Those categories include doctor and outpatient care, emergency treatment, hospital care, maternity and newborn services, mental-health and substance-use treatment, prescription drugs, rehabilitation, laboratory services, preventive care, and pediatric services. The category list is a baseline—not a promise that every treatment, provider, or setting will be covered.
Your plan’s documents determine the details: which services are included, which doctors and facilities are in network, whether prior authorization is required, and how much you pay. A service can be covered and still leave you with a deductible, copayment, or coinsurance. Employer plans, Medicare, Medicaid, short-term policies, grandfathered plans, and state-regulated products can follow different rules, so use the Marketplace explanation below as a common U.S. starting point rather than a universal policy description.

Think of coverage as several questions rather than a simple yes-or-no label. Is the service in a covered category? Is it covered for your diagnosis, age, frequency, and place of service? Is the clinician, facility, laboratory, or pharmacy in the plan’s network? Does the plan require a referral, prior authorization, step therapy, or a particular supplier? Finally, what portion is subject to your deductible, copayment, or coinsurance?
Those details can change the bill dramatically. A medication may be on the formulary but placed on a high-cost tier. A hospital procedure may be covered while an out-of-network facility or separate clinician is not. A screening that is generally preventive may be billed as diagnostic when symptoms are evaluated at the same visit. Checking the exact service and billing circumstances before care is the most reliable way to avoid surprises.

The 10 essential health benefit categories
Marketplace plans must cover services in these 10 categories. States can impose additional requirements, and each plan defines the covered services, limits, networks, and cost sharing inside the category.
| Category | What it usually means | Examples and limits to check |
|---|---|---|
| Ambulatory patient services | Outpatient care that does not require hospital admission | Office visits, outpatient procedures, and same-day care; check visit copays, specialist rules, and network status. |
| Emergency services | Evaluation and treatment for an emergency | Review the plan’s emergency and urgent-care cost sharing and its rules for follow-up care. |
| Hospitalization | Inpatient care, surgery, and overnight stays | Hospital network, deductible, coinsurance, admissions, and prior authorization can determine the bill. |
| Pregnancy, maternity, and newborn care | Care before, during, and after birth, including newborn care | Confirm obstetric providers, hospitals, prenatal services, delivery costs, and newborn enrollment procedures. |
| Mental health and substance-use services | Behavioral-health treatment such as counseling and psychotherapy | Check covered clinicians, visit limits if any, authorization, and cost sharing for outpatient and inpatient treatment. |
| Prescription drugs | Medications included on the plan’s formulary | Drug tiers, copays or coinsurance, quantity limits, step therapy, and prior authorization may apply. |
| Rehabilitative and habilitative services and devices | Services and equipment that help you recover or gain skills affected by injury, disability, or a chronic condition | Plans may distinguish rehabilitation from habilitation and set visit, device, or authorization rules. |
| Laboratory services | Diagnostic testing and laboratory work | The laboratory’s network status and whether a test is ordered for diagnosis or screening can affect your cost. |
| Preventive and wellness services and chronic disease management | Specified preventive care, screenings, counseling, vaccines, and ongoing management of chronic conditions | Cost sharing and eligibility depend on the service, circumstances, and provider network. |
| Pediatric services | Care for children, including oral and vision care | Pediatric dental and vision are part of the essential-benefit category; adult dental and vision are not essential health benefits. |
HealthCare.gov also identifies birth-control coverage and breastfeeding coverage among benefits plans must include within the Marketplace rules described there. The exact covered service, frequency, provider, and cost-sharing terms remain plan- and state-dependent.
Why a covered category is not a guarantee
An essential-benefit category is a legal coverage area, not an unlimited menu. A plan may cover outpatient care while excluding a particular procedure, limiting the number of therapy visits, requiring a particular clinical indication, or directing you to an in-network provider. State benefit requirements can change what a plan includes. Plans may also add benefits beyond the essential categories.

State and plan terms matter
Two Marketplace plans in different states—or even two plans in the same state—can use different medical policies, formularies, networks, exclusions, and authorization rules. Read the Summary of Benefits and Coverage, Evidence of Coverage, drug formulary, provider directory, and any policy-specific clinical requirements before assuming a service is payable.
Other types of insurance follow different baselines
Large self-insured employers are not required to provide the Marketplace essential health benefits package, although many employer plans do cover similar services. Medicare, Medicaid, short-term products, and grandfathered plans have their own rules. A grandfathered plan may not include some Affordable Care Act rights and protections that apply to newer coverage.
What may not be covered or may cost extra
There is no single exclusions list that applies to every U.S. policy. The following are important areas where readers commonly assume more coverage than the evidence supports:

- Adult dental and vision: They are optional rather than essential health benefits. A Marketplace plan may offer them, but the 10-category requirement does not guarantee adult exams, glasses, contacts, dental work, or orthodontia.
- Services outside the plan’s definition: A treatment can fall within a broad category yet be excluded for a particular diagnosis, setting, age, frequency, or medical-necessity rule.
- Out-of-network care: Non-emergency care from an out-of-network provider may be excluded or subject to substantially higher cost sharing. A directory can change, so verify network status with the plan and provider before care.
- Abortion coverage: Marketplace plans can have different abortion coverage or restrictions. Check the plan’s benefit materials and applicable state rules.
- Grandfathered-plan exceptions: Older plans may not include every ACA protection, including some rules about pre-existing conditions or dollar limits.
- Employer-specific arrangements: A self-insured employer plan may not use the Marketplace essential-benefit structure and can define benefits differently.
For a definitive answer on a treatment, use the plan documents and insurer rather than a general category list. HealthCare.gov’s consumer guidance puts it plainly: “If you want to find out if a particular service is covered, call the plan.”

