplace coverage, use the premium after any tax credit you are actually eligible to receive.
- Deductible and cost sharing: Compare deductibles, copayments, coinsurance and the annual out-of-pocket maximum.
- Network: Search the plan directory for your doctors, hospitals, therapists and pharmacies. Confirm directly with providers when a service is important.
- Prescription formulary: Check each medication’s tier, prior-authorization rules, quantity limits and preferred pharmacy requirements.
- Duration: COBRA is temporary. A Marketplace plan or public program may provide a longer-term route, but eligibility and renewal rules apply.
- Eligibility risk: Marketplace subsidies depend on household and income information; Medicaid and CHIP depend on state rules; an employer plan depends on the family member’s eligibility and timely request.
A plan with the lowest premium can cost more overall if you expect surgery, regular specialist visits, expensive drugs or other substantial care. Conversely, paying for COBRA’s familiar plan may not be economical if you rarely use care and qualify for significant Marketplace savings.
Choosing based on your situation
When continuity matters most
COBRA is the most direct way to keep the same group benefits. It can avoid changing doctors and may preserve progress toward the plan’s deductible or out-of-pocket maximum. Its trade-off is the full premium and a limited continuation period. Ask the administrator whether your existing deductible and spending credits carry over exactly as claims continue under the same plan.
When your income drops sharply
Leaving work may reduce household income enough to qualify for Marketplace financial assistance, Medicaid or CHIP. Complete the Marketplace application with a realistic estimate of annual household income and report changes when required. Do not assume that losing a job automatically makes you eligible for Medicaid; state rules and household details control the result.
When you need only a short bridge
If you expect new employment or other coverage soon, compare a short period of COBRA with a Marketplace plan rather than selecting solely on the first month’s premium. Verify whether the replacement plan has waiting periods, a different deductible year or a network that excludes your current providers. A short bridge still needs a confirmed effective date.

When a spouse’s plan is available
Contact that plan’s administrator as soon as you know your employer coverage will end. The general 30-day special enrollment deadline is easy to miss. Ask whether the plan requires a certificate of prior coverage, a termination letter or a specific enrollment form.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Can you switch from COBRA to a Marketplace plan?
You may apply for Marketplace coverage during the Special Enrollment Period created by the original loss of job-based insurance. However, voluntarily ending COBRA early generally does not create a new Marketplace Special Enrollment Period by itself. Waiting until COBRA expires, or losing it through certain involuntary circumstances, may create a qualifying event, but the details matter.
Do not cancel COBRA until the replacement plan has approved your enrollment and supplied a definite effective date. Otherwise, you could lose coverage without a guaranteed replacement. People who qualify for Medicaid or CHIP may apply at any time, but confirm the final decision and start date before terminating other insurance.
What Medicare means before age 65
Leaving work before 65 does not, by itself, make you eligible for Medicare. Some people under 65 qualify because of a qualifying disability or End-Stage Renal Disease. Those programs have their own eligibility and enrollment rules.

If you become Medicare-eligible while using COBRA, pay close attention to enrollment timing. Medicare.gov warns that COBRA does not replace timely Medicare enrollment. In relevant work-coverage situations, the Part B Special Enrollment Period is generally eight months after employment or qualifying employer coverage ends, whichever happens first. Confirm your dates with Medicare or a State Health Insurance Assistance Program counselor; a late decision can affect premiums and claim payment.

Common mistakes and how to prevent them
Assuming quitting eliminates Marketplace eligibility
HealthCare.gov’s guidance generally includes both resignation and termination when job-based coverage is lost. Apply within the 60-day period and let the application determine your available savings and programs.
Using the old payroll deduction as the COBRA price
Your former employer may have paid part of the premium while you worked. COBRA commonly shifts that cost to you and may add up to 2 percent. Request the exact monthly amount before electing.
Missing the spouse-plan deadline
The general 30-day period for another employer plan is shorter than the Marketplace period. Send the request promptly and obtain written confirmation.
Ending COBRA before replacement coverage is active
Voluntary early cancellation usually does not open a new Marketplace enrollment period. Keep COBRA until the new plan’s effective date unless a qualified adviser confirms another form of coverage.
Ignoring state-specific rules
The federal pathways described here do not cover every state continuation law, employer-plan term or state Medicaid rule. Smaller employers may be subject to state continuation requirements that differ from federal COBRA. Ask the plan administrator and your state insurance or Medicaid agency about rules that apply where you live.
A practical checklist after leaving work
- Ask human resources for the exact date your health coverage ends and request written proof of loss.
- List every covered person, current doctor, hospital, prescription and expected treatment.
- Calendar the 60-day Marketplace deadline, the 60-day COBRA election deadline and any 30-day employer-plan deadline that applies.
- Apply through the Marketplace to compare plans and screen for premium assistance, Medicaid and CHIP.
- Request the COBRA election notice and calculate its full monthly premium, including any administrative fee.
- Contact a spouse’s or family member’s plan administrator immediately if that route is possible.
- Compare start dates, total annual costs, networks, formularies and coverage duration in writing.
- Enroll only after confirming the replacement plan’s effective date, then keep all notices and payment records.
- If disability, End-Stage Renal Disease or another Medicare pathway may apply, verify Medicare dates with Medicare or SHIP.
There is no single best answer for everyone under 65. The right route depends on the date your coverage ends, your household income, where you live, the care you expect and whether keeping your current network is worth COBRA’s full price.