Coverage describes the plan’s obligation under its terms; it does not mean the service is free. Separate your monthly premium from what you pay when you receive care.
| Term | Meaning | Question to ask when comparing plans |
|---|---|---|
| Premium | The recurring amount you pay to keep the policy active, whether or not you use care | What will the annual premium be after any applicable subsidy? |
| Deductible | What you pay for certain covered services before the plan begins paying according to its terms | Does the deductible apply separately to prescriptions, medical care, or family members? |
| Copayment | A fixed dollar amount for a covered service, such as a visit or prescription | Are primary-care, specialist, urgent-care, and drug copays different? |
| Coinsurance | A percentage of the allowed cost that you pay after applicable deductible rules | What percentage applies to hospitalization, imaging, surgery, and specialty drugs? |
| Out-of-pocket maximum | The plan-year limit on your spending for covered services under the plan’s rules | Which expenses count, and which services or premiums are excluded from the limit? |
The out-of-pocket maximum does not turn an excluded service into a covered one. Premiums generally are separate from the maximum, and spending on services the plan does not cover does not receive the same protection. Your actual yearly cost depends on the care you use, the providers you choose, and the plan’s allowed amounts.
How to compare plans beyond the premium
Marketplace metal levels describe how the plan and member share costs; they do not mean one metal level covers a different set of essential-benefit categories. Compare the whole financial and access picture:
- Estimate the annual premium. Multiply the monthly premium by 12 and account for any subsidy shown in your Marketplace eligibility results.
- Check the deductible and cost-sharing structure. Look separately at primary-care visits, specialists, urgent care, emergency care, hospital services, imaging, laboratory work, and prescriptions.
- Review the out-of-pocket maximum. It is a key measure of financial exposure in a high-use year, but confirm what counts toward it.
- Verify your medications. Search the formulary for each drug and note the tier, quantity limits, prior authorization, step therapy, and specialty-pharmacy requirements.
- Verify providers and facilities. Confirm that your doctors, preferred hospital, behavioral-health clinicians, laboratories, and pharmacies are in network for the exact plan—not merely for the insurer generally.
- Model your likely year. A person who expects frequent visits, expensive drugs, pregnancy care, or planned surgery may prefer a different premium-deductible balance than someone who expects little care.
- Read the plan documents. Use the Summary of Benefits and Coverage for comparisons, then consult the Evidence of Coverage and policy-specific rules for details.
Looking only at the premium can produce a misleading bargain: a cheaper premium may be paired with a large deductible, higher coinsurance, or a narrow network. Conversely, a higher premium can be worthwhile if it reduces predictable costs for the care you expect.
Preventive care: when $0 is generally available
Most plans must cover specified preventive services without copayments or coinsurance, and Marketplace plans are included. The usual no-cost rule applies when the service is delivered by an in-network provider and the service meets the plan’s preventive-care criteria. The official guidance cautions that $0 is not guaranteed in every case.

Why a preventive visit can still generate a bill
- The provider or facility is out of network.
- A service is coded as diagnostic because you have symptoms or an existing condition rather than as screening.
- The visit includes additional treatment beyond the preventive service.
- The plan’s required preventive-service criteria, timing, age, or frequency are not met.
Before an appointment, ask the insurer how the service must be coded, whether the facility and clinician are in network, and whether any related tests are billed separately.
Pre-existing conditions and health status
Marketplace plans cover treatment for pre-existing conditions beginning when coverage starts. HealthCare.gov also says your health status does not determine your Marketplace premium. The important qualification is plan type: grandfathered plans may not include some ACA rights and protections, so confirm whether your policy is grandfathered and read its terms.
Annual and lifetime dollar limits
Within the scope described by HealthCare.gov, the health law prevents insurers from setting yearly or lifetime dollar limits on essential health benefits. Dollar limits can still apply to services that are not considered essential health benefits, and some grandfathered plans are exceptions. This rule does not eliminate exclusions, medical-necessity rules, network restrictions, deductibles, coinsurance, or copayments.
How to check whether a specific treatment is covered
When a clinician recommends a test, procedure, therapy, device, or medication, use a repeatable verification process rather than relying on a category label.
- Get the exact service description. Ask the clinician for the procedure or billing code, diagnosis code when relevant, drug name and strength, device description, and place of service.
- Read the plan’s benefit section. Search the Evidence of Coverage and Summary of Benefits and Coverage for the service category, exclusions, limits, and cost-sharing language.
- Check authorization requirements. Ask whether prior authorization, a referral, step therapy, a specific supplier, or a center-of-excellence rule applies.
- Confirm network participation. Verify every provider and facility involved, including anesthesiologists, laboratories, imaging centers, and pharmacies where applicable.
- Call the insurer. Ask for a reference number, the representative’s name, the allowed amount, your estimated deductible and coinsurance, and whether the answer is a benefit determination or only an estimate.
- Ask the provider to verify billing. Give the provider the insurer’s information and ask how the service will be coded. Keep written estimates and call records.
- Appeal a denial when appropriate. Request the denial reason and appeal instructions in the plan documents. A denial can mean the service was not authorized, was considered not medically necessary, was out of network, or was excluded; those are different issues with different remedies.
Common questions readers ask
Do essential health benefits have to be free?
No. Essential means the category must be included, not that members owe nothing. Deductibles, copayments, and coinsurance can apply unless a separate rule—such as qualifying preventive-care cost sharing—removes them.
Are children’s dental and vision benefits the same as adult benefits?
No. Pediatric oral and vision care are included in the pediatric essential-benefit category. Adult dental and vision are optional and depend on the plan or a separate policy.

What if a treatment is not named in the 10 categories?
Do not assume it is excluded or covered. Plans can include additional benefits, and a treatment may be classified under a broader category. Check the plan documents and ask the insurer using the exact service and diagnosis information.
Recommended Free Tools
Does reaching the out-of-pocket maximum make every medical bill free?
No. The maximum applies to covered services under the plan’s rules. Premiums and bills for excluded or otherwise non-covered care can remain your responsibility.
Can an insurer charge more because I have a pre-existing condition?
Marketplace rules described by HealthCare.gov cover pre-existing-condition treatment when coverage begins and do not use health status to set Marketplace premiums. Different policy types, especially grandfathered plans, can have different protections.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Additional checks before care
For planned care, verify the network at every level, not just the name on the insurance card. A hospital, surgeon, anesthesiologist, laboratory, imaging center, therapist, and pharmacy can participate under different arrangements. Ask the insurer whether each service will process as in network for the exact policy and date of care. Provider directories can change, so confirm close to the appointment.
Ask for the plan’s allowed amount and your estimated responsibility rather than relying only on a provider’s list price. The same covered service can produce different bills depending on whether the deductible has been met, whether coinsurance applies, and whether a separate facility fee is billed. Keep the reference number from every insurer call, copies of estimates, and the final explanation of benefits so you can compare what was promised with what was processed.

For prescriptions, check the formulary before the prescription is filled. A drug can be covered only after trying another medication, obtaining prior authorization, using a specialty pharmacy, or observing a quantity limit. If a clinician changes the drug or dose, check again because formulary placement and requirements can differ by product.

When coverage is denied, identify the reason before deciding what to do next. An administrative issue such as missing authorization is different from an exclusion, an out-of-network claim, or a medical-necessity decision. The plan’s denial notice should explain appeal rights and deadlines; follow those instructions and include supporting clinical information when appropriate.
Bottom line
For a Marketplace plan, start with the 10 essential health benefit categories, then move immediately to the plan-specific details. The real answer to “What does my insurance cover?” is the combination of the benefit language, medical-necessity and authorization rules, network, formulary, state requirements, and your cost-sharing terms. Compare likely annual premium plus care costs, verify preventive-service conditions, and call the insurer before a significant treatment.
Frequently Asked Questions
Are essential health benefits automatically free?
No. The categories must be included, but covered care can still be subject to a deductible, copayment, or coinsurance. Specified preventive services are generally available without cost sharing when delivered in network and when the plan’s criteria are met.

Not as part of the essential health benefit requirement. Pediatric oral and vision care are included in the pediatric category, while adult dental and vision benefits are optional and depend on the plan or a separate policy.
How can I verify a procedure before scheduling it?
Get the exact procedure and diagnosis information, read the plan’s benefit documents, check authorization and network rules, and call the insurer. Ask for the allowed amount, estimated cost sharing, and a reference number; also confirm how the provider will code the service.
Will the out-of-pocket maximum cover an excluded treatment?
No. The maximum limits spending for covered services under the plan’s rules. Premiums and charges for excluded, non-covered, or otherwise unprotected care can remain your responsibility.
Are pre-existing conditions covered when a Marketplace plan starts?
Marketplace plans cover treatment for pre-existing conditions beginning when coverage starts, and health status does not determine the Marketplace premium. Grandfathered plans may not include some ACA rights and protections, so check the policy type.
Why did a preventive service result in a bill?
The provider or facility may have been out of network, the service may have been coded as diagnostic, additional treatment may have been provided, or the plan’s age, timing, frequency, or other criteria may not have been met. Ask the insurer how the claim was processed.
The Bottom Line
Bottom line: A health plan’s category list is only the starting point. Your actual coverage and cost depend on the specific service, diagnosis, network, authorization rules, formulary, state requirements, and cost-sharing terms.
